The effectiveness and safety of lacosamide in children with epilepsy in a clinical practice setting.

BACKGROUND: Seizures in up to 30% of children with epilepsy become refractory to treatment, decreasing their quality of life. Studies suggest that lacosamide may be effective in pediatric patients with refractory epilepsy. AIMS: To assess the effectiveness and safety of lacosamide in a population of children with mostly focal refractory epilepsy. METHODS: Retrospective analysis of children aged <18years presenting to a single hospital in Spain. Data from baseline, and 3, 6, and 12months after lacosamide initiation were collected and analyzed. Response to lacosamide was categorized by seizure

Lacosamide in children with refractory status epilepticus. A multicenter Italian experience.

OBJECTIVE: Status epilepticus (SE) is considered a life-threatening medical emergency. First-line treatment with antiepileptic drugs (AEDs) consists of intravenous benzodiazepines followed by phenytoin. SE is considered refractory (RSE) when unresponsive to standard doses of the first two AEDs. Scarce evidence is available to support specific guidelines for the management of RSE in either adults or children. This study aimed to assess the efficacy and tolerability of intravenous (iv) lacosamide (LCM) in children affected by RSE. METHOD: Children with RSE who were treated with ivLCM were inclu

Clinical experience with using lacosamide for the treatment of epilepsy in a tertiary centre.

OBJECTIVE: Lacosamide is approved for the adjunctive treatment of partial-onset seizures in adults. Phase II/III clinical trials suggest that it is a safe, effective and well-tolerated medication. However, there is little post-marketing information available about this medication. METHODS: We report our clinical experience from a tertiary referral epilepsy centre, which has been using lacosamide for the past 18 months, with 128 patients treated during this time. RESULTS: Fifty-three patients (41%) achieved at least a 50% reduction in seizure frequency, with 14 patients (11%) achieving seizur

Lacosamide in refractory mixed pediatric epilepsy: a prospective add-on study.

Lacosamide is a new antiepileptic drug that is currently approved by the US Food and Drug Administration (FDA) for adults 17 years or older for partial-onset seizures. The authors reviewed 21 pediatric patients (<17 years) with various seizure types who were started on oral lacosamide as part of a prospective add-on study as adjunctive therapy for refractory epilepsy. Five patients were excluded due to less than 3 months of meaningful follow-up. Maintenance dosages used ranged from 2.4 to 19.4 mg/kg/d. Eight of 16 (50%) patients had greater than 50% reduction in seizure frequency with adjuncti

No pharmacokinetic interaction between lacosamide and valproic acid in healthy volunteers.

Two open-label, randomized, multiple-dose clinical studies evaluated the potential for pharmacokinetic interaction between the antiepileptic drugs lacosamide and valproic acid. The influence of lacosamide on valproic acid pharmacokinetics (trial A) and valproic acid on lacosamide pharmacokinetics (trial B) was investigated in 32 healthy male volunteers, 16 in each trial. Volunteers in trial A received valproic acid (300 mg bid) with randomization to either early or late addition of lacosamide (200 mg bid). Those in trial B received lacosamide (200 mg bid) with randomization to either early or

Lacosamide: an investigational drug for adjunctive treatment of partial-onset seizures.

Lacosamide, (R)-2-acetamido-N-benzyl-3-meth- oxypropionamide, is a new chemical entity specifically synthesized as an anticonvulsive drug candidate, which appears to have a novel dual mode of action. Its pharmacokinetic characteristics have been studied in young and elderly healthy adults, as well as in adults with epilepsy or diabetic neuropathic pain. After oral administration, lacosamide is rapidly and completely absorbed. An elimination half-life of 13 hours allows for twice-daily dosing. Lacosamide has a low potential for drug-drug interactions. Both oral and intravenous formulations of l

Fracture Risk With Lacosamide vs Levetiracetam Monotherapy in Adults With Epilepsy.

OBJECTIVES: Patients with epilepsy exhibit substantially elevated fracture risk. However, comparative skeletal safety data for newer antiseizure medications (ASMs) remain limited. The aim of this study was to compare fracture risk between lacosamide and levetiracetam monotherapy in adults with epilepsy. METHODS: This retrospective cohort study used the TriNetX Global Collaborative Network. Adults with epilepsy initiating lacosamide or levetiracetam monotherapy were identified, excluding those receiving concomitant ASMs. Propensity score matching (1:1) balanced cohorts on demographics, comorbi

Population Pharmacokinetic Model of Lacosamide in Chinese Children and Adults with Epilepsy.

OBJECTIVE: To examine the pharmacokinetics of lacosamide (LCM) in Chinese children and adults with epilepsy and to establish a population pharmacokinetic model. METHODS: A one-compartment model with first-order absorption and elimination was constructed using 482 plasma samples from 391 individuals. Nonlinear mixed-effects modeling accounted for intraindividual and interindividual variabilities. Covariate effects were evaluated through forward inclusion and backward elimination, and the model performance was validated using goodness-of-fit plots and visual predictive checks. RESULTS: The fin

The effect of plasma exchange on serum levels of lacosamide: A case report.

