Outcome of lateral internal sphincterotomy for anal fissure in a retrospective cohort of patients.

OBJECTIVE: Chronic anal fissures (CAF) is a common cause of severe anorectal pain in adults causing poor quality of life. In case of failure of conservative treatments, lateral internal sphincterotomy (LIS) is an effective cure. We aimed to analyse the outcomes of LIS in a consecutive patients' series operated in a high volume Surgical Coloproctology Unit. METHODS: A retrospective cohort study was conducted on 422 patients with CAF undergoing surgical treatment between January 2015 and December 2019 at our Surgical Coloproctology Unit. Patients over the age of 18, not responding or recurring

The Role of Lateral Internal Sphincterotomy in Haemorrhoidectomy: A Study in a Tertiary Care Center.

Background Hemorrhoids are a common condition that presents with bleeding per rectum, pain at rest and defecation, mucosal discharge, and prolapse. Surgical hemorrhoidectomy is the treatment method of choice for Grade 3 and Grade 4 hemorrhoids. Hemorrhoidectomy is associated with postoperative pain and no single surgical technique has been proved to significantly reduce the pain. We analyzed in our study the effect of lateral internal sphincterotomy with hemorrhoidectomy on postoperative pain, anorectal function, and retention of urine after the Milligan and Morgan technique. Methods This rand

Subcutaneous internal lateral sphincterotomy (SILS) versus nitroglycerine ointment in anal fissure: a prospective study.

AIM: To compare the results of subcutaneous internal lateral sphincterotomy under local anesthesia and nitroglycerin ointment treatments in acute and chronic anal fissures. METHODS: This was a comparative, prospective study of 340 patients of acute and chronic anal fissure. The patients voluntarily opted either for the surgical procedure under local anesthesia, or the nitroglycerin treatment. All the patients were followed with regular checkups and complaints were documented. RESULTS: Except for 1.75% patients having bleeding/hematoma formation, and with a dropout of 2.60% patients, no failu

Subcutaneous lateral internal sphincterotomy (SLIS)--a safe technique for treatment of chronic anal fissure.

OBJECTIVE: Subcutaneous lateral internal sphincterotomy (SLIS) is an effective treatment for fissure in ano but carries a definite risk of incontinence. The aim of this study was to assess the efficacy and complications of SLIS in patients with chronic fissure in ano. MATERIALS AND METHODS: All patients presenting with a chronic anal fissure who underwent SLIS were entered into a prospective database. This is a review of these patients over 5 year's period (September 2002-2007). All operations were performed or directly supervised by a consultant colorectal surgeon. Short-term follow-up was a

The chronic anal fissure treatment by internal partial lateral sphincterotomy and following anal manometry.

Anal fissure remains one of the most common proctologic problems. A number of reports have advocated the use of partial internal sphincterotomy as a treatment of chronic anal fissure. Our study shows the results of a retrospective analysis of our patients who underwent lateral internal sphincterotomy for the treatment of chronic anal fissure. To determine long time results we examined random sample of 75 operated patients. Apart from taking careful history and patient's assessment of the operation effect, the patients were investigated "per rectum", and even possibly rectoscopically. To asscer

Effect of lateral sphincterotomy on internal anal sphincter function. A computerized vector manometry study.

PURPOSE: This study was designed to investigate the effect of lateral sphincterotomy on internal anal sphincter function in patients with chronic anal fissure. METHODS: Using an eight-channel perfusion catheter and computerized data analysis, a prospective manometric study was performed on patients with chronic anal fissure undergoing lateral sphincterotomy (LS). RESULTS: Mean resting pressure (MRP) in patients with anal fissure (85.1 mmHg) was significantly higher (P = 0.012) than control subjects (63.3 mmHg). One week following LS there was a significant reduction in MRP (50.0 mmHg; P = 0.

Chronic fissure-in-ano: a randomized study comparing open and subcutaneous lateral internal sphincterotomy.

A prospective study comparing open and subcutaneous lateral internal sphincterotomy for chronic anal fissure was conducted. One hundred twelve patients were randomized to open (n = 54) or subcutaneous (n = 58) sphincterotomy. There was no significant difference in acute complications between the subcutaneous (8.6 percent) and open (7.4 percent) groups. Postoperative length of stay was significantly shorter for the subcutaneous group (1.7 +/- 0.2 days) than for the open group (2.3 +/- 0.1 days; P less than 0.001). Although the response rate to a pain questionnaire was less than 50 percent, the

Open haemorrhoidectomy combined with lateral internal sphincterotomy versus conventional haemorrhoidectomy: a prospective randomized comparative study.

