Management of an unusual extreme extension contracture of the wrist: role of a custom-designed exercise program in achieving a good range of movement and prevention of recontraction.

An extreme extension contracture of wrist with dorsal contracture of fingers 15 years after burn injury is described. Shortening of extensor tendons, secondary lengthening of flexor tendons, contracted wrist joint capsule, unusual dislocation of carpals, dorsal dislocation of metacarpophalangeal joints of fingers, and provision of sufficient amount of good-quality skin were some of the issues that had to be addressed in treatment. The contracture was released, the carpals and metacarpophalangeal joint dislocations were corrected and fixed with K wires, and the resulting defect was covered with

Total knee arthroplasty ligament balancing and gap kinematics with posterior cruciate ligament retention and sacrifice.

This cadaver study was undertaken to gain insight into the effects that posterior cruciate ligament retention and sacrifice would have on the amount of deformity correction obtained with medial and lateral structure release during total knee arthroplasty. Twenty-seven cadaveric specimens were used to sequentially release medial and lateral structures with and without posterior cruciate support. Each release sequence was tested in full extension and 90 degrees flexion. In full extension, the resulting change into valgus after release of the posterior cruciate ligament, posteromedial capsule/obl

Anterior transolecranon dislocation of the elbow in a child: A case report and review of literature.

Anterior transolecranon dislocation of the elbow is rarely observed in children, reported in only a small series. The present case involves an anterior transolecranon dislocation of the left elbow joint in a 7-year-old child, which was surgically treated. Two attempts of closed reduction failed because the radial head had buttonholed via the joint capsule. After its release, open reduction was easily performed; osteosynthesis of the olecranon was not performed. Remarkably, good result was obtained, despite a mild flexion deformity at the last follow-up. This case report aims to highlight this

Shoulder hydrodilatation for primary, post-traumatic and post-operative adhesive capsulitis.

BACKGROUND: Adhesive capsulitis (frozen shoulder) is characterised by pain and loss of range of motion of the glenohumeral joint. It can be present as primary (idiopathic) or secondary to surgery, trauma or other conditions that restrict the use of the shoulder joint. Various treatment options have been adopted including physiotherapy, manipulation under anaesthetic, hydrodilatation and arthroscopic or open capsular release but the optimal form of management remains uncertain. OBJECTIVES: The purpose of the study was to assess the clinical outcome of glenohumeral hydrodilatation in three coho

The posterior talofibular ligament: an anatomical study with clinical implication in clubfoot surgery.

BACKGROUND: The role of the posterior talofibular ligament (PTFL) in the clubfoot deformity remains unclear. We conducted an anatomical study to precise its topography and role in maintaining tibiotalar equinus in patients with clubfoot deformity. METHODS: Ten ankles were dissected using a wide posterior exposure. The PTFL was identified at the posterior aspect of the ankle and its relations with other anatomical structures were noted. RESULTS: After opening of the tibiotalar and subtalar joints, the superior aspect of the PTFL was clearly seen, running horizontally from the fibula to a prom

Thermo-Sensitive Dual-Functional Nanospheres with Enhanced Lubrication and Drug Delivery for the Treatment of Osteoarthritis.

Osteoarthritis is a typical degenerative joint disease related to a lubrication deficiency of articular cartilage, which is characterized by increased friction at the joint surface and severe inflammation of the joint capsule. Consequently, therapies combining lubrication restoration and drug intervention are regarded as a promising strategy for the treatment of osteoarthritis. In the present study, thermo-sensitive dual-functional nanospheres, poly[N-isopropylacrylamide-2-methacryloyloxyethyl phosphorylcholine] (PNIPAM-PMPC), are developed through emulsion polymerization. The PNIPAM-PMPC nano

Anatomic proximity of the peroneal nerve to the posterolateral corner of the knee determined by MR imaging.

BACKGROUND: The pie crusting technique has been extensively used to release the lateral soft tissue in total knee arthroplasty. However, it may place the peroneal nerve at direct injury risk when performed in a valgus knee. The aim of this study was to determine the anatomic proximity of the peroneal nerve to the posterolateral corner of the knee. METHODS: One hundred knees were measured on axial MR images for the proximity of peroneal nerve to the closest edge of the inner surface of joint capsule or the posterolateral corner of proximal tibia at the level of the joint line and the level of

A Modified Arthroscopic Outside-in Shoulder Release Approach for Severe Shoulder Stiffness.

OBJECTIVE: Arthroscopic release is effective for patients with shoulder stiffness, but the traditional inside-out procedure cannot effectively alleviate the mobility of some severe stiff shoulder and even cause itrogenic injuries sometimes. The aim of this study is to evaluate the clinical efficacy and advantages of a modified outside-in shoulder release approach for severe shoulder stiffness. METHODS: Included in this retrospective study were 15 patients (five male and 10 female) with severe shoulder stiffness who underwent modified outside-in shoulder release surgery at our hospital between

Lateral soft-tissue release through a medial incision: Anatomic comparison of two techniques.