PURPOSE: We describe the case of a 22-year-old male who developed thyroid storm necessitating therapeutic plasma exchange (TPE). The patient's past medical history was complicated by epilepsy, for which he took lacosamide. Little evidence was available to guide lacosamide dosing during TPE. Because of an exacerbation of the patient's underlying epilepsy in the context of the thyroid storm, we conducted therapeutic medication monitoring of lacosamide concentrations to guide management. SUMMARY: We arranged for measurement of the lacosamide concentration immediately before TPE (5.1 μg/mL) and 2

Lacosamide use during breastfeeding: A case report and a literature review.

Lacosamide, a voltage-gated sodium channel inhibitor, is an anti-seizure medication (ASM) from the age of 4. We report on the case of a woman treated with lacosamide for pharmacoresistant epilepsy who breastfed her infant for more than 6 months after birth. The infant's blood concentrations of lacosamide were 2.4 mg/L on Day 1 and less than 1 mg/L on Day 10 (reference values are 1-10 mg/L). No adverse drug reactions (ADRs) were reported for the infant. Eight cases of breastfeeding by mothers receiving lacosamide are described in the literature. These data confirm that a significant amount of l

DRESS is a Mess: A Case of Cross Reactivity Between Lacosamide and Lamotrigine.

Drug reaction with eosinophilia with systemic symptoms (DRESS) syndrome is a rare drug reaction often presenting with both cutaneous manifestations and potentially life-threatening internal organ involvement. The precise incidence of DRESS is still unclear as it is easily missed due to its highly variable clinical presentation. However, with an expected mortality rate of approximately 10 percent, it is important for clinicians to be familiar with pharmacologic etiologies commonly implicated in the pathogenesis. We present a case of DRESS syndrome attributed to cross-reactivity between two comm

Cardiac effects of rapid intravenous loading of lacosamide in patients with epilepsy.

BACKGROUND: Lacosamide (LCM) is a new antiseizure medication, and intravenous (IV) loading of LCM is recently used against status epilepticus. IV loading of LCM is usually well-tolerated; however, there are concerns about LCM-induced serious adverse cardiac events. This study was aimed at investigating whether rapid IV loading of LCM is associated with adverse cardiac or hemodynamic events in cases of epilepsy emergencies in real-world settings. METHODS: We reviewed medical records of consecutive adult epilepsy patients who received a single loading dose (400 mg) of IV LCM between January 201

The effects of lacosamide on cognitive function and psychiatric profiles in patients with epilepsy.

BACKGROUND: Cognitive and psychiatric problems are common in people with epilepsy. They can have multiple causes, including structural brain lesions, the active epilepsy, and the effect of anti-epileptic therapy. Since patients' treatment compliance and quality of life are affected by cognitive and emotional status, it is crucial for clinicians to understand how anti-seizure medications (ASMs) affect cognition and mood, and to choose the proper ASM. OBJECTIVE: To conduct a literature review of the impact on cognition and mood status of lacosamide (LCM) in people with epilepsy. METHODS: Wesea

Improvement of epilepsy with lacosamide in a patient with ring chromosome 20 syndrome.

BACKGROUND: Ring chromosome 20 syndrome is a rare chromosomal disorder characterized by refractory seizure, mental retardation, and behavioral problems. Although there are reports of the effective treatment of patients with antiepileptic drugs (AEDs), no study has reported the effects of lacosamide(LCM) in children with this syndrome. We report a 7-year-old boy with this syndrome whose refractory and behavioral abnormalities have been remarkably improved by treatment with LCM. CASE PRESENTATION: The patient was a 7-year-old boy with no medical or family history of epilepsy. He developed epile

[Lacosamide associated with high-degree block in a patient with trigeminal neuralgia].

INTRODUCTION: Lacosamide is an antiepileptic drug whose exact mechanism of action remains unknown. It acts by increasing the slow inactivation of the voltage-dependent sodium channels of the cell membranes. It is indicated in the treatment of focal seizures with or without secondary generalisation and is occasionally used as adjunct treatment in neuropathic pain. Although the most frequent side effects are mild (dizziness, diplopia, blurred vision, headache, tremor, etc.), others such as supraventricular tachyarrhythmias, changes in repolarisation, atrioventricular blocks and even cardiac arre

Lacosamide-induced excessive laughing in a patient with Lennox-Gastaut syndrome.

Lacosamide is one of the third-generation antiseizure drugs that block voltage-gated sodium channels by enhancing slow inactivation. The most common adverse effects of lacosamide include dizziness, headache, nausea, vomiting, diplopia, fatigue, and sedation. Less common side effects include memory impairment, weight gain, rash, and atrioventricular block. In this article, we describe a patient with Lennox-Gastaut syndrome who developed excessive laughing as a rare side effect of lacosamide with complete resolution after discontinuation of the medication. The present case illustrates that exces

Lacosamide use in the treatment of refractory epilepsy in tuberous sclerosis complex.

Lacosamide (LCS) was approved by the United States Food and Drug Administration (FDA) in 2008 as adjunctive therapy to other anti-epileptic drugs (AEDs) to treat focal-onset seizures, with or without secondary generalization. Its role in the treatment of epilepsy in individuals with tuberous sclerosis complex (TSC) has yet to be determined. This study evaluates LCS treatment of focal-onset refractory epilepsy in patients with TSC. From November 2009 to June 2014, 46 TSC patients followed by a single neurologist were treated with LCS. Forty-eight percent were responders (seizure reduction ≥50%)
