Involuntary Patient Assessment in Australia: A Mental Health or Public Health Response?

Involuntary assessment relates to detaining and transporting a person at risk of harming themselves or others, and without their consent, to hospital for examination and treatment. State and Territory statutory authorities generally allow police, paramedics and/or health practitioners to initiate involuntary assessment. Because of the stigma attached to mental illness, and to protect people from harming themselves or others in broader circumstances than mental illness alone, the Queensland government changed involuntary assessment powers. Instead of mental health legislation governing involunt

Evidence-informed obstetric practice during normal birth in China: trends and influences in four hospitals.

BACKGROUND: A variety of international organizations, professional groups and individuals are promoting evidence-informed obstetric care in China. We measured change in obstetric practice during vaginal delivery that could be attributed to the diffusion of evidence-based messages, and explored influences on practice change. METHODS: Sample surveys of women at postnatal discharge in three government hospitals in Shanghai and one in neighbouring Jiangsu province carried out in 1999, repeated in 2003, and compared. Main outcome measures were changes in obstetric practice and influences on provid

Safety and efficacy of early medical abortion at home between 10+0 and 11+6 weeks' gestation: a retrospective review.

INTRODUCTION: Since 2020, legislation in Britain has allowed both mifepristone and misoprostol to be taken at home for early medical abortion (EMA). In England and Wales, legislation restricts this to 9+6 weeks, but in Scotland this is permitted up to 11+6 weeks. Data on the outcomes of EMA at home >10 weeks' gestation is limited and so we aimed to establish the safety and efficacy of this in comparison with EMA in a hospital setting. METHODS: We conducted a retrospective review from our abortion service in Edinburgh, Scotland of outcomes of EMA at home versus hospital between 10+0 and 11+6 w

[The birth of the Italian workers' compensation authority and its contribution to improving health conditions of workers and general population].

In 1898 the compulsory work accident insurance was introduced in the Italian legislation: both public and private organizations could provide insurance to the workers. In 1933 the Cassa Nazionale Infortuni (founded in 1884) was unified to other small public funds in a single body, the Italian Workers' Compensation Authority (INAIL). During the post-war recovery INAIL founded hospitals and wards fully dedicated to work traumas (Orthopaedic Traumatic Centres or Burns Centres) and opened rehabilitation and prosthetic centres for injured workers. In this view, INAIL and Italian National Olympic Co

Pediatric golf cart trauma: Not par for the course.

INTRODUCTION: Golf cart trauma in southeast Georgia represents a significant source of morbidity in the pediatric population. We believe these events are related to the introduction of new state legislation that allows local authorities to govern golf cart operation. METHODS: We performed a retrospective review from 2010 to 2016 of children involved in golf cart traumas (n = 46). We recorded age, gender, Glasgow Coma Scale score (GCS), Injury Severity Score (ISS), location of event, and patient position during event. Outcomes included injury type and length of stay (LOS). Pasteurisation and the control of milkborne infection in Britain.

Infections carried in milk, particularly salmonellosis and campylobacter enteritis, have continued to feature in Great Britain in recent years. Less commonly reported infections included an outbreak in 1984 in England due to Streptococcus zooepidemicus, in which 12 people, eight of whom died, were admitted to hospital. The implementation of legislation in 1983 requiring heat treatment of cows' milk for sale to the public reduced the incidence of milkborne infection in Scotland compared with previous years and compared with England and Wales, where, without legislative control, outbreaks contin

Changes in Secondhand Smoke Exposure After Smoke-Free Legislation (Spain, 2006-2011).

INTRODUCTION: In 2011, the Spanish partial smoke-free legislation was extended to affect all enclosed settings, including hospitality venues and selected outdoor areas. This study evaluated the change in self-reported exposure to secondhand smoke among the adult, nonsmoking population. METHODS: Two cross-sectional surveys were conducted on nationally representative samples of the adult (≥18 years) nonsmoking Spanish population. One was conducted in 2006 (6 months after the first ban) and the other in 2011, 6 months after the new ban was implemented. We assessed the prevalence and 95% confiden

Child pedestrian injuries in the United States. Current status of the problem, potential interventions, and future research needs.

Each year in the United States, more than 50,000 children are injured as pedestrians, of whom approximately 1800 die, 18,000 are admitted to the hospital, and 5000 have significant long-term sequelae. Prevention must rest on a multifaceted approach at the local, state, and national level and should include pedestrian skills training programs, parent education, legislation, environmental modifications, and vehicle design changes.

Port Moresby road traffic accident survey.

Road Traffic Accidents (RTA's) are an important cause of morbidity and mortality in Papua New Guinea today. A survey of casualties from RTA's attending Port Moresby General Hospital over a seven-month period from November 1982 to June 1983 showed that of 209 patients seen, one-third were in cars and one-quarter were in utilities. The most dangerous positions in vehicles were (1) in the open back of a utility or truck, and (2) in the front seat (driver or passenger) of a car or utility. Only 13% of injured front-seat occupants of cars or utilities were wearing a seat-belt; in one-third of vehic

Acute cannabis intoxication in the emergency department: the effect of legalization.

BACKGROUND: On October 17, 2018, the Cannabis Act decriminalized the recreational use of cannabis in Canada. This study seeks to determine how legalization of cannabis has impacted emergency department (ED) visits for acute cannabis intoxication. METHODS: We conducted a retrospective chart review at an academic ED in Hamilton, Ontario. We assessed all visits with a cannabis-related ICD-10 discharge code 6 months before and after legalization (October 17, 2018) to determine cases of acute cannabis intoxication. The primary outcome was the rate of ED visits. Secondary outcomes included number o

Training and activities of pharmacist prescribers in a California pilot project.

The training of pharmacists allowed to prescribe under protocol in California health manpower pilot projects (HMPPs) and the specific program at San Francisco General Hospital (SFGH) are described. From 1972 to 1983, several bills were passed allowing pharmacists to prescribe and administer drugs or devices in HMPPs under specified conditions. The main objective of the HMPPs was to train pharmacists to assist physicians and other providers in providing good follow-up care for chronically ill patients. Programs for pharmacists consisted of a training phase followed by an employment phase in whi

Impact of legislative changes on patterns of antipsychotic prescribing and self-poisoning in Scotland: 2000-06.

Recently, national guidelines have advocated greater use of atypical rather than typical antipsychotics in the treatment of schizophrenia. In addition, there have been safety concerns regarding the potential cardiotoxicity of certain antipsychotics taken in overdose. This has led regulatory authorities in the United Kingdom to restrict the use of thioridazine. The overall impact of these legislative changes on patterns of antipsychotic prescribing has received comparatively little attention. Therefore, we sought to examine the effects on community prescribing practices, and to determine whethe

Induced abortion: epidemiological aspects.

Sir Dugald Baird sketches the history of abortion legislation in Great Britain from the beginning of the century. In his views the 1967 Abortion Act has been one of the most important and beneficial pieces of social legislation enacted in Britain in the last 100 years. It has, however, brought problems both of administration in the hospitals and to individual doctors and nurses, particularly when the patients are young single women and even schoolgirls. One of the consequences of the Abortion Act has been a fall in maternal mortality and perinatal mortality rates. Abortion does not seem to be
