Instability of the patellofemoral joint in Rubinstein-Taybi syndrome.

Twenty-five (3.4%) of 732 individuals with Rubinstein-Taybi syndrome were noted to have instability of the patellofemoral joint. We believe that this is in fact an underrepresentation of the true incidence, as we were able to identify only those patients whose symptoms were most severe. These individuals typically had symptoms before or during their adolescent growth spurt, had bilateral involvement, and in some instances, ceased to ambulate because of their patellofemoral problems. A subset of these individuals underwent patellar realignment surgery, with those whose treatment did not include

[Treatment of bilateral dysplastic instability of the hip joints].

In the article are described the peculiarities of the treatment tactics and presented an integral assessment of treatment efficiency of 563 patients with bilateral dysplastic instability of the hip joints (bilateral dislocation, bilateral primary subluxation, dislocation of one and subluxation of the other joint), carried out in the hospital of the institute named after M. I. Sitenko in 1955-1989. There is noted a necessity of achievement of anatomo-functional symmetry of the hip joints as a strategic aim of treatment and an expediency of application of "anticipation estimate" principle while

[The value of arthroscopy for diagnosis of instability of the shoulder joint].

As a glenohumeral instability is defined each case of transient recurrent and permanent decentration of the joint. We propose to differentiate primary and secondary glenohumeral instabilities. Arthroscopical diagnostic is helpful to classify the luxation and subluxation and for verify transient instabilities. In cases clinically snapping phenomena were noticed, arthroscopic findings were documented. A relation between those phenomena and arthroscopic findings is discussed.

[Possibilities of sonography in the diagnosis of instabilities of the shoulder joint].

We present ultrasound investigation as a diagnostic tool for shoulder instability. The following structures are visible in the posterior transversal plane: subcutis, deltoid muscle, infraspinatus muscle, infraspinatus tendon, humeral head, scapula, dorsal glenoid rim, dorsal labrum, and the joint capsule. The relation of the humeral head to the glenoid can be judged statically and dynamically. Anterior and posterior instabilities and secondary signs of dislocation such as Hill-Sachs lesion and effusion can be seen. This technique is easy to learn. By comparing the injured with the normal shoul

[Secondary ligament instabilities in the area of the elbow joint].

Secondary capsular or ligamentous injuries around the elbow joint, which are generally a rare finding, can give rise to medial, lateral, anterior, and posterior instability. Medial instability is especially bothersome during sporting activities, such as javelin throwing and baseball, and requires tightening or replacement of the ulnar collateral ligament. On the lateral side it is particularly the subluxation of the head of the radius that makes surgical tightening of the annular ligament essential. Anterior instability, which would cause the joint to dislocate anteriorly, is extremely rare an

Assessing ulnar instability of the metacarpophalangeal joint of the thumb.

Seven hundred and fifty normal thumbs were examined to determine the normal range of radioulnar mobility of the metacarpophalangeal joint with the joint in full extension, 15 degrees of flexion, and full flexion. Full flexion was the position of greatest stability. Twenty-five adult unembalmed cadaver thumbs were examined to determine the optimal position for testing the metacarpophalangeal joint for ulnar collateral ligament stability. When radial stress was applied to the metacarpophalangeal joint of 20 specimens after cutting the adductor aponeurosis and capsule, minimal ulnar instability w

Post-traumatic instability of the metacarpophalangeal joint of the thumb.

Of eithty-six patients with collateral ligament instability of the metacarpophalangeal joint of the thumb, sixty-six (77 per cent) had ulnar and twenty (23 per cent) had radial instability, while in addition twenty-four had fractures and twenty-nine had volar subluxation of the proximal phalanx. Of the sixty-nine patients operated on, sixty-five had an abnormality of the ulnar or radial collateral ligament proper or of its attachment; two, subluxation of the extensor pollicis longus; and two, stretching of the adductor expansion. Stability was restored by collateral ligament repair or reconstr

Chronic ankle and subtalar joint instability in the athlete.

Chronic ankle and subtalar joint (STJ) instability is a common clinical entity and the physician must be able to determine the exact pathologic condition for proper treatment. There are many diagnostic techniques that can be used to evaluate the ankle joint and STJ. These diagnostic techniques do not take the place of a proper patient history taking. Appropriate and aggressive rehabilitation should be attempted in all cases of chronic ankle and subtalar instability before electing surgical reconstruction.

[Surgical treatment of anterior instability of the shoulder joint].

