Calyceal diverticula: Clinical, radiological and histopathological findings of an uncommon entity with presumed congenital origin.

Calyceal diverticula (CD) are relatively uncommon urologic conditions that generally follow an asymptomatic course and rarely require medical intervention. CD are thought to have a congenital origin due to abnormalities during the process of ureteral bud formation. Clinically and radiologically, they can mimic multiple neoplastic and non-neoplastic renal processes, with potentially relevant differences in the management of these patients. Symptoms are usually associated with the presence of stones, obstruction to the drainage of the diverticulum, large size, or secondary infection. In chronic

[Management of stones in renal caliceal diverticula].

Stones in renal caliceal diverticula with no symptom or infection can be managed with surveillance. In other cases, extracorporeal lithotripsy (ESWL) can be the first line treatment for all diverticular stones, whatever their renal location. In case of ESWL failure, the procedure choice depends of the location of the diverticulum. Percutaneous surgery is indicated for lower and posterior diverticula. The puncture must be directly targeted to the stone. Flexible ureteroscopy is indicated for upper and mid, anterior or posterior, diverticula. Laparoscopy can be indicated for anterior, mid and lo

Bilateral additional renal arteries and an additional right renal vein associated with unrotated kidneys.

We report a rare anomaly of the kidneys and its vessels, which were found during the routine dissection of a 68-year-old male cadaver. The anomaly consisted of bilateral additional renal arteries originating from the abdominal aorta and an additional right renal vein accompanying the additional right renal artery. These anomalies were associated with unrotated kidneys with extrarenal calices and pelves. All the additional vessels were located posterior to the ureter with a close relationship to the ureteropelvic junction on the right side. Additional renal vessels arise as a result of the comp

[Renal ultrasonic diagnosis in pregnancy. Prospective study of pyelo-calyceal changes in the mother and their obstetric implications].

An acute pyelonephritis is still a serious complication of pregnancy. Urinary stasis plays an important role in the pathogenicity of this infection. 232 patients without symptoms were studied to find out how much urinary stasis occurred by measuring dilatations of the pyelo-calices by ultrasound. The authors suggest that a correlation between the bacteriological examination of the urine and the ultrasound appearance of the kidney can be established. Asymptomatic urinary infection occurs more frequently when there is dilatation of the pelvis of the kidney greater than 12 mm in the right kidney

Predictors of lower pole renal stone clearance after extracorporeal shock wave lithotripsy.

PURPOSE: Lower pole renal stones are well known to show a poor stone clearance rate after extracorporeal shock wave lithotripsy (ESWL) (Dornier Medical Systems, Marietta, Georgia). In the current study we analyzed several anatomical factors as predictors of lower pole stone clearance that may be used to indicate the usefulness of ESWL in such patients. MATERIALS AND METHODS: Between January 1993 and October 2000, 63 patients with a unilateral single lower pole stone of 2 cm. or less were included in the study. Excretory urography was done to determine the lower infundibulopelvic angle, calice

Combination extracorporeal shock wave lithotripsy and percutaneous extraction of calculi in a renal allograft.

Renal calculi are a well documented although uncommon complication of kidney transplantation and may be associated with significant morbidity in this immunosuppressed population with a single functioning kidney. We describe a patient who presented with 2 episodes of staphylococcal bacteremia associated with a ureteral structure and struvite calculi involving the calices, renal pelvis and proximal ureter of a cadaveric renal allograft. The patient was treated successfully with a combination of extracorporeal shock wave lithotripsy, percutaneous extraction and balloon dilation of the ureteral st

Transperitoneal Laparoscopic Pyelolithotomy in Pelvic Ectopic Kidneys: Experience From a Northern Indian Tertiary Care Institution.

Introduction The ectopic pelvic kidneys have a higher likelihood of developing renal stones due to urinary stasis caused by the abnormal position of the renal pelvis, altered course of the ureter, and kidney malrotation. This retrospective study highlights the safety, efficacy, and feasibility of performing transperitoneal laparoscopic pyelolithotomy in cases of pelvic ectopic kidney. Methodology The 15 patients with ectopic pelvic kidneys and nephrolithiasis underwent laparoscopic pyelolithotomy. The kidney was exposed either by moving the bowel or using a trans-mesocolic approach. A surgical

Caveat Emptor: The Heat Is "ON"-An In Vivo Evaluation of the Thulium Fiber Laser and Temperature Changes in the Porcine Kidney During Dusting and Fragmentation Modes.

Introduction: We sought to examine the intrarenal fluid and tissue temperature alterations during dusting and fragmentation with the thulium fiber laser (TFL) in an in vivo porcine kidney. Materials and Methods: In two female Yorkshire pigs, temperature was continuously measured within the upper, interpolar, and lower calices along with the renal pelvis using multipoint thermal sensor probes; another temperature probe was situated at the tip of the ureteroscope. Four experimental protocols were performed for each animal: dual lumen ureteroscope with both warmed (37°C) irrigation and room tempe

Percutaneous nephrolithotomy in the spinal cord injury population.