BACKGROUND: Open haemorrhoidectomy is an effective treatment for haemorrhoidal disease, but it is associated with significant postoperative pain and early bleeding. Lateral internal sphincterotomy (LIS) has been recommended as an adjunctive procedure to reduce sphincter spasm and facilitate post-operative healing. This study was designed to compare postoperative pain and bleeding in patients treated with an open haemorrhoidectomy with or without a lateral internal sphincterotomy. METHODS: This prospective, randomized, comparative study was conducted at Baghdad Teaching Hospital from March 2024

Literature review of the role of lateral internal sphincterotomy (LIS) when combined with excisional hemorrhoidectomy.

BACKGROUND AND AIM: Pain following hemorrhoidectomy is a distressing sequel to the procedure. Various methods have been used to alleviate post-hemorrhoidectomy pain; among these methods is the lateral internal sphincterotomy (LIS). This review aimed to analyze all studies that evaluated the impact of LIS on the outcome of excisional hemorrhoidectomy. PATIENTS AND METHODS: Seventeen studies were included after organized search of the literature using electronic databases including PubMed/Medline and EMBASE. The studies included comprised 2180 patients with median age of 44 years. Variables sel

Unilateral versus bilateral lateral internal sphincterotomy: a randomized controlled trial for chronic fissure in ano.

BACKGROUND: Standard treatment for Chronic Anal Fissure (CAF) is unilateral Internal Sphincterotomy (UIS). Still there is recurrence and the risk of of incontinence. Bilateral Internal Sphincterotomy (BIS) as a treatment for CAF has not been adequately evaluated. METHODS: A prospective randomized controlled trial of UIS at 3 o'clock compared with BIS at 3 and 9 'O' clocks. The outcome variables were: post operative pain, recurrence and incontinence. RESULT: There was 104 and 107 cases in UIS and BIS group respectively with similar age and sex. There were 12 recurrences in UIS and only one in

Ultra slow wave pressure variations in the anal canal before and after lateral internal sphincterotomy.

Ultra slow waves (USW's) in the anal canal are discrete pressure fluctuations with a low frequency (1-2/minute) and high amplitude (> or = 10% above or below baseline resting pressure). To investigate the nature of these USW's, anorectal manometry was performed in 20 control subjects as well as in 58 patients presenting with anal fissure or symptomatic hemorrhoids, before and 2 weeks after lateral internal sphincterotomy. USW's could be demonstrated in two control subjects and in 29 patients. The median value of maximum anal resting pressure (MARP) in the two control subjects with USW's was si

Lateral subcutaneous internal sphincterotomy for anal fissure. Technic and experience with 22 cases in children.

Lateral sphincterotomy is probably the operation of choice for most cases of intractable anal fissure in children. The lower half of the sphincter is divided in the left or right lateral position through a small external incision at the anal verge. An incision in the anal canal itself is avoided, postoperative pain is minimal and the risk of secondary hemorrhage is eliminated. The operation can be done as an outpatient procedure. Only one patient developed minor anal incontinence.

Laser Fissurectomy Combined with Limited Lateral Internal Sphincterotomy for Chronic Anal Fissure: A Minimally Invasive Technique.

Chronic anal fissure (CAF) is a common benign anorectal condition characterised by severe pain and internal anal sphincter (IAS) hypertonia. Although lateral internal sphincterotomy (LIS) remains the surgical gold standard, the risk of postoperative faecal incontinence remains a clinically significant concern. We describe a minimally invasive hybrid technique combining 1470-nm diode laser fissurectomy with a limited closed LIS. The laser ablates the chronic ulcer triad whilst a tailored sphincterotomy confined to the distal one-third of the IAS addresses the underlying hypertonia. Based on pub

Minimal Lateral Internal Sphincterotomy (LIS): Is It Enough to Cut Less Than the Conventional Tailored LIS?

PURPOSE: Anal fissure is a common anorectal condition, yet its pathogenesis remains unclear. Lateral internal sphincterotomy (LIS) is the gold standard treatment for chronic anal fissures that do not respond to conservative treatment; however, it has a risk of anal incontinence. We believe that fibrosis of the internal anal sphincter is an important factor in the pathogenesis of chronic anal fissure. In this study, we describe the minimal LIS method, a minimally invasive method where only the fibrotic portion of the internal anal sphincter is cut. We also describe the outcomes of this method.