BACKGROUND: The distal soft tissue procedure is an integral part of hallux valgus surgery, providing soft tissue balance and alignment restoration of the first metatarsophalangeal joint. Various approaches have been established to this end. For techniques that do not include a separate dorsal incision, lateral release may be achieved via a transarticular approach or via a medial incision and a dorsal flap over the first metatarsal. Compared to the double-incision technique, these techniques are not only cosmetically superior and thus meet the demands of most surgeons and patients. MATERIAL AN

Open posterior capsular release with an osteotome in total knee arthroplasty does not place important neurovascular structures at risk.

PURPOSE: Posterior capsular contracture is a potential consequence of osteoarthritis, post-traumatic arthritis, and surgical procedures of the knee. Many patients who undergo TKA will be found to have some degree of flexion contracture intraoperatively, which necessitates posterior capsular release. There is no information in the literature about the safety of posterior capsular release done during TKA. The present cadaveric study investigates the safety of posterior capsular release during TKA. METHODS: This study involved ten fresh-frozen cadaver specimens, each of which underwent three suc

Arthroscopic First Metatarsophalangeal Arthrodesis for Repair of Fixed Hallux Varus Deformity.

Arthroscopic first metatarsophalangeal arthrodesis for fixed hallux varus deformity can be very difficult because narrowing of the medial joint space results in difficult access. The abductor hallucis tendon and the medial capsule can be released through a small proximal plantar medial incision. This will convert the deformity into a flexible one and open up the medial joint space. This allows arthroscopic arthrodesis using the standard dorsolateral and medial portals. The plantar medial incision can also be used for arthroscopy of the metatarsosesamoid compartment and insertion of a screw for

Early Anti-Inflammatory Effect of Bupivacaine-Meloxicam Therapy in a Rabbit Model of Arthrofibrosis.

Arthrofibrosis is a common complication following total knee arthroplasty and is the result of a dysregulated immuno-inflammatory cascade, which culminates in excessive scar deposition by myofibroblasts within the knee joint. Pharmacotherapies for arthrofibrosis are limited, with treatment reliant on manipulation under anesthesia or lysis of adhesions with or without component revision. Bupivacaine (a local anesthetic) and meloxicam (a nonsteroidal anti-inflammatory drug) combination therapy may exert antifibrotic properties by limiting inflammatory cell recruitment and the subsequent release

Arthroscopic treatment of the hip in early and midstage degenerative joint disease.

The purpose of this study was to present the technique of arthroscopic capsule release in patients with early and midstage degenerative joint disease of the hip. In 22 patients we performed an arthroscopic capsular release of the hip joint capsule with simultaneous synovectomy and percutaneous drilling of areas of bone edema. The age of the patient ranged from 28 to 65 years (mean 52 years). There were 14 male and 8 female patients. All patients had suffered from a significant reduction of quality of life. In 15 of the 22 patients hip arthroplasty was already indicated in another institution.

Congenital Unilateral Hypertrophy of the Foot Intrinsics: A Rare Case and Review of Literature.

INTRODUCTION: Diffuse skeletal idiopathic hyperostosis is a non-inflammatory systemic skeletal condition in which there is ossification of ligaments, tendons, and joint capsule. Although the radiological changes and clinical manifestation of diffuse idiopathic skeletal hyperostosis (DISH) in the spine have been well defined in the literature, the changes in the knee and their implications on knee replacement are unclear. CASE REPORT: A 60 year -year-old patient presented with pain, stiffness, and decreased arc of movement at the right knee. The X-rays showed ossification of the joint capsule,

Patellar instability with recurrent dislocation due to patellofemoral dysplasia. Results after surgical treatment.

Twenty-one patients (25 knees) with recurrent dislocation of the patella due to patellofemoral dysplasia were treated with a combined distal and proximal realignment of the extensor mechanism of the knee. In addition to traditional medial transfer of the tibial tuberosity, lateral release, and medial joint capsule duplication, vastus medialis transposition and deepening of the intercondylar sulcus on the proximal joint surface of the femur were done. A follow-up ranging from 1-14 years (mean 6 years) found the results excellent or good in 22 knees, fair or poor in three. One redislocation occu

Adhesive capsulitis of the hip: a review.

Adhesive capsulitis of the hip (ACH) is a rare clinical entity. Similar to adhesive capsulitis of the shoulder, ACH is characterized by a painful decrease in active and passive range of motion as synovial inflammation in the acute stages of the disease progresses to capsular fibrosis in the chronic stages. Once other diagnoses have been ruled out, management of ACH is tailored to reduce inflammation in the acute stages with NSAIDs, intra-articular steroid injections, and targeted physical therapy while biomechanical dysfunction in the spine, hip, sacroiliac joint, or lower limb joints is addre

Arthroscopy of the symptomatic shoulder arthroplasty.

BACKGROUND: Assessment of a painful or stiff shoulder arthroplasty can be challenging. The cause of pain can sometimes be easily identified. However, some patients have normal levels of inflammatory markers, normal plain films, and no clinical signs to indicate a diagnosis. Indolent organisms may not raise blood marker levels or result in obvious radiologic findings such as loosening. We report the utility of performing arthroscopy in these patients for a diagnostic advantage. METHODS: We retrospectively reviewed the health records of all patients who underwent diagnostic shoulder arthroscopy
