A case of juxtaglomerular cell tumor with novel histologic features.

Juxtaglomerular cell tumors constitute a very small group among the renal neoplasms. Most secrete renin, causing hypertension and hypokalemia due to secondary hyperaldosteronism. They have characteristic pathological features. For the accurate evaluation, diagnosis has to be confirmed by immunohistochemical staining for renin or electron microscopic demonstration of renin protogranules. This report presents a case of renal juxtaglomerular cell tumor in a 63-year-old man with previously unreported histological findings in addition to classic morphology.

[Histopathological study of a case with glaucoma due to Sturge-Weber syndrome].

PURPOSE: To investigate by histopathology the cause of secondary glaucoma due to Sturge-Weber syndrome. CASE: A 10-year-old boy with Sturge-Weber syndrome and glaucoma in the right eye is reported. Trabeculectomy was performed because of uncontrolled intraocular pressure and the trabeculectomy specimen was examined histologically by both light and electron microscopy. RESULTS: Histological examination of the trabeculectomy specimen showed the ciliary muscle dislocated anteriorly and there was no Schlemm's canal. The spaces in juxtacanalicular connective tissue(JCT) were replaced by vascular

Hypertension secondary to renin-secreting juxtaglomerular cell tumor: case report and review of 38 cases.

A 15-year-old girl with severe high renin hypertension caused by a juxtaglomerular cell tumor (JCT) was successfully treated with the calcium channel blocker nifedipine until surgical removal effected a permanent cure. This case was incorporated into a review of the 37 cases previously published. Comparison of the children and adolescents with the adult population showed that the features of JCT were similar in the two groups except for the average duration of symptoms prior to diagnosis (pediatric group 2.6 years vs. 6.0 years for the adult group). Analysis of all 38 cases demonstrated the fo

Acute muscular paralysis in an adult with subclinical Bartter's syndrome associated with gentamicin administration.

We report an adult case of asymptomatic Bartter's syndrome with the first presentation of hypokalemic paralysis triggered by gentamicin injection. Marked hypokalemia and hypomagnesemia associated with excessive kaliuresis and magnesiuria were found. Plasma renin activity and aldosterone concentration were high, but blood pressure was normal. Renal biopsy revealed hypercellularity of the renin-producing cell of the juxtaglomerular apparatus. Muscular paralysis subsided after potassium chloride supplementation. Hypokalemia was corrected with potassium and magnesium supplements and the use of dic

Surreptitious diuretic ingestion and pseudo-Bartter's syndrome.

A patient with profound hypokalemia satisfied the criteria for Bartter's syndrome, including hyperreninemia, aldosteronism, normal blood pressure, and hyperplasia of the juxtaglomerular apparatus. Two screening tests of urine and one of plasma for diuretic agents gave negative results. A third urinary sample gave negative results for thiazide but positive for furosemide; the fourth and fifth samples gave negative results for furosemide but positive for thiazide. Urinary prostaglandin excretion was normal. We conclude that this apparent case of Bartter's syndrome was caused by long term surrept

Clinical, pathological and genetic aspects of a form of cystic disease of the renal medulla: familial juvenile nephronophthisis (FJN).

A combined clinical, pathological and genetic study of 13 cases of FJN is reported. The clinical features are conistent with those described in the literature, except that short stature is not a feature in this group. Hyperplasia of the juxtaglomerular apparatus has been seen in 5 of 9 cases where histology was done. The genetic studies support the view that FJN is an autosomal recessive hereditary disease. Because their patterns of inheritance differ, FJN and medullary cystic disease (MCD) are separate entities. We suggest simplification of the nomenclature to: medullary cystic disease of chi

Juxtaglomerular cell tumor of the kidney--a new classification scheme.

OBJECTIVE: To introduce a new classification scheme of juxtaglomerular cell tumor (JCT) of the kidney for differential diagnosis of hypertension and renal cell carcinoma. METHODS: Five cases of JCT have been diagnosed and treated surgically in our hospital during the last 4 years. Through a search in PubMed, we incorporated 7 large series of case reports of JCT into a review of 71 cases previously published in the literature. Clinical presentations (blood pressure), laboratory examinations [serum potassium, plasma renin activity (PRA), aldosterone (ALD), and renal venous sampling for renin as

[Renin-secreting renal tumor. A case report].

