Evaluation of leukocyte depletion of packed red blood cells for the prevention of clinically observed transfusion reactions at a medical center in Eastern Taiwan.

OBJECTIVES: The incidence of febrile nonhemolytic transfusion reactions (FNHTRs) is correlated with the level of cytokines released by donor leukocytes in blood bags during storage, which is the most common transfusion reaction. The study aimed to reveal whether the use of leukocyte-poor red blood cells (LPRBCs) can reduce the incidence of transfusion reactions to promote patient safety. MATERIALS AND METHODS: From January 2014 to June 2022, 158,122 blood transfusion reports were collected from a medical center in Eastern Taiwan. Data were categorized into three groups according to usage: pre

CD8-depleted donor leukocyte transfusions for cytomegalovirus antigenemia in patient with acute lymphoblastic leukemia treated with allogeneic CD34(+) cell transplantation.

A 24-year-old man with acute lymphoblastic leukemia received an allogeneic CD34(+) cell transplant from an HLA-mismatched sibling because of refractory disease. The CD34(+) cells were enriched by the immunomagnetic method. One month after the transplant his situation became complicated due to cytomegalovirus (CMV) antigenemia, which was resistant to treatment with ganciclovir. He was treated with CD8(+) cell-depleted donor lymphocyte transfusions (CD8-depleted DLT). After CD8-depleted DLT, the CMV antigenemia disappeared completely. This case report suggested that CD8-depleted DLT was an effec

Donor leukocyte transfusions for treatment of leukemic relapse after bone marrow transplantation. EBMT Immunology and Chronic Leukemia Working Parties.

Allogeneic bone marrow transplantation is an effective treatment of leukemia. Intensive chemo- and radiotherapy used for conditioning and T-cells of the graft contribute to the control of leukemia. Animal experiments indicate that transfusion of lymphocytes from the marrow donor convert into complete chimerism without producing graft-versus-host disease, if delayed for two months or more. Transfusion of donor leukocytes (DLT) after marrow transplantation has induced lasting remissions in the majority of patients with chronic myelogenous leukemia (CML) in hematological or cytogenetic relapse, s

Regeneration of blood-forming organs after autologous leukocyte transfusion in lethally irradiated dogs. I. Distribution and cellularity of the bone marrow in normal dogs.

Marrow cellularity in adult beagles (1-2 yr old) is highest in centrally located bones, with values between 8000 and 12,000 nucleated cells per sq mm. It decreases gradually towards the peripheral parts of the body, reaching values below 1000 per sq mm in bones distal to the elbow and knee. The first tail segment always contains some active marrow. The fifth segment has only stromal elements. In spongy bones fat cells appear to be distributed at random among the blood-forming elements. In the middle part of the femur the fatty marrow predominates in the center of the cylinder, while the subend

Glanzmann's thrombasthenia during pregnancy complicated by large subchorionic hematoma managed with antifibrinolytics, human leukocyte antigen-matched platelet transfusion, and primary cesarean delivery: a case report.

A 29-year-old primipara with Glanzmann's thrombasthenia presented for prenatal care at 8 weeks of gestation. Pregnancy remained uncomplicated until 22 weeks of gestation when a subchorionic hematoma, measuring 5.8 × 4.1 × 6.7 cm, was diagnosed and managed outpatient. At 28 weeks of gestation, the subchorionic hematoma was significantly expanding to 11 × 13 × 3.7 cm (∼30% of the placental surface). The patient was admitted for antepartum surveillance and steroid treatment. Fetal and maternal status were reassuring. At 36 weeks of gestation, there was active extravasation from the subchorionic h

Cytokine mRNA and protein expression in a mixed leukocyte reaction before and after allogeneic transfusions. Groupe Coopératif de Transplantation d'Ile de France.

BACKGROUND: The precise mechanism by which pretransplant blood transfusions may favorably influence the graft outcome in human transplantation remains unknown. Here, we explored whether the mechanism might be related to an alteration of cytokine response to transplantation antigens. METHODS: Eight patients awaiting kidney transplantation were selected to receive a single planned pretransplant blood transfusion. Before transfusion and 7 days after transfusion, peripheral blood mononuclear cells from these patients were isolated and in vitro stimulated in a one-way mixed leukocyte reaction (MLR

Relapse of chronic myeloid leukemia after allogeneic bone marrow transplant: the case for giving donor leukocyte transfusions before the onset of hematologic relapse.

Fourteen patients with chronic myeloid leukemia (CML) relapsing after allogeneic bone marrow transplant (BMT) were treated with leukocyte transfusions from the original marrow donor (sibling, n = 9; volunteer unrelated, n = 5). The relapse was defined at the molecular level in two cases, cytogenetically in five cases and hematologically in seven cases. Ten patients responded, seven of seven patients with cytogenetic/molecular relapse compared with three of seven with hematologic relapse (P < .03). All five recipients of cells from unrelated donors responded. Eight of the 10 responders have ach

Induction of persistent allograft tolerance in the rat by combined treatment with anti-leukocyte function-associated antigen-1 and anti-intercellular adhesion molecule-1 monoclonal antibodies, donor-specific transfusion, and FK506.