In the article are presented data on the operative treatment of 52 patients (54 operations) with anterior instability of the arm joints. Patients have been distributed into 4 groups depending upon the character of intra-articular damages, revealed in the process of arthrotomy. Each group of patients was subjected to adequate operative intervention, based upon Bankart's method. The authors proceeded from the principle that the arm joint instability treatment should be aimed at removal of intra-articular damages. There have been restored integrity of the damaged cartilage cushion and the joint c

[The value of ultrasound in evaluating instabilities of the glenohumeral joint].

In a prospective study we evaluated diagnostic ultrasound investigation for shoulder instabilities. We measured the passive aptranslation In 150 healthy persons in relation to the dorsal glenoid as well as the inferior subluxation in relation to the acromion. After establishing normative datas we measured 23 patient with unidirectional instabilities and 34 with multidirectional instabilities. The humeral head position of a normal joint is 8 to 10 mm dorsally to the glenoid. In the control group the anterior translation of the dominant shoulder is significantly higher as in the nondominant shou

[Clinical picture of anterior instability of the shoulder joint].

The authors have studied the clinical picture of 360 patients with primary traumatic dislocations and the long-term results have been observed in 286 patients. The analysis of 50 arthrotomies of the shoulder performed in cases of anterior instability has been made. A direct relationship between the clinical course of instability and the severity of the intra-articular lesions appearing during the first traumatic dislocation has been established. Such lesions include ruptures or avulsions of the anterior portion of the cartilage wall, ruptures of the joint capsule, avulsion of the capsule from

Evans' repair of lateral instability of the ankle joint.

A follow-up study is presented of 26 ankle joints with recurrent lateral instability, treated with surgical repair by the Evans method. The median follow-up period was 3.1 (range 1.1-6.9) years. Normal stability was achieved in 73 per cent of the ankles, and another 12 per cent were improved. Similar results are obtained with other methods of surgical repair. Preoperatively 82 per cent of the joints had a positive anterior drawer symptom. This was only found in 12 per cent postoperatively, and these ankles were all classified as having poor results, indicating the importance of reconstruction

The classification and early diagnosis of knee joint instability.

A working classification of knee joint instability includes anatomic and pathologic classifications. The anatomic classification defines the direction of the instability causing the abnormal function to the patient. A structural classification delineates the pathologic lesion. An ability to correlate one with the other ensures proper understanding of the subject. An accurate diagnosis following the acute injury is the basis of effective treatment. Residual chronic disability in many instances will be avoided. Injuries involving the cruciate ligaments constitute difficult diagnostic problems. A

Proximal radioulnar joint, but not posterolateral, instability in patients with symptomatic minor instability of the lateral elbow.

BACKGROUND: The elbow joint is stabilized laterally by a complex of bony and ligamentous structures, such as the lateral ulnar collateral ligament and radial lateral collateral ligament (RLCL). Pathogenesis of many elbow conditions like symptomatic minor instability of the lateral elbow (SMILE) is based on ligamentous incompetency. The aim of this study is to measure ligament elongation in SMILE patients and determine the correlation with the development of this pathologic condition. MATERIALS AND METHODS: Twenty-eight patients were included in this study: half of them were operated for suspe

[The "positioned image" of the talo-calcaneus joint in instability of the posterior distal ankle joint].

With regard to the very common disease of sprained ankles the combined tear with ruptures of ligaments of the subtalar joint are possible. Even an isolated instability is known as the reason of long lasting post-traumatic complaints. Using the apparatus of Scheuba, well-known in diagnosing instabilities of the knee and ankle joint, a new standardized method controlling instabilities of the subtalar talo-calcanear joint is introduced. In an experimental investigation this radiological technique has been tested out in cadaveric ankle joints (nine joints) with artificial ligamentous lesions. The

Lateral instability of the knee joint and disorder of the ankle joint extension disorder in men.

OBJECTIVE: Introducion: The flexion of the knee joints in the movement of the squat may be accompanied by physiological varus deformity that increases smoothly with the lowering of the center of gravity, followed by a return to the starting position. Observing the disturbances of the physiological movement of the knee joints in the frontal plane, the authors correlated them with the mobility of the ankles in the sagittal plane. The aim: To show the relationship of knee joint motion disorders in the frontal plane with the movement of the ankle joints in the sagittal plane in the movement of the

Lesser Metatarsophalangeal Joint Instability: Minimally Invasive Surgery Treatment Alternatives.

Lesser metatarsophalangeal (MTP) joints instability is usually produced by the rupture of the plantar plate. The treatment algorithm may change in patients with advanced stages of the plantar plate rupture, and surgical treatment is proposed to avoid MTP dislocation. Palliative surgery with minimally invasive surgery (MIS) can offer a less aggressive approach and enable excellent clinical correction and a good function. The authors proposed an algorithm for MIS treatment of MTP instability based on a combination of different osseous and/or soft tissues surgical procedures that the authors will
