Unusual side effects of lamotrigine therapy.

An 8-year-old boy developed tremor, unsteadiness, chorea, and eye movement abnormalities on starting lamotrigine for myoclonic jerks. Investigations for a neurodegenerative disorder were negative. Symptoms and signs resolved on stopping lamotrigine. He was well and asymptomatic on follow-up after 4 years. Another 7-year-old boy who started on lamotrigine for suspected absence seizures developed abnormalities of eye movement with associated cognitive decline, which also resolved on discontinuing the medication. Eye movement abnormalities, involuntary movements, and behavioral changes appear to

[Toxic epidermal necrolysis due to lamotrigine].

A 10-year-old girl developed a progressive rash and high fever more than a month after the start of lamotrigine as add-on medication for therapy-resistant epilepsy. A skin biopsy indicated toxic epidermal necrolysis (Lyell syndrome). Because her situation deteriorated and the lesions of the skin and mucosa progressed, she was transferred to a specialised centre for burn patients. Despite maximal supportive treatment, she died 3 weeks after the first skin lesions. Even with a low initial dosage of lamotrigine followed by a slow increase, a fatal toxic epidermal necrolysis is still possible. Thi

Lamotrigine-induced severe cutaneous adverse reactions.

PURPOSE: We systematically reviewed and analyzed published and unpublished cases of Stevens-Johnson syndrome (SJS), or toxic epidermal necrolysis (TEN) associated with lamotrigine (LTG) therapy to identify characteristics of these reactions. METHODS: We performed a MEDLINE search (January 1985 to April 1998) and citation tracking for published reports. In addition, reports were requested from the Uppsala Monitoring Centre of the World Health Organization (WHO). Published and WHO cases of LTG-associated SJS or TEN were included if the causal relationship was assessed as either possible, probab

Suspected lamotrigine-induced toxic epidermal necrolysis.

OBJECTIVE: To describe a patient who developed toxic epidermal necrolysis (TEN) possibly secondary to lamotrigine use. CASE SUMMARY: A 74-year-old white man with a history of probable complex partial seizures was admitted to the neurology service for a prolonged postictal state. His antiepileptic regimen was changed while he was in the hospital to include lamotrigine. After 19 days of hospitalization and 14 days of lamotrigine therapy, the patient became febrile. The next day he developed a rash which progressed within 4 days to TEN, diagnosed by skin biopsy. All suspected drugs were disconti

Lamotrigine in rapid-cycling bipolar disorder.

BACKGROUND: We evaluated the antidepressant and mood-stabilizing effects of lamotrigine, a novel anticonvulsant, in a group of rapid-cycling bipolar patients. Most were already nonresponders or poor partial responders to other conventional mood-stabilizing agents. METHODS: This open, naturalistic, and prospective study was conducted with five rapid-cycling bipolar patients (DSM-IV). Each received lamotrigine titrated to a minimum dose of 150 mg/day as monotherapy or in combination with other psychotropic agents. Patients were assessed with the Global Assessment Scale (GAS), Beck Depression In

[Stevens-Johnson syndrome after lamotrigine treatment].

We report a 18 years old female with Lennox-Gastaut syndrome under treatment with sodium valproate, carbamazepine and clonazepam. When seizures increased we stopped carbamazepine and introduced lamotrigine slowly. One month later the girl developed haemorrhagic erosions in mucoses and limbs with deterioration of her general state. Skin biopsy confirmed the diagnosis of erythema multiforme, the Stevens-Johnson's form. The immediate withdrawal of lamotrigine and treatment with antihystaminics and steroids was followed of a slowly favourable course with disappearance of symptomatology one month l

Lamotrigine, Contraceptives, and Psychiatry: A Narrative Review.

Lamotrigine is widely used in neurology and psychiatry, particularly in epilepsy and mood disorders, due to its favorable efficacy and safety profile. However, clinically relevant pharmacokinetic interactions between lamotrigine and hormonal contraceptives have been increasingly recognized, raising concerns regarding therapeutic stability and reproductive safety in women of reproductive age. This narrative review examines the bidirectional interactions between lamotrigine and hormonal contraceptive methods, focusing on their pharmacokinetic mechanisms, clinical consequences, and implications f

Sinus Bradycardia in an Adolescent Taking Lamotrigine.

