Immunopathogenesis of antibody-mediated rejection in liver transplantation.

Antibody-mediated rejection (AMR) after liver transplantation (LT) is increasingly recognized as a cause of graft dysfunction, chronic injury, and graft loss, but remains difficult to diagnose because serologic, histologic, and clinical findings are often incomplete or overlapping. This review focuses specifically on liver AMR and distinguishes liver-specific evidence from mechanisms inferred from experimental models or other solid-organ transplants. We summarize how donor-specific antibodies (DSA), complement activation, Fc receptor-mediated effector mechanisms, endothelial activation, and mi

International Society for Heart and Lung Transplantation Scientific Statement on pulmonary antibody-mediated rejection and proposed graft, antibody, and pathology (GAP) definition.

Antibody-mediated rejection (AMR) is an increasingly recognized form of rejection and cause of graft failure after lung transplantation. AMR has been the focus of extensive research over the past decade. Despite growing awareness and recent advances in our understanding of AMR, outcomes remain dismal with a 2-year survival of only 20%. The International Society for Heart and Lung Transplantation convened a multidisciplinary workgroup of experts in AMR to review the most up-to-date research and clinical experience and to update the 2016 definition. The workgroup was divided into 9 subgroups cov

Anti-Ce in haemolytic disease of the foetus and newborn.

OBJECTIVES: To report a delayed case of severe haemolytic disease of the foetus and newborn (HDFN) due to Anti-Ce. BACKGROUND: HDFN due to maternal antibodies is potentially fatal. As a result, antibody levels and foetal anaemia are periodically monitored and risk assessed throughout pregnancy. HDFN due to Anti-Ce is rare. CASE REPORT: A 29-year-old Caucasian female with low titre Anti-Ce and Anti-e antenatally delivered a term baby girl that required multiple transfusions and hospital admissions early in life. The apparent clinical severity of HDFN resulted in investigative testing of a mat

Updated management for antibody-mediated rejection: opportunity to prolong kidney allograft survival.

PURPOSE OF REVIEW: Antibody-mediated rejection (ABMR) is an important barrier to achieve long-term kidney allograft survival. Human leukocyte antibody (HLA)-incompatibility and ABO-incompatibility are the two main mechanisms of ABMR. Nevertheless, the advances in managing ABMR have changed the paradigm for kidney transplantation. This review aimed to emphasize the HLA-incompatibility and ABO-incompatibility kidney transplant and update the management of ABMR. RECENT FINDINGS: HLA-incompatibility kidney transplantation is a strong risk factor for ABMR. Donor-specific antibody (DSA) is a surrog

Harnessing the B Cell Response in Kidney Transplantation - Current State and Future Directions.

Despite dramatic improvement in kidney transplantation outcomes over the last decades due to advent of modern immunosuppressive agents, long-term outcomes remain poor. Antibody-mediated rejection (ABMR), a B cell driven process, accounts for the majority of chronic graft failures. There are currently no FDA-approved regimens for ABMR; however, several clinical trials are currently on-going. In this review, we present current mechanisms of B cell response in kidney transplantation, the clinical impact of sensitization and ABMR, the B cell response under current immunosuppressive regimens, and o

Development and validation of a universal blood donor genotyping platform: a multinational prospective study.

Each year, blood transfusions save millions of lives. However, under current blood-matching practices, sensitization to non-self-antigens is an unavoidable adverse side effect of transfusion. We describe a universal donor typing platform that could be adopted by blood services worldwide to facilitate a universal extended blood-matching policy and reduce sensitization rates. This DNA-based test is capable of simultaneously typing most clinically relevant red blood cell (RBC), human platelet (HPA), and human leukocyte (HLA) antigens. Validation was performed, using samples from 7927 European, 27

Evidence-based platelet transfusion guidelines.

Transfused platelets (plts) are either pooled random-donor platelet (plt) concentrates or single-donor apheresis plts. When stored for 5 days, all of these products are equally efficacious. A 10,000/microL prophylactic plt transfusion trigger has been documented to be both hemostatically efficacious and cost effective in reducing plt transfusion requirements. The optimal plt dose/transfusion is being evaluated in an ongoing clinical trial. Therapeutic plt transfusions to control or prevent bleeding with trauma or surgical procedures require higher transfusion triggers of 100,000/microL for neu

Detection and clinical relevance of antibodies after transplantation.

Until recently, the role of antibodies in graft failure has been hampered by poor methods of defining specificity. Development of solid phase assays using purified major histocompatibility complex (MHC) molecules has greatly advanced our ability to monitor anti-human leukocyte antigen (HLA) antibodies in patients and to distinguish between HLA and non-HLA antibodies. The purpose of this chapter is to describe the methods for detecting antibodies and what we have learned in recent years regarding the role of well-defined antibodies to HLA and non-HLA antigens. Use of the complement-dependent ly

Differential immunogenicity of paternal HLA Class I antigens in pregnant women.

More insight into the differential immunogenicity of human leukocyte antigen (HLA) mismatches will be beneficial for donor selection in clinical transplantation. In this study the immunogenicity of HLA antigens was analyzed by examining the antibody profiles in women who have been pregnant. In total 888 women, who had pregnancy induced HLA alloantibodies, were included in this study, while 413 women who had not been immunized by their pregnancy, served as controls. First it was analyzed whether women expressing particular HLA antigens are more likely to produce HLA alloantibodies. Next we dete

Dynamic nature of maternal multiple RBC alloantibodies after vaccination: Case report and immunological mechanisms review.

