Lumbar Spinous Process-Splitting Technique for Ligamentum Flavum Cyst Removal.

BACKGROUND: Laminotomy for lumbar stenosis is a well-defined procedure and represents a routine in every neurosurgical department. It is a common experience that the uni- or bilateral paraspinal muscle detachment, together with injury of the supra- and interspinous ligaments, can lead to postoperative pain. In the literature, the application of a minimally invasive technique, the lumbar spinous process-splitting (LSPS) technique, has been reported. METHODS: In this study, we present a case series of 12 patients who underwent LSPS from September 2019 to April 2020. Two patients had a cyst of t

Full-Endoscopic Resection of Ligamentum Flavum Cyst in Lumbar Spine.

BACKGROUND: Cysts of the ligamentum flavum (LF) are rare occurrences, with only a few cases reported in literature. They are a result of age-related degenerative changes in the LF. The cyst compresses the neural elements and causes neurogenic symptoms. Endoscopic resection of the LF cyst has not been described in literature. In this report, we describe our experience with endoscopic resection of an LF cyst in the L3-L4 region. CASE DESCRIPTION: A 54-year-old female patient presented with claudication in both lower limbs and evidence on magnetic resonance imaging of a LF cyst in the L3-L4 regi

Measurements of ligamentum flavum thickening at lumbar spine using MRI.

INTRODUCTION: As the ligamentum flavum (LF) covers most of the posterolateral part of the lumbar spinal canal, its thickening can be attributed to the development of lumbar canal encroachment. Nevertheless, there have been few reports describing the natural history of the LF. METHOD: To investigate the natural history and to subsequently clarify the pathogenesis of LF thickening, we conducted a transverse radiological study of the LF at the lumbar spine using magnetic resonance images. PATIENTS: One hundred and sixty-two patients complaining of low back pain and/or leg pain were evaluated (n

Spontaneous haematoma of ligamentum flavum. Case report and literature review.

Spontaneous infarction of the ligamentum flavum is a very rare cause of mielo-radicular compression. In the literature only four cases are reported, all characterized by a clinical history of slowly progressive mielo-radiculopathy and good outcome after surgical treatment. A 70 year-old female patient presented with a four months clinical history of spontaneous, sub-continuous, progressive lumbar pain with bilateral irradiation to the L4-L5 dermatomers, right leg monoparesis and hypoaesthesia affecting tactile, thermal and pain sensivity, urinary incontinence and constipation. CT scan and MRI

Comparative Gene-Expression Analysis of the Ligamentum Flavum of Patients with Lumbar Spinal Canal Stenosis: Comparison between the Dural and Dorsal Sides of the Thickened Ligamentum Flavum.

Thickening of the ligamentum flavum is the main factor in the development of lumbar spinal canal stenosis (LSCS). Although previous studies have reported factors related to ligamentum flavum thickening, its etiology has not been clarified. Furthermore, it is often difficult to set proper controls to investigate the pathologies of thickening due to differences in patient characteristics, such as age, sex, obesity, and comorbidities. This study aimed to elucidate the pathologies of ligamentum flavum thickening by comparing the dural and dorsal sides of the thickened ligamentum flavum in patients

Thoracic ligamentum flavum ossification causing progressive myelopathy: a case report.

BACKGROUND: Ossified ligamentum flavum (OLF) in the thoracic segments is a rare but significant cause of progressive myelopathy characterized by spinal canal stenosis. While OLF is commonly reported in East Asian populations, it remains under-recognized in other regions, including India. CASE PRESENTATION: We present a case of a 60-year-old Indian male laborer who developed progressive lower-limb weakness, sensory deficits, and immobility over several months. This case is notable for its occurrence in an Indian patient, within a population in which OLF remains significantly underreported, and

Exploring lncRNA Expression Patterns in Patients With Hypertrophied Ligamentum Flavum.

OBJECTIVE: Hypertrophy ligamentum flavum (LFH) is a common cause of lumbar spinal stenosis, resulting in significant disability and morbidity. Although long noncoding RNAs (lncRNAs) have been associated with various biological processes and disorders, their involvement in LFH remains not fully understood. METHODS: Human ligamentum flavum samples were analyzed using lncRNA sequencing followed by validation through quantitative real-time polymerase chain reaction. To explore the potential biological functions of differentially expressed lncRNA-associated genes, Gene Ontology (GO) and Kyoto Ency

Thoracic ossification of ligamentum flavum manifesting holocord syringomyelia: case report.

It has been reported that syringomyelia is rarely associated with degenerative spinal disorders, but the case of holocord syringomyelia is never reported. We here present a case of a 59-year-old woman with right shoulder pain, dysesthesia of the right hand, and gait disturbance. Radiographically, examinations of the spine demonstrated holocord syringomyelia with ossification of ligamentum flavum at T2/3 level. Holocord syringomyelia was reduced remarkably after posterior decompression at the T2/3 level, and her symptoms also improved. We speculated that holocord syringomyelia might have develo

Matrix metalloproteinase promotes elastic fiber degradation in ligamentum flavum degeneration.

Ligamentum flavum (LF) hypertrophy in lumbar spinal canal stenosis (LSCS) is characterized by a loss of elastic fibers and fibrosis. Chronic inflammation is thought to be responsible for the histological change but the mechanism underlying elastic fiber degradation remains unclear. Given that matrix metalloproteinase (MMP)-2 and -9 have elastolytic activity and are partly regulated by inflammatory cytokines such as interleukin (IL)-6, in this study, we investigated whether MMPs mediate LF degeneration using 52 LF samples obtained during lumbar surgery, including 31 LSCS and 21 control specimen

Herniated intervertebral disk induces hypertrophy and ossification of ligamentum flavum.

STUDY DESIGN: In vitro experiment using degenerated human ligamentum flavum (LF) and herniated intervertebral disk (IVD). OBJECTIVES: To investigate the role and effect of degenerated and herniated IVDs on LF hypertrophy and ossification. SUMMARY OF BACKGROUND DATA: Spinal stenosis is caused, in part, by hypertrophy and ossification of the LF, which are induced by aging and degenerative process. It is well known that degenerated IVDs spontaneously produce inflammatory cytokines. Therefore, we hypothesized that degenerated IVD may affect adjacent LF through secreted inflammatory cytokines. M

Case of ossification of the ligamentum flavum causing cauda equina compression.

BACKGROUND: Ossification of the ligamentum flavum (OLF) is characterized by heterotopic bone formation within the ligamentum flavum. It typically causes progressive, chronic spinal canal stenosis, but only rarely acute cauda equina syndromes. CASE DESCRIPTION: A 29-year-old patient presented with cauda equina syndrome characterized by saddle numbness, urinary incontinence, and bilateral foot drop. She underwent an urgent decompression for a disc herniation, at which time the surgeons encountered severe OLF, causing cauda equina compression. After performing a computed tomography scan to asses