We previously reported that a short course of treatment with anti-LFA-1 and anti-ICAM-1 monoclonal antibodies (mAbs) led to a persistent acceptance of mouse cardiac allografts, which resulted from the induction of allospecific tolerance. In the present study, we tested the effect of anti-LFA-1 and anti-ICAM-1 mAbs on rat allograft rejection and analyzed the mechanisms underlying allograft tolerance. In sharp contrast to the mouse case, a short course of treatment with anti-LFA-1 and anti-ICAM-1 mAbs led to a persistent acceptance in only half of the treated rats when MHC was compatible but mis

Efficacy of granulocyte transfusions collected from nonstimulated donors.

Series of leukocyte transfusions were given to adult patients with previously untreated acute non-lymphocytic leukemia during the granulocytopenic phase of remission induction therapy with doxorubicin and cytosine arabinoside. Peripheral blood leukocytes for transfusion were obtained by continuous flow centrifugation from unstimulated normal persons. Forty-five consecutive patients were eligible to receive leukocyte transfusions depending upon the availability of the blood cell separator for leukapheresis of donors and, to a lesser extent, clinical need. A series of leukocyte transfusions was

HLA alloimmunization with leukocyte concentrates from HLA-matched and HLA-non-matched donors in patients with Hunter's syndrome.

The incidence and characteristics of HLA alloimmunization following transfusions of leukocyte concentrates as a source of enzyme replacement were determined in male patients with Hunter's syndrome. Five patients were given leukocyte concentrates from HLA matched donors (Group I) and another five patients received leukocyte concentrates from non-HLA matched donors (Group II). No other blood products were transfused in either group. Immune response pattern of HLA alloimmunization measured as the proportion of screening cells manifesting cytotoxicity with patient's sera obtained during the follow

Consequences of prior alloimmunization during granulocyte transfusion.

The transfusion of leukocyte-containing blood products can lead to the production of antibodies to antigens on the surface of leukocytes. Such antibodies can be detected by a variety of techniques including assays for lymphocytotoxicity, granulocytotoxicity, and leukoagglutination. In order to evaluate the effect of preformed anti-leukocyte antibodies during granulocyte transfusion therapy, recipient beagles were sensitized to donor foxhound antigens. After being made granulocytopenic with cyclophosphamide, these animals were transfused with a set dose of granulocytes collected by continuous f

Management of patients with hematologic malignancies and aplastic anemia who are refractory to platelet transfusions.

Patients with hematologic malignancies or aplastic anemia may have reduced responses to platelet transfusions after multiple transfusions of standard red blood cells or platelet components. This situation, conventionally described as 'refractoriness' to platelet transfusions, may result from immune or non-immune causes. Non-immune causes include fever, infections, hypersplenism, disseminated intravascular coagulation, antibiotics, or veno-occlusive disease. Immune causes include platelet-reactive alloantibodies, which are typically antibodies to human leukocyte antigens (HLA) or, less commonly

Universal leukocyte reduction.

Leukocyte reduction of blood components, in the United States, is generally reserved for conditions in which a clinical indication has been documented. There is no evidence that either Creutzfeldt-Jakob disease or variant Creutzfeldt-Jakob disease are transmitted by transfusion in humans or that leukocyte reduction of blood components could reduce their transmission. A number of adverse outcomes following transfusion are alleged to be the result of white blood cells. At this point in time, there are insufficient clinical data to justify the universal leukocyte reduction of blood components.

Transfusion-related adult respiratory distress syndrome.

Leukocyte antibodies are the major cause of nonhemolytic transfusion reactions. One of the less frequent but severe forms of this reaction is the adult respiratory distress syndrome, or transfusion-related acute lung injury. This rare phenomenon is the result of leukocyte antibodies forming immune complexes with granulocytes, complement activation, sequestration of activated granulocytes in the pulmonary capillary bed, and their degranulation associated with release of proteolytic and cytotoxic substances. This cascade causes endothelial damage with increased capillary permeability. Plasma flu

The clinical benefits of the leukoreduction of blood products.

Many adverse events associated with the transfusion of allogeneic blood products have been shown to be related to the presence of allogeneic leukocytes in the blood product transfused. Until recently little attention has been paid to the leukocytes present in various blood components, however, over the past two decades it has been shown that the removal of such "passenger" leukocytes is associated with improved clinical outcomes. These include: the reduction in the incidence and severity of febrile transfusion reactions; reducing the CMV transfusion transmission risk; reducing the risk of allo

[Transfusion-Related Acute Lung Injury a serious, underdiagnosed transfusion-related event].

UNLABELLED: Transfusion-Related Acute Lung Injury (TRALI) has been diagnosed very rare until recent years. However, the growing interest in TRALI allows to asses, that it is the second commonest cause of transfusion association death. In Poland only single cases of TRALI have been published. AIM: We analyzed 34 cases with dyspnea reported as a post-transfusion event and examined leukocyte antibodies, which are supposed to be an important pathogenic factor in TRALI. RESULTS: 34 patients were classified into: the group A--patients with a pulmonary oedema after exclusion of other reasons (TRALI