We reported a case of juxtaglomerular cell tumor, which excessively produced renin, resulting in secondary hypertension. A 25-year-old woman complained of headache and nausea. Hypertension and elevation of plasma renin activity were found by physical and laboratory examination. US and CT showed a space occupying lesion at upper pole of the right kidney. Angiography showed a hypovascular area at the corresponding area of the right kidney. Renin-secreting renal tumor of the right kidney was diagnosed and right nephrectomy was performed. Postoperatively, blood pressure and plasma renin activity b

Renal hypotension in sodium and fluid deprivation: experimental findings in renin-depleted rats.

Arterial hypotension of renal origin occurred as consequence of low plasma renin activity in the presence of sodium and extracellular fluid volume depletion. Secretory insufficiency of the renin-producing juxtaglomerular cells and sodium and volume deprivation, simultaneously, were achieved by removing the "clamped" kidneys in renal hypertensive, sodium- and volume-depleted rats leaving in situ the contralateral kidneys deprived of renin during the preceding period of hypertension. It is suggested that renal hypotension after acute losses of sodium and extracellular fluid may also develop in p

Studies on renal juxtaglomerular cells. III. The effects of experimental renal disease and hypertension in the rat.

Hypertension in rats produced by constriction of one renal artery was associated with degranulation of juxtaglomerular cells in the contralateral, undamped, kidney. These findings are consistent with those of other investigators. Furthermore, the degree of granulation (JGI) in the unclamped kidney was inversely correlated with the level of blood pressure (r = -0.7). Degranulation of JG cells also occurred in rats made hypertensive by application of a "figure-of-eight" ligature to one kidney and removal of the other one, except when the interference in blood supply was so severe that scarring r

Characteristic changes of the juxtaglomerular cells before and after treatment of pseudo-Bartter's syndrome due to furosemide abuse.

Histological and ultrastructural studies of juxtaglomerular cells (JGC) were performed in a patient with pseudo-Bartter's syndrome due to furosemide abuse. The biopsy done before the treatment revealed a large number of secretory granules and mitochondria, dilated rough endoplasmic reticulum and a well-developed Golgi apparatus in JGC. The JGC granules, some of which contained crystalloid structures showed various shapes and sizes. In the biopsy carried out after the cessation of furosemide intake, these morphological changes were markedly improved with a reduced activity of the renin-angioten

Renovascular hypertension from segmental renal artery stenosis: importance of segmental renal vein renin sampling.

We report a case of severe hypertension in a girl with no evidence of parenchymal renal disease. Arteriography revealed segmental stenosis and a saccular aneurysm affecting a branch of the left renal artery. Segmental renal vein renin sampling demonstrated a markedly increased rate of renin secretion from the ischemic portion of the left kidney. Partial nephrectomy resulted in normalization of the blood pressure. The resected kidney showed striking hyperplasia of the juxtaglomerular apparatus and medial fibromuscular dysplasia as the etiology of the arterial stenosis. This case emphasizes the

Relevant Equine Renal Anatomy, Physiology, and Mechanisms of Acute Kidney Injury: A Review.

Regulation of renal blood flow is by both extrinsic and intrinsic systems. Intrinsic regulation occurs via the afferent and efferent arterioles and tubuloglomerular feedback mechanisms with activation of the juxtaglomerular apparatus. Mechanisms of acute kidney injury are frequently associated with changes in renal blood flow. Acute tubular necrosis and apoptosis are common in horses following ischemic or toxic insults and in sepsis-associated acute kidney injury. Sepsis-associated renal injury often has a complex mechanism of disease involving both functional and obstructive changes in intrar

An extreme example of the neonatal form of Bartter's syndrome.

A male infant is described who had polyuria over the 4 months of his life with urine volumes exceeding 1,000 ml/kg per day, severe serum electrolyte losses, metabolic alkalosis and increased plasma renin activity (56 ng/ml per hour). He had a normal blood pressure and glomerular filtration rate when fluid replete. The urine flow rate was about 25% of the glomerular filtration rate. Renal histology showed hyperplasia of the juxtaglomerular apparatus and abnormalities of the proximal tubules. The features of this case suggest an extreme form of Bartter's syndrome presenting from the first days o

Unilateral renovascular hypertension in rats. Measurements of medullary granules, juxtaglomerular granularity and cellularity, and areas of adrenal zones.

To produce hypertension, the left renal artery was clipped in 20 rats. Six rats had a sham operation. After 4 months, some of the rats showed only a moderate increase in blood pressure, and a second clipping of the same artery was performed. When hypertension became severe in some of the rats, 2 to 4 weeks later, all rats were sacrificed. The "clipped" rats were divided into four groups depending on whether they had little change in blood pressure, acute hypertension, chronic hypertension, or chronic and superimposed acute hypertension. The average adrenal weights were increased in the two gro
