Treatment algorithm for osteochondral injuries of the knee.

The treatment of osteochondral fractures and OCD lesions in the knee is controversial. Many new procedures and techniques have been developed recently to address osteochondral lesions, indicating that no single procedure is accepted universally. Our treatment algorithm is based on the age of the patient, skeletal maturity, and the presence of adequate subchondral bone attached to the chondral lesion. Most nondisplaced lesions in the patient with open physes will heal with conservative treatment. The onset of skeletal maturity indicates a need for a more aggressive treatment approach. If adequa

Injuries about the knee and tibial/fibular shafts.

Injuries to the knee and tibial/fibular shafts are extremely common, with knee injuries alone accounting for over 1.3 million emergency department visits yearly in the United States. Many of these injuries will present with straightforward radiographic findings, but others will have a subtle or complex appearance. This article reviews injuries of the knee and proximal tibial/fibular shaft with emphasis on normal anatomic features that, when disrupted, indicate the presence of subtle but important bone or soft tissue trauma. Although the emphasis is on plain radiography, magnetic resonance (MR)

[Tendon injuries of the knee joint extensor mechanism].

Diagnosis, aetiology and therapy of injuries to the extensor mechanism of the knee joint are presented. 22 cases of ruptures and discisions of the lig. patellae or the quadriceps tendon could be reexamined. All cases but one, with operation within the first week after injury, suturing with absorbable or non absorbable material, immobilisation for 6 weeks and following physical therapy, had a good functional result. No rerupture occurred. In one case a limitation of the range of movement had to be seen afer an open discision of the lig. patellae. After transposition of the tuberositas tibiae, d

Arthroscopy in acute injuries of the knee joint.

Arthroscopy was performed during the acute phase of injury in 84 knees (79 patients). A satisfactory view of the joint was obtained in all cases, and no complications occurred. About two-thirds of the patients had injuries associated with violent rotation-abduction. In about one-third of the patients operation could be avoided. In cases with haemarthrosis, serious ligament injury was present in nearly 50 per cent. Complete arthroscopy was associated with few diagnostic errors. Clinical examination often led to uncertain or incorrect diagnosis even when performed under anaesthesia by experience

Complex knee injuries from bouldering: A case series.

INTRODUCTION/OBJECTIVES: Bouldering, a dynamic climbing discipline performed without ropes, is associated with high-energy ground falls. While most existing climbing injury literature has focused on the upper extremities, our data indicated an increasing number of complex knee injuries related to bouldering. This review was conducted to characterize the injury patterns sustained from ground falls during bouldering, and to compare the relative burden of these injuries with those sustained in other high-participation sports. METHODS: A retrospective case series was performed, including climbers

[Injuries of the knee extensor complex in children].

BACKGROUND: Fractures of the knee extensor complex are relatively rare injuries in children. We distinguish between a pure tendon rupture and ruptures with bony avulsions. A mid-substance tear in children is a relatively rare injury, whereas bony avulsions due to skeletal immaturity are seen more often. Sole bony fractures of the patella also only account for a low number of paediatric related fractures. Sleeve fractures are characterized by an avulsion of cartilage and, usually, bone pulled off from the distal patella pole. Avulsion fractures of the tibia tubercle are found in the group of sp

Sex and Gender Differences in Pediatric Knee Injuries.

The participation of females in sports has increased significantly since the passage of Title IX. Sports participation may place young athletes at risk for knee injuries, including patellofemoral pain syndrome (PFPS), osteochondritis dissecans (OCD), and anterior cruciate ligament (ACL) rupture. Differences in anatomy, hormone production, and neuromuscular patterns between female and male athletes can contribute to disparities in knee injury rates with female athletes more vulnerable to PFPS and ACL injury. Biological differences between sexes alone cannot fully explain worldwide differences i

Concomitant ligamentous and meniscal injuries in floating knee.

