BACKGROUND: Seborrheic keratosis is a very common benign skin tumor in man. Melanoma is rare but is the most dreaded of all malignant skin tumors. A melanoma arising in a seborrheic keratosis is distinctly rare. We are reporting such a case occurring in an 86-year-old man.

Seborrheic keratosis (SK) consists of a localized proliferation of basaloid keratinocytes, often accompanied by hyperkeratosis and hyperpigmentation. In irritated SK, these features are associated with areas of squamous differentiation with larger keratinocytes and squamous cell eddies. This work is concerned with the evaluation of apoptosis, as demonstrated by the TUNEL method, in the different varieties of SK. Skin biopsies from 10 patients with seborrheic keratoses were examined by electron microscopy for the presence of Langerhans' cells. Comparing seborrheic keratoses with normal skin of the same patient and with normal skin from controls, neither an increased number of Langerhans' cells nor an increased number of specific granules, Birbeck granules, nor abnormal Langerhans' cells, was found. Melanosomes in a Langerhans' cell were observed in 2 seborrheic keratoses, suggesting phagocytic activity of the Langerhans' cell.

PURPOSE OF REVIEW: Patients commonly present to the otolaryngologist for management of benign facial lesions. It is important to be able to both recognize these lesions and offer patients the most effective therapies currently available. We present a rare case of clonal seborrheic keratosis (SK) with focal Bowen disease (BD) (squamous cell carcinoma in situ) accompanied by sebaceous differentiation. An 89-year-old woman presented with a pale reddish-brown plaque on the left buttock. A 19-year-old woman with three seborrheic keratosis on her right abdomen and five seborrheic keratosis on her both buttocks is presented. That developed at the age of five and two months prior to the visit. At our initial dermatological examination, we noticed three oval, well defined, brown tumors on her right abdomen, and several round, well defined, brown nodules on her both buttocks. Desmosomes exhibit quantitative alterations in neoplastic diseases.
