Laryngeal collagen injection as an adjunct to medialization laryngoplasty.

OBJECTIVES/HYPOTHESIS: Dysphonia associated with laryngeal paralysis may be identified in the short term postoperatively or may develop years after successful medialization laryngoplasty. In selected cases, laryngeal collagen injection permits further medialization of one or both vocal folds by small increments to improve phonation after medialization thyroplasty. The study seeks to determine whether collagen injections result in measurable improvements in voice quality and vocal function when offered to select patients who have received medialization thyroplasty. STUDY DESIGN: Retrospective

Laryngoplasty: results and problems after the creation of a neoglottis.

Voice rehabilitation in the laryngectomy patient has been a major goal of reconstructive laryngeal surgery. 36 patients underwent laryngectomy with the creation of a phonatory neoglottis as outlined by Staffieri. In 13 patients good voice rehabilitation was achieved. In 23 patients who failed to develop voice production, altered swallowing function was the major problem.

Laryngeal Electromyography-Guided Injection Laryngoplasty: A Prospective Series.

OBJECTIVE: Laryngeal electromyography (LEMG) is a well-established method for botulinum toxin (Botox) injections and has also been advocated for the treatment of vocal fold paralysis. We hypothesize that LEMG may also help with the therapeutic injection of other substances. We report on 64 subjects and 118 LEMG-guided injection laryngoplasty procedures. Fiber-optic laryngoscopy was performed after injection to assess the validity of this approach. MATERIAL AND METHODS: Between 2020 and 2026, 64 subjects were selected for LEMG-guided injections, totaling 118 vocal fold injections. Inclusion cr

Laryngeal spasm and tracheostomy after hydroxyapatite injection laryngoplasty.

INTRODUCTION: Adductor spasmodic dysphonia (Ad-SD) is usually treated by botulinum toxin injection into the thyroarytenoid muscles. In older patients, atrophy of these muscles is responsible for glottic leak, causing presbyphonia and is managed by vocal fold medialization using autologous fat or hydroxyapatite (HA). We report the first case of uncontrollable laryngeal spasm requiring tracheostomy after hydroxyapatite injection laryngoplasty in a patient with spasmodic dysphonia and presbyphonia. CASE REPORT: An 83-year-old man had been treated for Ad-SD by botulinum toxin injections every six

Voice aerodynamics following office-based hyaluronate injection laryngoplasty.

OBJECTIVES: Neuromuscular control, glottal conformation and aerodynamics are the major factors affecting voice performance. We aimed to characterise the degree to which voice improvements following office-based intracordal hyaluronate injection laryngoplasty (HIL) depend on changes in voice aerodynamics in patients with unilateral vocal fold paralysis (UVFP), by assessing the correlations between these parameters. DESIGN: Prospective case series. SETTING: Otolaryngology Clinics in a Medical Center. PARTICIPANTS: Patients with UVFP within 6 months of their first outpatient visit who received

Voice outcome measures after flexible endoscopic injection laryngoplasty.

OBJECTIVE: To report voice outcome measures after injection laryngoplasty using the transnasal or transoral flexible endoscopic technique. METHODS: A retrospective review of all patients who underwent flexible endoscopic injection laryngoplasty between June 2010 and August 2016 was carried out. Only those patients who had pre- and post-injection voice outcome measures recorded were included. Voice outcome measures recorded included perceptual voice evaluation using GRBAS, Voice Handicap Index-10 (VHI-10), maximum phonation time (MPT) and closed quotient (CQ) before and after treatment. Calcium Hydroxylapatite Pulmonary Embolism after Percutaneous Injection Laryngoplasty.

