From compliance to complexity: the impact of in vitro diagnostic regulation for clinical microbiology laboratories.

OBJECTIVES: The implementation of the European in vitro medical devices regulation (IVDR) poses significant challenges, yet its impact has not previously been quantified in clinical microbiology. The aim of this study was to quantify the number of tests in a large clinical microbiology laboratory that may be affected by IVDR. METHODS: The study was performed in a clinical microbiology laboratory of a large academic hospital in the Netherlands. We calculated proportion of CE-marked tests and laboratory-developed tests (LDTs), and we calculated the number of test results generated by these test

External quality assessment for detection of colorectal cancer by Septin9 DNA methylation in clinical laboratories.

BACKGROUND AND AIMS: The incidence and mortality rate of colorectal cancer (CRC) are increasing worldwide. Septin9 methylated (mSEPT9) DNA in circulation can be used as a non-invasive detection method to assist in the early diagnosis of CRC; however, the detection methods and procedures are complicated. This study aimed to evaluate the ability of clinical laboratories to detect Septin9 methylation in plasma cell-free DNA (cfDNA). MATERIALS AND METHODS: We prepared a sample panel consisting of positive and negative Septin9 methylation cells and CRC cells. Three positive samples with different

Brucella Exposure Risk Events in 10 Clinical Laboratories, New York City, USA, 2015 to 2017.

From 2015 to 2017, 11 confirmed brucellosis cases were reported in New York City, leading to 10 Brucella exposure risk events (Brucella events) in 7 clinical laboratories (CLs). Most patients had traveled to countries where brucellosis is endemic and presented with histories and findings consistent with brucellosis. CLs were not notified that specimens might yield a hazardous organism, as the clinicians did not consider brucellosis until they were notified that bacteremia with Brucella was suspected. In 3 Brucella events, the CLs did not suspect that slow-growing, small Gram-negative bacteria

Clinically amyopathic dermatomyositis: Clinical, laboratory, and histopathological features.

BACKGROUND: Despite the advancements in the categorization of clinically amyopathic dermatomyositis (CADM), the classification and diagnosis of its subtypes are still challenging. The aim of our study was to describe the clinicopathological features of CADM and assess the differences between amyopathic dermatomyositis (ADM) and hypomyopathic dermatomyositis (HDM). METHODS: This retrospective study included 43 patients with CADM diagnosed at our institution from 2016 to 2020. Patients were subclassed into ADM (n = 30) and HDM (n = 13) groups to assess their clinicopathological differences. RE

Laboratory Monitoring and Measuring of Clinical Anticoagulants.

Haemostasis reflects a balance between procoagulant and anticoagulant forces. A relative excess of procoagulant forces and/or a relative lack of anticoagulant forces can predispose individuals to an increased risk of thrombosis. Conversely, a relative lack of procoagulant forces and/or a relative excess of anticoagulant forces can predispose individuals to an increased risk of bleeding. It is possible in several clinical situations to mitigate bleeding risk using various approaches, including provision of procoagulant material; conversely, thrombosis risk can be mitigated by using various clin

Clinical and laboratory characterizations of hepatic capillariasis.

Hepatic capillariasis is a rare and neglected parasitic disease caused by infection with Capillaria hepatica in human liver. The disease is not well described and the information for the disease's clinical manifestation, laboratory findings and disease management strategy is not well reported. The limited information for this neglected infection often results in the delay of diagnosis or misdiagnosed to other diseases, therefore the real prevalence or severity of the infection may be underestimated. More case report with systemic analysis and features summary of this disease is needed to bette

Clinical and laboratory features of intestinal tuberculosis.

BACKGROUND/AIMS: As increasing numbers of Crohn's disease (CD) cases are being recognized in India, so the differential diagnosis of CD and gastrointestinal tuberculosis (GITB) is becoming increasingly important. If patients are misdiagnosed with GITB, toxicity may result from unnecessary anti-TB therapy and treatment of the primary disease (ie, CD) gets delayed. We therefore aimed to assess the accuracy of various parameters that can be used to predict GITB diagnosis at index evaluation. MATERIALS AND METHODS: This was a prospective, unicentric, observational study carried out in the gastroe

Clinical and Laboratory Diagnosis of Peroxisomal Disorders.

The peroxisomal disorders (PDs) are a heterogeneous group of genetic diseases in man caused by an impairment in peroxisome biogenesis or one of the metabolic functions of peroxisomes. Thanks to the revolutionary technical developments in gene sequencing methods and their increased use in patient diagnosis, the field of genetic diseases in general and peroxisomal disorders in particular has dramatically changed in the last few years. Indeed, several novel peroxisomal disorders have been identified recently and in addition it has been realized that the phenotypic spectrum of patients affected by

Use of the Clinical Laboratory in Psychiatric Practice.

In this review, the authors address the general principles of prescribing psychiatric medications and discuss how the clinical laboratory can be used to guide prescribing practices. Treatment considerations in different settings and for different medications are discussed. Because the clinical laboratory is advancing in its technology, so should the clinician's knowledge of how to use the clinical laboratory. The authors propose recommendations and a simple algorithm for how to use medication levels in blood and other fluids to guide care in medications without well-defined therapeutic windows

Bioterrorism and the Role of the Clinical Microbiology Laboratory.

Regular review of the management of bioterrorism is essential for maintaining readiness for these sporadically occurring events. This review provides an overview of the history of biological disasters and bioterrorism. I also discuss the recent recategorization of tier 1 agents by the U.S. Department of Health and Human Services, the Laboratory Response Network (LRN), and specific training and readiness processes and programs, such as the College of American Pathologists (CAP) Laboratory Preparedness Exercise (LPX). LPX examined the management of cultivable bacterial vaccine and attenuated str

Detection of intestinal protozoa in the clinical laboratory.

Despite recent advances in diagnostic technology, microscopic examination of stool specimens remains central to the diagnosis of most pathogenic intestinal protozoa. Microscopy is, however, labor-intensive and requires a skilled technologist. New, highly sensitive diagnostic methods have been developed for protozoa endemic to developed countries, including Giardia lamblia (syn. G. intestinalis/G. duodenalis) and Cryptosporidium spp., using technologies that, if expanded, could effectively complement or even replace microscopic approaches. To date, the scope of such novel technologies is limite

The clinical and laboratory assessment of the sleepy child.

Excessive sleepiness is defined as sleepiness that occurs in a situation when an individual would usually be expected to be awake and alert. Hypersomnia is characterized by recurrent episodes of excessive daytime sleepiness (EDS) or prolonged nighttime sleep, which affects the everyday life of the patient. Clinical surveys have reported that EDS is a complaint observed in up to 68% of normal high school children, with a negative impact in academic achievement and extracurricular activity. Clues toward recognizing childhood daytime sleepiness may be sleeping longer hours than expected for age,
