Eye inflammation in psoriatic arthritis.

In a study of 112 patients (49 men, 63 women) with psoriatic arthritis, ocular inflammation was noted in 35 (31.2%). Conjunctivitis was the most common lesion, being found in 19.6% (10 males, 12 females). Iritis occurred in 7.1% (5 men, 3 women), episcleritis in 1.8% (1 man, 1 woman), and keratoconjunctivitis sicca in 2.7% (3 women). 20 patients (10 men, 10 women) had radiological sacroiliitis and 11 of these (7 men, 4 women) had ankylosing spondylitis according to the New York criteria. Eye lesions were noted in 7 of the patients with sacroiliitis, 3 having iritis (15%) and 4 (20%) conjunctiv…

Clinical statistics of endogenous uveitis: comparison between general eye clinic and university hospital.

We compared uveitis patients who attended a general eye clinic (n = 183) with those who attended the ophthalmology department of a university hospital (n = 550) to examine factors that affect the clinical statistics of uveitis outpatients. We observed that diabetic iritis and herpetic iritis were significantly more frequent in the clinic whereas Vogt-Koyanagi-Harada disease and Behcet's disease were significantly more common in the university hospital. Among the so-called three leading uveitis, Behcet's disease and Vogt-Koyanagi-Harada disease were relatively rare in the general clinic; they m…

Antibodies to measles virus in ankylosing spondylitis.

IgG antibodies to measles virus were measured by a solid-phase radio-immunoassay in serum specimens from 31 patients with confirmed ankylosing spondylitis (AS), from 8 patients with symptoms and signs resembling AS and from 39 patients hospitalized for various non-rheumatological disorders. The patients with AS and without iritis/uveitis had a 4-fold increased amount of measles antibodies in their serum specimens compared with the control patients (p less than 0.01). The AS patients with iritis and the patients with symptoms and signs resembling AS were, in this respect, similar to the control…

[Ocular complications in patients with human immunodeficiency virus infection].

PURPOSE: To evaluate the recent clinical manifestations of ocular complications in patients with human immunodeficiency virus(HIV) infection. METHODS: We retrospectively reviewed the medical records of 94 new HIV-positive patients examined at the International Medical Center of Japan between April 1, 2002, and March 31, 2003, and listed the details of ocular complications. The use of highly active antiretroviral therapy (HAART) and CD 4 cell counts was also recorded in patients with cytomegalovirus (CMV) retinitis and iritis/vitritis. RESULTS: The following ocular complications were recogniz…

Cogan's syndrome: a treatable cause of neurosensory deafness.

Cogan's syndrome, nonsyphilitic interstitial keratitis associated with vestibuloauditory deficits, can vary in its presentation. This paper describes an unusual case in which deafness developed while the patient was receiving topical therapy with prednisolone and homatropine for mild bilateral iritis and right-sided nodular episcleritis. The 10-year-old boy's hearing improved dramatically with high-dose systemic steroid therapy. The importance of being aware that deafness may occur in children undergoing treatment for "routine" iritis is emphasized.

Ocular jellyfish stings.

BACKGROUND: Corneal stings from the sea nettle (Chrysaora quinquecirrha) indigenous to the Chesapeake Bay are usually painful but self-limited injuries, with resolution in 24 to 48 hours. METHODS: Five patients who developed unusually severe and prolonged iritis and intraocular pressure elevation after receiving corneal sea nettle stings were followed for 2 to 4 years. RESULTS: Decreased visual acuity, iritis, and increased intraocular pressure (32 to 48 mmHg) were noted in all cases. Iritis responded to topical corticosteroids and resolved within 8 weeks. Elevated intraocular pressure respo…

Serum antibody levels of uveitis patients to bovine retinal antigens.

Serum samples from 91 uveitis patients and controls were tested by the enzyme-linked immunosorbent assay technique to determine their relative antibody titers to bovine retinal S antigen and to a detergent extract of saline-washed bovine retina, or P antigen. Only those patients whose uveitis fell into the categories of toxoplasmosis, iritis, herpes keratouveitis, pars planitis, sarcoidosis. Behçet's syndrome, and Vogt-Koyamagi-Harada syndrome are presented in this study. Significantly elevated titers (P less than 0.05) of antibody to S antigen were found in the diagnostic categories of iritis…

Tonic pupil in leprosy.

Pupil abnormalities in leprosy usually result from chronic iritis with loss of stroma, iris miosis, a sluggish reaction to light, and poor dilation in response to anticholinergic mydriatics. We report two patients with long-standing lepromatous leprosy who developed tonic pupils characterized by mydriasis, absence of reaction to light and hypersensitivity to weak cholinergic solution. Examination revealed iritis and iris atrophy. In both cases, instillation of dilute 0.1% pilocarpine caused miosis in the affected eyes. Tonic pupil occurs in many conditions, but its association with leprosy had…

Onchocerciasis in Sierra Leone 3: Relationships between eye lesions and microfilarial prevalence and intensity.

