Preclinical safety evaluation of nafithromycin (WCK 4873) with emphasis on liver safety in rat and dog.

Ketolide antibiotics are known to cause hepatic injury. Nafithromycin, a novel lactone ketolide was therefore assessed for hepatic safety through range of preclinical in vitro (metabolic stability, CYP inhibition/induction assays) and in vivo (mass balance and repeat dose toxicity) studies. Repeat-dose toxicity studies in rat and dog revealed that nafithromycin did not cause adverse hematological, biochemical and histopathological changes suggestive of systemic or hepatobiliary safety concern at exposures 3-8 fold higher than targeted therapeutic exposures. The only histological finding notice

Azithromycin, clarithromycin and telithromycin inhibit MUC5AC induction by Chlamydophila pneumoniae in airway epithelial cells.

BACKGROUND: Airway mucus hypersecretion is an important problem in chronic respiratory diseases including bronchial asthma. Chlamydophila pneumoniae is recently confirmed to be a pathogen in bronchial asthma, but the relationship between C. pneumoniae and mucus hypersecretion is uncertain. In this study, we examined whether C. pneumoniae induces MUC5AC mucin in airway epithelial cells. We also examined the effects of macrolide and ketolide antibiotics on the C. pneumoniae-induced mucus production. METHODS: MUC5AC production in bronchial epithelial cells after stimulation with C. pneumoniae wa

Telithromycin in the treatment of acute bacterial sinusitis, acute exacerbations of chronic bronchitis, and community-acquired pneumonia.

Acute bacterial sinusitis (ABS), acute exacerbations of chronic bronchitis (AECB), and community-acquired pneumonia (CAP) are common conditions and constitute a substantial socioeconomic burden. The ketolides are a new class of antibacterials with a targeted spectrum of antibacterial activity. In vitro, telithromycin is active against common bacterial pathogens that cause upper and lower respiratory tract infections, including some isolates that are resistant to other antibiotic classes. In 2004, telithromycin was the first ketolide antibiotic approved for clinical use by the US Food and Drug

14- and 15-membered lactone macrolides and their analogues and hybrids: structure, molecular mechanism of action and biological activity.

Classical macrolides containing 14- or 15-membered aglycone lactone rings are basic and widely used antibacterial agents in clinical therapy. Macrolides exert antibacterial effects by blocking the exit peptide tunnel during translation at bacterial ribosomes. However, one macrolide occasionally manifests different docking modes or binding sites at ribosomes, which presents difficulties in designing macrolide modifications. In addition to their medical importance, lactone macrolides possess interesting chemistry driven by numerous intramolecular interactions or cascade transformations. Lactone

Nature nurtures the design of new semi-synthetic macrolide antibiotics.

Erythromycin and its analogs are used to treat respiratory tract and other infections. The broad use of these antibiotics during the last 5 decades has led to resistance that can range from 20% to over 70% in certain parts of the world. Efforts to find macrolides that were active against macrolide-resistant strains led to the development of erythromycin analogs with alkyl-aryl side chains that mimicked the sugar side chain of 16-membered macrolides, such as tylosin. Further modifications were made to improve the potency of these molecules by removal of the cladinose sugar to obtain a smaller m

Solithromycin inhibition of protein synthesis and ribosome biogenesis in Staphylococcus aureus, Streptococcus pneumoniae, and Haemophilus influenzae.

The continuing increase in antibiotic-resistant microorganisms is driving the search for new antibiotic targets and improved antimicrobial agents. Ketolides are semisynthetic derivatives of macrolide antibiotics, which are effective against certain resistant organisms. Solithromycin (CEM-101) is a novel fluoroketolide with improved antimicrobial effectiveness. This compound binds to the large 50S subunit of the ribosome and inhibits protein biosynthesis. Like other ketolides, it should impair bacterial ribosomal subunit formation. This mechanism of action was examined in strains of Streptococc

Drug interactions during therapy with three major groups of antimicrobial agents.

This review focuses on drug-drug interactions with three major groups of antimicrobial agents: macrolides (including azalides and ketolides), quinolones, which are widely used for the treatment of bacterial infections, and azoles, which are used for antifungal therapy. Macrolides and the ketolide telithromycin are potent inhibitors of CYP3A4 and thus interfere with the pharmacokinetics of many other drugs that are metabolised by this enzyme. In contrast, although closely related, azithromycin is not a cytochrome inhibitor. All quinolones form complexes with di- and trivalent cations and, there

Antibacterial effect of telithromycin (HMR 3647) and comparative antibiotics against intracellular Legionella pneumophila.

The activity of the ketolide telithromycin (HMR 3647) against intracellular Legionella pneumophila strain L-1033 was compared with the activities of erythromycin and levofloxacin. To assay intracellular antibacterial activity, human monocytes were allowed to adhere to wells in 24-well tissue culture plates and were then exposed to L. pneumophila cells for 1 h to allow phagocytosis to occur. Antibiotics were added to the wells after removal of unphagocytosed bacteria. Quantitative bacterial cell counts were made from lysed monocytes at 0, 24, 48, 72 and 96 h. The antibacterial effects of antibi

[Clinical presentation, diagnosis and treatment of Mycoplasma pneumoniae pneumonia].

Mycoplasma pneumoniae is a common respiratory pathogen in community acquired pneumonia. In severe cases extrapulmonary manifestations of the disease may be prominent. Proof of infection is found, though usually delayed, through serologic methods. A calculated therapy of community acquired pneumonia, especially in younger patients with interstitial infiltrates, should therefore include effective treatment for Mycoplasma pneumoniae. Macrolide antibiotics are the most commonly used antibiotics in the treatment of mycoplasma infection, alternatives include ketolides, quinolones and tetracyclines.