We evaluated 23 male spinal cord injury patients who underwent percutaneous nephrolithotomy for the success rate of stone removal and the incidence of operative complications. There were 18 quadriplegic and 5 paraplegic patients, and 5 had bilateral procedures. Of the kidneys 7 had staghorn calculi, 8 had pelvic and caliceal combinations, 6 had large multiple caliceal stones, 4 had large (more than 2.5 cm.) pelvic stones and 3 had pelvic stones less than 2.5 cm. Placement of a nephrostomy tube and stone extraction were performed as a single procedure with the use of general anesthesia in all b

Renal abnormalities in the Bardet-Biedl syndrome.

Four cases of BARDET-Biedl syndrome (BBS) are described which all suffer from renal abnormalities. Polyuria or polydipsia with impairment of renal concentration capacity were the earliest signs of renal dysfunction. Renal insufficiency developed in 3 cases and hypertension in two. Urographic abnormalities were demonstrated in all patients. The most remarkable features were cystic spaces communicating with the calices and lobulation of kidney. Caliceal clubbing and caliectasis surrounded by narrowed, unscarred parenchyma were frequent findings. Previous investigators reported various renal hist

Pelvicalyceal anatomy on the accessibility of reusable flexible ureteroscopy to lower pole calyx during retrograde intrarenal surgery.

OBJECTIVE: This study aimed to evaluate the pelvicalyceal anatomy on accessibility of reusable flexible ureteroscopy (fURS) to the lower pole calyx during retrograde intrarenal surgery (RIRS). METHODS: Here, 854 patients with ureteral or kidney stones with access to a renal collecting system using reusable fURS were classified into either the accessible group, in whom the deepest lower pole calyces could be touched; and the inaccessible group, in whom the deepest lower calyces could not be touched. We measured the infundibulopelvic angle (IPA), infundibular width (IW), infundibular length (IL

[Retroperitoneoscopic resection of the calyceal diverticulum wall in an 8-year-old child].

A renal calyceal diverticulum is a cystic cavity within the renal parenchyma, which is lined by transitional epithelium and communicates with the calyx or, more rarely, the renal pelvis. In children, this anomaly is extremely rare. The article describes the resection of the middle calyceal diverticulum in an 8-year-old child using retroperitoneoscopic access.

[Flow technic analysis of the pyelo-calical system with reference to the origin of urinary calculi. II. 1st experimental results].

Theoretical considerations, part I (1), gave rise to the hypothesis that hydrodynamic disturbances in the upper urinary tract could have an influence on the formation of urinary calculi. Since it is not yet practically possible to find out the course of flow purely by calculation, experimental investigations were carried out using static glass models of the renal pelvis; the results, which are presented here, indicate the problems involved and our approach.

[Spontaneous rupture of the kidney pelvis calix system--causes, diagnosis and therapy].

The symptoms, diagnosis and management of the spontaneous non-traumatic rupture of the urinary collecting system are described. It is important to distinguish this frank rupture of the diseased renal pelvis from extravasation due to pyelosinusal or fornical backflow. Most cases of the latter variety are caused by acute calculous ureteral obstruction and observed during intravenous pyelogram with abdominal compression. They can be managed conservatively. Frank rupture of the renal pelvis are caused by an acute obstruction in a collecting system affected by infection, chronic obstruction, calcul

Percutaneous nephrolithotomy for caliceal diverticular calculi: a novel single stage approach.

PURPOSE: Current percutaneous treatment of symptomatic caliceal diverticular calculi involves renal access, stone removal, dilation of the diverticular communication, fulguration of the cavity and placement of a nephrostomy tube. We reviewed the outcomes of patients undergoing a novel single stage percutaneous nephrolithotomy technique for radiopaque caliceal diverticular stones that eliminates ureteral catheterization and entry into the renal collecting system. MATERIALS AND METHODS: A total of 21 patients (8 male and 13 female including 1 bilateral) with a mean age of 42.4 years underwent p

Retrograde flexible ureteroscopic approach for pyelocaliceal calculi.

INTRODUCTION: Our goal was to investigate the efficacy of flexible ureteroscopy (FU) in the treatment of pyelocaliceal calculi. MATERIALS AND METHODS: Between September 2002 and December 2004, a total of 41 patients with multiple (23 cases), pelvic (7 cases), and inferior caliceal (11 cases) SWL-resistant calculi underwent FU. We used a 7.5-F flexible ureteroscope with pressure irrigation and electro-hydraulic lithotripsy. The fragments were retrieved with triradiate graspers or tipless baskets. RESULTS: A double J stent had been previously placed in 34% of the patients. Dilation of the uret

Adult polycystic kidney disease: a review of 100 cases.

Herein we review 100 cases of adult polycystic kidney disease. Flank pain was the most common presentation in these cases. In 20% of the patients calculi were noted and 10% had obstruction. The average patient age at death was 57 years. The importance of tomography is emphasized with key radiographic features, such as bilateral large kidneys, inhomogeneous nephrogram, caliceal distortion and poorly defined renal margins. Cystic involvement of the liver and spleen may help to confirm the diagnosis. Differential diagnosis includes bilateral multiple simple cysts, angiomyolipomatosis and bilatera