Lamotrigine is commonly prescribed for the treatment of neurological conditions and is increasingly being prescribed for psychiatric conditions as well. Although largely well tolerated, it is known to have a number of potential side effects, and in March 2021, the FDA issued its most recent warning for the medication due to its increased risk of cardiac arrhythmias. In this report, we describe a case in which an adolescent patient was found to be bradycardic after starting lamotrigine for antidepressant augmentation, with a gradual return to normal heart rate as the medication was subsequently

Rechallenge of lamotrigine after development of rash.

Lamotrigine (LTG) is associated with the potential for a life-threatening rash (eg, Stevens-Johnson syndrome or toxic epidermal necrolysis). The incidence has been linked to rapid titration and an interaction with valproic acid that can increase the level of LTG. Providers often have difficulty discriminating between serious versus benign rashes, and the package insert recommends discontinuing the medication at the first sign of a rash. Therefore, many patients end up being taken off LTG when it may have been effective for them. We present a case where LTG is reintroduced with a faster initial

Brugada pattern in a patient medicated with lamotrigine.

The authors report the case of a 52-year-old woman with depressive syndrome, treated with lamotrigine for about five months, who went to the emergency department for atypical precordial pain. The electrocardiogram (ECG) revealed a 2-mm downsloping ST-segment elevation and negative T waves in V1 and V2. Due to suspicion of ST-elevation acute coronary syndrome, cardiac catheterization was performed, which revealed normal coronary arteries. The initial ECG was suggestive of type 1 Brugada pattern, but subsequent serial ECGs were less typical. A flecainide test showed the same pattern. After disco

The effect of lamotrigine on valproic acid concentrations.

PURPOSE: To determine whether lamotrigine affects serum concentrations of valproic acid. METHODS: Pre-morning-dose serum valproic acid concentrations were measured in 76 subjects with epilepsy (48 M, 28 F, age range 6-20 years, mean age 14 years) in whom lamotrigine was added while the dose of valproate and other medication remained unchanged. In a comparison group, either acetazolamide or gabapentin was added to sodium valproate. RESULTS: Far more subjects (26/76 = 34%) had an increase of >25% in valproic acid concentration with lamotrigine than those who had a decrease of >25% (4/76 = 5.3%

Lamotrigine administration in panic disorder with agoraphobia.

Several anticonvulsants, although as yet not lamotrigine (LTG), have been found useful in the treatment of panic disorder with (PDA) or without agoraphobia. We administered LTG (200 mg/d) to 4 outpatients with PDA, as an augmentation therapy (3 patients with chronic and severe agoraphobia) or monotherapy (1 drug-naive patient with first-onset PDA) in a 14-week trial. The patient under LTG monotherapy improved significantly, whereas PDA symptoms in 2 of the other patients improved to some extent.

[Trichotillomania and comorbidity--lamotrigine in a new perspective].

Trichotillomania an impulse disorder that causes people to pull out the hair) is a relatively rare psychiatric illness, though the number of patients in need are definitely much higher than that we encounter in psychiatric practice. According to ICD-10 and DSM-IV-TR trichotillomania is an impulse disorder but latest researches in neurobiology and picture taking procedures seem to provide convincing evidence of it's being a relative of addictions. The article is a glimpse into the core of the newest literature also focusing on therapeutical solutions. The therapeutical effects of mode stabilize

[Lamotrigine in the treatment of bipolar disorder, a review].

BACKGROUND: Lamotrigine is being prescribed increasingly for the treatment of bipolar disorder. In this article we review the current literature on the subject. AIM: To review the literature in order to assess the present position of lamotrigine in the treatment of bipolar disorder. METHOD: We reviewed the literature from 1985 to 2005 by means of PubMed and we also consulted the bibliographic references for the articles we found. In addition, we studied a review article by Goldsmith e. a. (2003). RESULTS: Although a considerable amount of research has been done, evidence for the efficacy of

Adverse reactions of Topiramate and Lamotrigine in children.

PURPOSE: To review the adverse drug reactions (ADRs) of Topiramate and Lamotrigine among children in Israel, and to compare the two drugs, based on their side effect profile and tolerability among this population. METHODS: We performed a cross-sectional study. Four paediatric neurologists from three different tertiary medical centres in Israel documented all cases of children from birth to the age 18 years, treated with Topiramate and/or Lamotrigine in their respective outpatient clinics and hospital wards. All present ADRs and their characteristics were recorded. RESULTS: Reports on 45 and
