BACKGROUND: Although abortion is technically legal in Zambia, the reality is far more complicated. This study describes the process and results of galvanizing access to medical abortion where abortion has been legal for many years, but provision severely limited. It highlights the challenges and successes of scaling up abortion care using implementation science to document 2 years of implementation.
METHODS: An intervention between the Ministry of Health, University Teaching Hospital and the international organization Ipas, was established to introduce medical abortion and to address the lack

The ability to receive gender affirmation within legal and medical contexts has positive impacts on transgender and gender-diverse (TGD) population health. Yet, legal gender affirmation, which involves obtaining gender-concordant identity documents, remains restricted in several US states that require documentation of medical or surgical interventions. Meanwhile, some gender-affirming medical treatments can compromise long-term reproductive potential, leading to iatrogenic infertility; however, insurance coverage of fertility services for TGD individuals is limited. Inability to access fertili

INTRODUCTION: Individuals with opioid use disorder (OUD) in the criminal-legal system commonly present co-occurring mental health disorders. However, evidence-based treatment for high-risk populations such as those with co-occurring disorders is often unavailable within jails and prisons. Coordination of timely and affordable access to behavioral health treatment following incarceration is critical to address the multidimensional needs of people with co-occurring needs. However, the role of co-occurring disorders among adults with OUD and criminal-legal involvement who are accessing community-

PURPOSE: Our study's goal was to investigate the individuals who took a pre-military examination and were pronounced "unfit for military service," in terms of etiological grouping, legal blindness, and whether or not the illness could have been prevented.
METHODS: The files of 174 individuals deemed "unfit for military service" due to eye disease at the State Hospital Ophthalmology Department between January 2018 and January 2022 were retrospectively evaluated. The disorders were classified as refractive error, strabismus, amblyopia-related, congenital, hereditary, infectious/inflammatory, de

BACKGROUND: About one quarter of women in Bangladesh are denied menstrual regulation (MR) due to advanced gestation [J Fam Plann Reprod Health Care 41(3):161-163, 2015, Issues Brief (Alan Guttmacher Inst) (3):1-8, 2012]. Little is known about barriers to MR services, and whether women denied MR seek abortion elsewhere, self-induce, or continue the pregnancy.
METHODS: After obtaining authorization from four health facilities in Bangladesh, we recruited eligible and interested women in to the study and requested informed consent for study participation. We conducted in-depth interviews with 20

The protracted process involved in family disruption provides the opportunity for different types of mental health intervention. In addition to the traditional diagnostic and evaluative services typically requested by the legal system, preventive and therapeutic programs can be developed. Implementation of a program by the UCLA Section on Legal Psychiatry is described.

BACKGROUND: Criminal legal system-involved individuals with serious mental illness (SMI) experience more challenges accessing mental health and other community services than those without a history of criminal legal system involvement. A formative qualitative study was conducted to explore feasibility and acceptability and inform the adaptation of a mental health peer navigation intervention for individuals with SMI reentering the community after jail incarceration.
METHODS: In-depth qualitative interviews and focus-group discussions were conducted with mental health peer navigators (i.e., ce

People with HIV (PWH) often encounter health-harming legal needs that impede their access to care, including structural issues such as racism, discrimination, unstable housing, and stigma. Medical-Legal Partnerships (MLPs) have emerged as a promising strategy to address these challenges within HIV care settings. This study aimed to identify the characteristics and strategies of MLPs that are most effective in improving HIV care continuum outcomes. A mixed-methods analysis was conducted utilizing data from a cross-sectional survey of 60 providers in MLPs. Categorical features of MLPs, such as t

INTRODUCTION: "Person-centred" and "people-centred" HIV prevention programmes both seek to scale up access to HIV prevention services. A "person-centred" approach presents a vision of a client with agency in decision-making, engaged and empowered, working with providers in a process that is not disease-centric but focused on addressing, holistically, a client's needs. A "people-centred" approach recognizes the broader role of family and community, as well as the influence of the political and legal environment as barriers or facilitators to HIV services. In both cases, human rights are a criti

INTRODUCTION: People living with HIV (PLWH) often face health-harming legal needs (HHLNs), including housing instability, discrimination, immigration concerns, benefit denials and interpersonal violence, which can undermine engagement in care and health outcomes. Medical-legal partnerships (MLPs) integrate legal services into healthcare settings to address these drivers of health inequities and improve access to HIV prevention and care.
METHODS: This study presents mixed-methods implementation findings from three federally funded MLP initiatives serving PLWH in New York City, Philadelphia, Or

Over 1 billion people across low-income and middle-income countries live in urban informal settlements, where these residents face severe health disparities. In this Viewpoint, we argue that the absence of legal recognition in informal settlements is a foundational cause of the causes of ill health, as the absence of recognition contributes to unsafe housing and inadequate access to water, sanitation, and other life-sustaining basic services. Despite the importance of legal recognition, few public health researchers have explored how legal barriers influence health outcomes in these communitie

BACKGROUND: Individuals with intellectual disability (ID) and legal involvement are a unique population with complex needs. To date, there has been limited research exploring the demographic and clinical profiles of individuals with ID and legal involvement that are in crisis and how they differ to individuals with ID without legal involvement.
METHOD: 130 adults with ID and a history of legal involvement were compared to 617 without legal involvement who had experienced at least one crisis in terms of crisis presentation and outcome.
RESULTS: Overall, those with a known history of legal inv

PURPOSE: The landscape of the medico-legal risk in emergency medicine in Canada is unknown. The objective of this study was to describe and analyze factors associated with medico-legal risk in cases in the emergency department (ED).
METHODS: We conducted a descriptive analysis of closed medico-legal cases (hospital complaints, regulatory authority (i.e., College) complaints, and civil-legal actions) involving physicians practicing primarily emergency medicine in Canadian EDs. We used data from closed cases involving the Canadian Medical Protective Association from January 2018 until December

The Middle East and North Africa (MENA) region is one of the few areas experiencing an increase in HIV prevalence, attributed to insufficient political commitment, stigma, and the criminalization of key populations (KPs). This study provides a human rights-based analysis of the experiences of KPs and individuals living with HIV (PLHIV). Using a convergent mixed-methods design, data were collected through an online survey (N=100) and qualitative interviews/focus groups (N=38) across Arabic-speaking MENA. Findings indicate that participants perceive their socio-legal environments as hostile, lea

Penal code was revised in Rwanda in 2012 allowing legal termination of pregnancy resulting from rape, incest, forced marriage, or on medical grounds. An evaluation was conducted to assess women's access to abortion services as part of an ongoing program to operationalize the new exemptions for legal abortion. Data was collected from eight district hospitals; seven gender-based violence (GBV) centers and six intermediate courts. Three focus group discussions and 22 in-depth interviews were conducted with key informants. At hospitals, of the 2,644 uterine evacuation records (July 2012-June 2014)

CONTEXT: Although Colombia partially liberalized its abortion law in 2006, many abortions continue to occur outside the law and result in complications. Assessing the costs to the health care system of safe, legal abortions and of treating complications of unsafe, illegal abortions has important policy implications.
METHODS: The Post-Abortion Care Costing Methodology was used to produce estimates of direct and indirect costs of postabortion care and direct costs of legal abortions in Colombia. Data on estimated costs were obtained through structured interviews with key informants at a randoml