BACKGROUND: To identify and characterize the concomitant ligamentous and meniscal injuries in floating knee. METHODS: A total of 37 cases of floating knee were enrolled. Arthroscopic or open surgical examination of the knee, Lachman test, posterior drawer's test, and varus and valgus stress tests under anesthesia were carried out to determine the incidence of knee injury. RESULTS: Through arthroscopic and open surgical examinations, a medial meniscal tear was detected in 14 (37.8%) cases and a lateral meniscal tear in 11 (29.7%). Twenty-one (56.8%) patients had anterior cruciate ligament (AC

Updates in biological therapies for knee injuries: menisci.

The preservation of meniscal tissue is paramount for long-term joint function, especially in younger patients who are athletically active. Many studies have reported encouraging results following the repair of meniscus tears, including both simple longitudinal tears located in the periphery and complex multiplanar tears that extend into the central third avascular region. However, most types of meniscal lesions are managed with a partial meniscectomy. Options to restore the meniscus range from an allograft transplantation to the use of synthetic and biological technologies. Recent studies have

Injuries of the posterolateral corner of the knee.

The complex anatomy of the posterolateral corner of the knee is due largely to the evolutionary changes in the anatomic relationships of the fibular head, the popliteus tendon, and the biceps femoris muscle. Recent research has improved our understanding of the popliteus complex, particularly the role of the popliteofibular ligament. Biomechanical studies provide a scientific basis for clinical examination of the knee with suspected injury of the posterolateral corner. All grade-I and most moderate grade-II injuries of the posterolateral structures can be treated nonoperatively, but residual l

Magnetic resonance imaging of knee injuries in children.

The appearances of knee injuries on MR imaging are less well documented in children than adults. Some patterns of injury are shared by both groups of patients, e. g. meniscal damage. The frequency of specific injuries may differ, e. g. anterior cruciate ligament (ACL) tear. Congenital abnormality, coexistent pathology and previous treatment of the knee appear to be associated with meniscal problems. Discoid menisci are seen most frequently in children and have unique features on MR scans. Cruciate ligament tears are difficult to diagnose in the smallest children. The ACL may not be identified

[Epidemiology of internal knee injuries in Alpine skiing].

The incidence of sport injuries in Switzerland is twice as high compared to traffic injuries. Soccer and alpine skiing play the most important role in sport injuries. For the last twenty winter seasons all winter sport injuries were analysed at the hospital of Davos. About 85% are injuries caused by alpine skiing resulting in a total of 24,684 for the last twenty years. There is a great decline in the incidence of lower leg fractures with a contrary development of knee injuries. The treatment-quotient (amount of knee injuries in alpine skiers per 10(4) kilometers of vertical drop skied) has in

[Ultrasonic examination of ankle and knee ligament injuries].

Ultrasound examinations and stress radiographs of 61 patients with suspected ligamentar lesion of the ankle and knee joints were compared in order to clear, whether radiography could be replaced by sonography. In our experience, the opening of the joint gap can also be adequately demonstrated even without ionizing radiation and the results achieved by ultrasound were nearly as accurate as by X-rays. Being a rather simple, inexpensive and real time method, producing acceptable visual information and practicable on almost all preexisting equipments, sonography is advised for detecting of ligamen

Arthroscopic Assessment and Treatment of Dancers' Knee Injuries.

In brief: The author saw 16 dancers with a history of dance-related knee injuries that had defied conservative treatment. Arthroscopic examination revealed 15 meniscal tears and four cases of chondromalacia patellae. The meniscal tears were treated by partial arthroscopic meniscectomy and the chondromalacia patellae by shaving, drilling, and debridement of the patella. After surgery the results were rated by means of a physical examination and a patient questionnaire. The results were generally excellent, with 13 of the 16 dancers returning to a preoperative level of dance activity. However, t

Vascular complications in injuries about the knee joint.

In 1,377 patients who had severe injuries about the knee joint, vascular complications were present in 28 (2%). Injuries to the popliteal artery or its branches present serious problems. Amputation was necessary in 13 patients, and 12 patients had a Volkmann's syndrome. In fractures of the lower end of the femur or upper end of the tibia, and in knee dislocations, the importance of early diagnosis of the acute ischemic syndrome cannot be overemphasized. Tibial plateau fractures produced the highest percentage of vascular complications and indicate immediate application of therapeutic measures.
