In addition to the well known changes in bone structure, myelofibrosis can result in extramedullary tumour formation; in the gastr-intestinal tract, proliferation of reticulum can lead to fibrotic changes. This results in appearances similar to linitis plastica or Crohn's disease. In the discussion, the authors refer to tumours in other organs which have been described in the literature and which can be demonstrated radiologically.

We describe five patients who were initially thought to have Crohn's disease and were treated accordingly. The original diagnosis was based upon clinical presentation, roentgenograms, and histological examination, but subsequent follow-up showed that diagnosis to be in error. The following diagnoses were established instead: tuberculosis, Actinomyces Israeli infection, reaction to gold therapy, metastatic cancer, and linitis plastica. We stress the importance of considering conditions that can mimic Crohn's disease.

Signet-ring cell carcinoma of the urachus and bladder is a rare lesion. Its histological picture is unique and similar to linitis plastica of stomach. The tumor arises through metaplasia of the totipotent transitional epithelium lining the urachus. It is a highly malignant tumor with propensity for local invasion and distant metastasis, and carries a uniformly poor prognosis. Herein we report one such rare case of urachal carcinoma who is free of disease for 2 years after surgical treatment.

Differential diagnostic aspects of colon stenoses are discussed using the example case of a female patient presenting with multilocular colon metastases, who had lobular breast cancer 9 years previously. Typical is linitis plastica, which can indicate tumorous infiltration not only of the stomach, but also of the large intestine. Other endoscopic imaging and histological studies may, however, fail. The pathologist requires the anamnestic data relating to the breast cancer for exact assignment of the tumorous infiltration.

We present the case of a 55-year-old man who was admitted with new abdominal ascites and change in stool caliber. The colonoscopy examination revealed a severe stricture with inflamed friable mucosa measuring 8 cm in length in the rectosigmoid colon. Histologically, poorly differentiated adenocarcinoma with signet ring cells was seen. The patient underwent esophagogastroduodenoscopy (EGD), which was suggestive of linitis plastica of the stomach. On microscopic examination, biopsy reported poorly differentiated adenocarcinoma with occasional signet ring cells as the primary source.

Combined chemotherapy has demonstrated a degree of efficacy in gastric carcinoma. As 5-fluorouracil (5FU) and cisplatinum are two of the most active drugs, we have tested the efficacy of combined 5FU and cisplatinum in a prospective phase II trial. Nausea and vomiting in patients with advanced gastric malignancy and mechanical obstruction are distressing and difficult to manage. Gastric metastasis from breast cancer is often difficult to diagnose due to nonspecific symptoms and variable endoscopic findings.