Injection medialization laryngoplasty is a procedure that has many advantages in treating vocal fold paralysis; however, undesired complications can occur. We experienced a case of a pulmonary embolism, suspected in a patient who had undergone an injection laryngoplasty with calcium hydroxylapatite (CaHA). The patient suffered dyspnea after undergoing the injection laryngoplasty. Chest embolism computed tomography (CT) scan revealed a new lesion of enhancing materials at the pulmonary vasculature in the right upper lobe. The CaHA embolism was suspected, and the patient was treated with warfari

Polydimethylsiloxane for injection laryngoplasty: two cases necessitating tracheotomy.

The surgical treatment of glottic insufficiency due to lesions of the recurrent laryngeal nerve has become a routine procedure in the last few decades. In particular, injection laryngoplasty with polydimethylsiloxane (PDMS) has proved to be an easy, effective and safe method for vocal fold medialization. It is a biologically inert substance having almost ideal properties as a filler; complications related to its intralaryngeal use such as migration, or granuloma formation are extremely rare and allergic reactions have not been reported as yet. We discuss two cases representing the first descri

Laryngeal abscess after injection laryngoplasty with micronized AlloDerm.

OBJECTIVE: Patients with unilateral vocal cord paralysis usually present with dysphonia and occasionally with swallowing problems. Operative management includes thyroplasty type I, injection laryngoplasty, arytenoid adduction, and reinnervation. Recent publications have documented the safety of micronized AlloDerm (Cymetra, LifeCell Corporation, Branchburg, NJ) for injection laryngoplasty, but we report the first documented case of a laryngeal abscess after injection laryngoplasty with Cymetra. STUDY DESIGN: Single case report of a laryngeal abscess after injection laryngoplasty with Cymetra.

Contralateral laryngoplasty after supraglottic laryngectomy with vertical extension.

Eight patients with supraglottic carcinoma extending onto an arytenoid or true vocal cord have undergone supraglottic laryngectomy with vertical extension including the resection of an arytenoid. In these patients, the contralateral superior thyroid cornua was used to reconstruct the resulting defect. In this technique, the thyroid cornua is mobilized and green-stick fractured across the posterior commissure, maintaining its blood supply by leaving the inferior and middle pharyngeal constrictor attached. This muscle-cartilage pedicle flap provides bulk for the posterior glottis and can be anch

Multimodal Shortwave Infrared Imaging for Visualization of Injection Laryngoplasty.

OBJECTIVE: Injection laryngoplasty relies on accurate placement of filler, which can be difficult to visualize in the visible light spectrum (400-700 nm). Shortwave infrared (SWIR, 1000-2000 nm) wavelengths offer advantages due to increased penetration depth and reduced scattering. The study aims to perform a multispectral characterization of the larynx in the SWIR and to provide accurate visualization of injection laryngoplasty via SWIR imaging techniques. STUDY DESIGN: Basic science. SETTING: Laboratory. METHODS: Larynges were prepared from fresh porcine samples and preserved human cadave

Injection medialisation laryngoplasty: an alternative approach for challenging cases.

OBJECTIVE: Conventional injection medialisation laryngoplasty techniques may be compromised by patient-specific factors such as marked kyphosis, an anteriorly positioned larynx or intolerance to nasendoscopy. This paper describes a technique for successful injection medialisation laryngoplasty where conventional methods are precluded, in an 88-year-old man with presbyphonia on a background of Parkinson's disease. METHODS: After induction of general anaesthesia, a transoral introducing needle, shaped by tactile manipulation to match the curvature of a 'C-MAC' intubating video-laryngoscope 'D-b

Injection Laryngoplasty for Management of Neurological Vocal Fold Immobility.

Injection laryngoplasty as used in this chapter is synonymous with the term "injection augmentation." Injection augmentation is a technique designed to enhance glottic closure in patients with glottic insufficiency, or failure of glottic closure, by injecting material into the lateral aspect of the vocal fold to move the vibrating surface to the midline. This type of injection augments the vocal fold and improves glottic closure. Injection augmentation originated over 100 years ago. However, the current indications, techniques, and materials have changed significantly. Paralysis, defined as lo