Red blood cell (RBC) alloimmunization in pregnancy remains a major cause of hemolytic disease of the fetus and newborn (HDFN). Multiple maternal alloantibodies are uncommon and present significant diagnostic, and transfusion challenges. We report a case of a 35-year-old gravida, with anti-D, anti-C, anti-Fya, and anti-M alloantibodies detected at 10 weeks gestation. Initial titers of anti-D and anti-C were low and stable under serial monitoring. Non-invasive fetal genotyping confirmed an RHD-positive fetus. Following routine maternal vaccination against influenza (28 weeks), pertussis (30 week

Rare Autoantibody Mimics Anti-C and Anti-e Specificity in Patient with Aplastic Anemia.

BACKGROUND: Alloantibodies for the Rh blood group are the most immunogenic antibodies found in the Chinese population, typically causing acute or delayed hemolytic transfusion reactions and fetal and neonatal hemolytic diseases. Autoantibodies are generally considered nonspecific, and approximately 50% of warm antibodies are secondary to a variety of diseases, especially hematologic tumors. In this case report, a rare autoantibody that mimics anti-C and anti-e specificity was identified. METHODS: A 17-year-old adolescent with aplastic anemia was awaiting transfusion due to anemia. Predicted S and s phenotypes from genotyping results among Thai populations to prevent transfusion-induced alloimmunization risks.

BACKGROUND: S and s antigens of the MNS system are of clinical importance because alloanti-S and -s have usually caused delayed hemolytic transfusion reactions and hemolytic disease of the fetus and newborn. Various red cell genotyping has been established to predict the phenotypes to solve serological test limitations. OBJECTIVES AND METHODS: This study aimed to determine S and s genotype frequencies and to estimate the alloimmunization risks among central, northern and southern Thai populations. Altogether, 1237 blood samples from Thai blood donors were included. Only 150 samples were teste

Anti-A and anti-B: what are they and where do they come from?

Intravenous immunoglobulin (IVIG) is made from thousands of donors having a variety of blood groups. All of the donors being used for IVIG production, with the exception of group AB donors, have in their plasma antibodies of variable titer commonly known as isohemagglutinins or ABO antibodies. As blood groups O and A are the most commonly found in the world population, most of the plasma used in IVIG production is from donors having these blood groups, with group B and group AB donors being fewer in number. Consequently, all batches of IVIG contain antibodies that are reactive with individuals

Red cell alloantibodies in Lao blood donors.

Data of the prevalence of red cell alloantibodies in blood donors will assist in improving safe blood supply for patients needing transfusion. The purpose of this study was to determine prevalence of red cell alloantibodies in Lao blood donors. Antibody screening and identification by multiple phase indirect antiglobulin standard tube assay were performed in serum samples from 1,181 blood donor individuals following routine work at the Lao Red Cross National Blood Transfusion Center. The overall prevalence of alloantibodies was 3.90%. All of the antibodies found were naturally occurring antibo

[Overview and methods for determination of antiplatelet antibodies and thrombocyte antigens].

The last twenty years have been characterized by great interest in the study of the role of antiplatelet antibodies and platelet antigens in the mechanism of thrombocytopenia. The use of numerous serologic methods for the determination of antiplatelet antibodies has contributed to the better understanding and differential diagnosis of immunologically induced thrombocytopenias. Development of the methods of molecular biology has allowed for a more accurate determination of platelet antigens and assessment of the prevalence of particular antigens in the population. These concepts have improved t

Affinity of human anti-factor VIII antibodies for functional polystyrene supports.

Human anti-factor VIII antibodies (anti-FVIII) neutralize Factor VIII (FVIII) procoagulant activity. These antibodies appear in about 5-15 per cent of severely affected patients with haemophilia A treated with FVIII concentrates (Mannucci, 1993). In order to obtain non-thrombogenic materials able to interact specifically with anti-FVIII, amino acids residues that mimic part of the FVIII molecule recognized by anti-FVIII have been grafted. Several cross-linked polystyrenes were functionalized with sulphonate and tyrosine sulphamide groups or tyrosine derivatives sulphamide groups such as methyl

Flow cytometric analysis of anti-platelet antibodies in idiopathic thrombocytopenic purpura.

Anti-platelet antibody measurement may be important in defining the pathogenesis of thrombocytopenic states. In this paper we compared three anti-human immunoglobulin reagents by using them to detect anti-platelet antibodies on the platelet surface and in the serum of 14 patients with chronic idiopathic thrombocytopenic purpura (ITP) and 22 thrombocytopenic disorders. Samples were analyzed by both flow cytometry and a fluorescence microscope. In ITP patients, the direct test was positive in 50% of the cases, while the indirect technique proved to be positive in a slightly higher number of thos

An adaptive quantification scheme for profiling serum antibody level in transplant immunology.

Luminex single antigen bead (SAB) assays are widely used in transplantation medicine for detecting HLA-specific antibodies, but the relative nature of the assay signal complicates interpretation and comparability. Also, applying fixed cutoffs to SAB results is problematic, as mean fluorescence intensity (MFI) values are inherently relative, subject to drift, and influenced by both technical and biological variation. Yet, reporting and comparing MFI values to other MFI or fixed thresholds remain widespread in clinical practice, often leading to inconsistent or misleading assessment of the immun