The relationship between severe onchocercal eye lesions (iritis, sclerosing keratitis, optic atrophy and choroidoretinitis) and (i) the prevalence and intensity of microfilariae (mf) of Onchocerca volvulus in skin snips from the iliac crest and outer canthus, and (ii) the prevalence of mf in the cornea and anterior chamber of the eye, was studied in 1414 persons from forest and savanna villages and 312 attenders at eye clinics. Ecologically the savanna of Sierra Leone more closely resembles the forest than the dry Sudan-savanna areas of West Africa, and in persons aged 30 years or more the com…

Ocular manifestations of Mycoplasma pneumoniae infection.

Ocular manifestations of Mycoplasma pneumoniae infection, other than conjunctivitis, are uncommon. Optic disk swelling, optic nerve atrophy, retinal exudates and hemorrhages, and cranial nerve palsies have been infrequently reported. We describe a 15-year-old patient who developed bilateral optic disk edema and iritis during an acute infection with M. pneumoniae and review the world literature on findings associated with ocular manifestations of infection with this pathogen. Although our patient experienced complete resolution of iritis and optic disk edema after 6 weeks, several patients desc…

[Nonspecific eye autoantibodies in uveitis].

Under certain pathologic circumstances autoantigens lead to the formation of autoantibodies. In uveitis, autoimmunologic phenomena will also be discussed. We studied 149 sera of uveitis patients for various organ specific and non-organ specific autoantibodies. We found antisarcolemal autoantibodies (ASA) mainly in patients with acute iritis (59% positive) but also in panuveitis (40%) and iridocyclitis (29%). On the other hand antiendothelial antibodies (AEA) could be found in 43% of patients with chorioretinitis. Antisinusoidal antibodies (SA) have been detected in iritis and panuveitis patien…

Effect of ketorolac on herpes simplex virus type one ocular infection in rabbits.

Corticosteroids can exacerbate viral ocular infections. Ketorolac tromethamine is an effective nonsteroidal anti-inflammatory agent that may be a useful substitute for corticosteroids following ocular surgery. In this study, rabbits ocularly infected with herpes simplex virus type 1 (HSV-1) were treated topically four times daily with 0.5 percent ketorolac or 0.1 percent dexamethasone for 7 days after infection. Severity of the infection was determined by scoring corneal opacity and HSV-1 corneal ulcerations with the Draize scale as well as iritis and conjunctivitis. Ten days after treatment e…

Resolution of Pigmented Keratic Precipitates Following Treatment of Pseudophakic Endophthalmitis Caused by Propionibacterium Acnes.

Propionibacterium acnes is a well-recognized cause of chronic postoperative endophthalmitis after cataract surgery. The subtle clinical signs of this infection and its initial favorable response to topical steroids may lead to delayed diagnosis and treatment. Two patients with culture proven P. acnes endophthalmitis after cataract surgery that presented with prominent pigmented keratic precipitates (KPs) and low-grade iritis in the involved eye were managed. Both cases had initial treatment with diagnostic pars plana vitrectomy (PPV) with intraocular antibiotic (IOAB) injection. Secondary trea…

Chorioretinal scars in Fuchs' heterochromic iridocyclitis.

To determine the incidence of chorioretinal lesions in patients with Fuchs' heterochromic iridocyclitis, we reviewed the records of all patients with this disease seen at the Indiana University Uveitis Service, Indianapolis, since 1963. For comparison the records of age- and sex-matched patients with HLA-B27-positive iritis were also reviewed. The incidence of chorioretinal lesions in one or both eyes of patients with Fuchs' heterochromic iridocyclitis was significantly higher than in the patients with HLA-B27-positive iritis or than expected in the general population. Among the types of lesio…

Comparison of osteitis condensans ilii and ankylosing spondylitis in female patients: clinical, radiological and HLA typing characteristics.

A study comparing 12 patients with ankylosing spondylitis (AS) to 25 with osteitis condensans ilii (OCI), referred to a rheumatic disease center, was carried out to determine whether OCI represents a varient of AS in women. In the group with OCI, chronic lumbodorsal pain was present in 9, 36%, a 'fibrositis' syndrome in 6, 24%, and 16, 64% had recurrent episodes of polyarthralgia. A definite arthritis with effusion developed in 10 patients, 40%. No patient with OCI had iritis or colitis, whereas 4 patients with AS had iritis and four had colitis. Radiographs of the spine showed no evidence of …

Antinuclear antibody studies in juvenile chronic arthritis.

This study of children with chronic arthritis confirms that patients at greatest risk for chronic iridocyclitis have a young age of onset and positive antinuclear antibodies (ANA). Children who are ANA negative have a low risk of iridocyclitis. When it occurs it is at a significantly older age. No child developing juvenile chronic arthritis over 9, even with positive ANA, had chronic iritis. There was remarkable variation in ANA titres; these correlated with erythrocyte sedimentation rate, active arthritis, and iridocyclitis. Maximum ANA titres were often seen very early in the disease but at …