Distribution, characterization and genetic bases of erythromycin resistance in staphylococci and enterococci originating from livestock.

The occurrence of staphylococci and enterococci expressing increased resistance to erythromycin (ERY) and, in particular, to macrolide-lincosamide-streptogramin B (MLS(B) ) antibiotics was investigated in dairy cattle, pigs and turkeys. Three hundred rectal (cloacal) swabs of each animal species were examined. A total of 120 and 71 staphylococci and enterococci, respectively, with increased resistance to ERY were identified. These were most frequent in turkeys (42.3% of positive animals), followed by pigs and dairy cattle (6.7% and 6.0% of positive animals, respectively). Similarly, MLS(B) -re

In vitro susceptibilities of seven Leptospira species to traditional and newer antibiotics.

Human leptospirosis is generally treated with penicillin or doxycycline. We studied the susceptibilities of 11 serovars (seven species) of Leptospira to 14 antibiotics. With the exception of chloramphenicol, all tested agents were at least as potent as penicillin and doxycycline, with the macrolide and ketolide drugs producing the lowest MICs (and minimal bactericidal concentrations).

Recent progress in novel macrolides, quinolones, and 2-pyridones to overcome bacterial resistance.

Macrolides, such as clarithromycin and azithromycin, having good activity against pathogens such as Legionella, Chlamydia, Campylobacter spp, Branhamella spp, Pasteurella multocida and streptococci, have gained wide acceptance for the treatment of both upper and lower respiratory tracts, as well as cutaneous infections. Emergence of bacterial resistance, particularly in gram-positive bacteria, has been observed. Macrolide-resistant Streptococcus pneumoniae and S. pyogenes are found in France and many other countries, resulting in failure of therapy for pneumonia, pharyngitis, and skin infectio

Apparent anaphylactoid reaction after treatment with a single dose of telithromycin.

BACKGROUND: Telithromycin, the first ketolide antimicrobial agent available in the United States, is related to the macrolide class of antibiotics. Reports of immediate-type hypersensitivity reactions with macrolides or ketolides are rare overall. OBJECTIVE: To describe a patient with a severe, apparent mast cell-mediated reaction to telithromycin. METHODS: A 54-year-old woman with a history of hypertension was prescribed telithromycin by her primary care physician for presumed bacterial sinusitis. Shortly after ingesting the first dose of telithromycin she developed severe shortness of brea

Increased bacterial resistance: PROTEKT US--an update.

Community-acquired respiratory tract infections (CARTIs), including community-acquired pneumonia, acute exacerbations of chronic bronchitis, and acute bacterial sinusitis, remain substantial sources of morbidity and mortality in the US. The increasing prevalence of antimicrobial-resistant strains among the common respiratory pathogens, particularly Streptococcus pneumoniae, has complicated the choice of antimicrobials and threatens the efficacy of treatment for CARTIs. This highlights the need both for ongoing surveillance efforts to track current resistance patterns and for the development of

[Telithromycin].

For the time being telithromycin is the only representative of a new group of antibiotics - ketolides - available for clinical use. Its chemical structure is derived from erythromycin, but it is better resorbed from the digestive tract and is efficacious against most macrolide-resistant strains of Streptococcus pneumoniae and Streptococcus pyogenes. Telithromycin is registered for the treatment of respiratory infections: sinusitis, acute exacerbation of chronic bronchitis, community-acquired pneumonia and streptococcal tonsillopharyngitis. The drug is well tolerated. It is less likely to induc

Telithromycin-induced acute interstitial nephritis: a first case report.

Telithromycin, a ketolide antibiotic used for the treatment of community-acquired respiratory infections, is widely prescribed in primary care practice. Treatment-related adverse events are mainly of gastrointestinal origin and generally mild in intensity. The authors report the first case of telithromycin-induced severe acute interstitial nephritis. Practitioners should be aware of the possibility that telithromycin therapy could result in this form of drug-induced acute renal failure.

An Approach to Modify 14-Membered Lactone Macrolide Antibiotic Scaffolds.

A ketolide derivative with (12R)-configuration was obtained via a novel ketene acetal in acidic conditions. The structure of this atypical β-keto ketene acetal intermediate within the macrocyclic system has been determined by NMR and X-ray methods. The use of basic conditions at an elevated temperature yielded new, doubly α,β-unsaturated ketone macrolide derivatives with (4E)-configuration as two conformational isomers of folded-in or folded-out conformations.

Clinical microbiology and infectious diseases - 11th European Congress. 1-4 April 2001, Istanbul, Turkey.

This report covers the four different sessions of the congress. All sessions included the presentation of papers about treatment of respiratory tract infections (RTIs). RTIs account for 25% of all the community-acquired infections and are the second most frequent among nosocomial infections after urinary tract infections. Recent years have witnessed a great increase in resistance to antibiotics among pathogens causing RTIs. These changes have rendered several 'classical' antibiotics less effective in this indication. The drug industry has collectively made a great effort to develop new antibio

Telithromycin: visual disorders.

Telithromycin is a macrolide antibiotic with a negative risk-benefit balance. Its efficacy is no better than that of other macrolides. Telithromycin carries a risk of serious adverse effects, including loss of consciousness, QT prolongation, severe liver damage, aggravation of myasthenia gravis, and a high risk of drug interactions. In Finland, a 7-year pharmacovigilance review identified 20 reports of visual disorders among patients taking telithromycin, including blurred vision, accommodation disorder and diplopia, mainly in young patients and women. Visual disorders occurred in about 1% of
