Care planning for long-term conditions – a concept mapping.

OBJECTIVE: This article focuses on approaches within clinical practice that seek to actively involve patients with long-term conditions (LTCs) and how professionals may understand and implement them. Personalized care planning is one such approach, but its current lack of conceptual clarity might have impeded its widespread implementation to date. A variety of overlapping concepts coexist in the literature, which have the potential to impair both clinical and research agendas. The aim of this article is therefore to explore the meaning of the concept of care planning in relation to other overl

Methicillin-resistant Staphylococcus aureus in long-term-care facilities.

Owing to a high prevalence of methicillin-resistant Staphylococcus aureus (MRSA) among residents, long-term-care facilities (LTCFs) have become substantial reservoirs of this microorganism. Few data on the natural history of MRSA colonization in this setting are available. The cumulative incidence appears to be approximately 20% per year, and more than half of carriers have persistent colonization. Several host-related factors, such as antibiotic use, invasive devices, and poor infection control practices, increase the risk of colonization. Clinical experience suggests that subsequent MRSA inf

Care of patients with long-term indwelling urinary catheters.

The complications and management of long-term indwelling catheters used for urinary retention and incontinence were reviewed. Research evidence from 1992 - 2002 was located through searches of CINAHL (38 articles), and Medline (89 articles). Fifty studies were critiqued for this review. The most common complications of long-term indwelling catheters are bacteriuria, encrustation, and blockage. Less common is the prevalence of bacteremia and renal disease. Risk factors for bacteriuria include female gender, older age, and long-term indwelling catheter use. Urinary white blood cells are the best

Stem cell transplantation: supportive care and long-term complications.

With increasing hematopoietic stem cell transplant (HSCT) activity and improvement in outcomes, there are many thousands of HSCT survivors currently being followed by non-transplant clinicians for their healthcare. Several types of late sequelae from HSCT have been noted, and awareness of these complications is important in minimizing late morbidity and mortality. Late effects can include toxicities from the treatment regimen, infections from immunodeficiency, endocrine disturbances, growth impairment, psychosocial adjustment disorders, second malignancies, and chronic graft-versus-host diseas

Long-term care & presidential politics: pushing for a plan.

For the past few years, policymakers have debated income security (Social Security) and health security (Medicare, patients' rights), while virtually ignoring the long-term care crisis. However, logically, long-term care should be at the core of these conversations because it is inextricably bound to both issues.

A microbiologic survey of long term care urinary catheters.

Culture and sensitivity data were collected on over 500 isolates from indwelling urinary catheters in 23 Nebraska long-term care facilities. Four percent of all nursing home patients had indwelling urinary catheters. The prevalence of bacteriuria in catheterized patients was 79%., and the most frequent isolates were E. coli, Proteus mirabilis, and Enterococcus species. Thirteen percent of catheterized patients were receiving systemic antibiotics, and 85% had at least one urinary bacterial isolate resistant to the antibiotic being administered. Antibiotic resistance correlated positively with a

Prescription and intake patterns in long-term and ultra-long-term benzodiazepine treatment in primary care practice.

Pharmacoepidemiological data show that long-term treatment accounts for a considerable part of benzodiazepine prescriptions in primary care practice. Prescription and intake patterns were, therefore, investigated in a study of 196 patients who had been treated with benzodiazepines for longer than six months by internists or general physicians in private practice. Patients were 64 years of age on average, half of them being 65 years or older. Females were in the majority (73.5%). The average duration of tranquilizer or hypnotic intake was 11.0 years, or 5.3 years for the current benzodiazepine

Frailty and outcomes in VA long-term care residents.

BACKGROUND: The Veterans Affairs Frailty Index (VA-FI) has been implemented in subgroups of U.S. Veterans. The objectives were to operationalize the VA-FI in long-term care residents and examine its associations with mortality, falls, and fractures. METHODS: This retrospective cohort study included Veterans ≥65 years who stayed ≥90 days in Community Living Centers (CLCs). We calculated the VA-FI by dividing the count of health deficits by 31. We categorized the residents in 4 groups: non/pre-frail, mildly, moderately and severely frail. Mortality, the first events of falls and fractures were

Preventing Aspiration Among Older Adults in Long-term Care.

BACKGROUND: More than 60% of older adults living in long-term care facilities are at higher risk for pneumonia, aspiration, and malnutrition due to oropharyngeal dysphagia. Survey findings at a long-term care facility serving veterans revealed variation in dysphagia-related care performance and knowledge gaps. This quality improvement project aimed to reduce aspiration and pneumonia rates by 25% and the daily incidence rate of abnormal vital signs, indicating microaspiration events. METHODS: Plan-Do-Study-Act was utilized in a long-term care facility serving veterans. Rates of aspiration, pne

Vaccines in Long-Term Care Settings: A Narrative Review.

BACKGROUND: Older people living in long-term care facilities represent a particularly vulnerable segment of the population, who disproportionately bear the burden of infectious diseases, as recently highlighted by the COVID-19 pandemic. SUMMARY: Older long-term care residents typically cumulate several risk factors for infection and experience serious life-threatening outcomes once infected. These common infections are often compounded by the collective living environment, where it is more difficult to contain the spread of infection. Moreover, the staff may represent an additional reservoir

Long-term Care of the Adult Liver Transplant Recipient.

While outcomes after liver transplantation have increased over the last two decades, this is primarily as a consequence of a reduction in early deaths and survival of those who survive the first 6 months has not significantly changed. Causes of premature death and graft loss include cardiovascular disease, renal impairment, malignancy and some infections. As the number of transplant recipients increase, care is being given by primary and secondary care clinicians. Management of the well patient is crucially dependent on careful assessment and where appropriate intervention, especially of cardi

Marfan syndrome: A therapeutic challenge for long-term care.

Marfan syndrome (MFS) is an autosomal dominant genetic disorder caused by mutations in the fibrillin-1 gene. Acute aortic dissection is the leading cause of death in patients suffering from MFS and consequence of medial degeneration and aneurysm formation. In addition to its structural function in the formation of elastic fibers, fibrillin has a major role in keeping maintaining transforming growth factor β (TGF-β) in an inactive form. Dysfunctional fibrillin increases TGF-β bioavailability and concentration in the extracellular matrix, leading to activation of proinflammatory transcription fa

Managing pain medications in long-term care: nurses' views.

The purpose of this study was to explore nurses' perceptions of their current practices related to administering pain medications to long-term care (LTC) residents. A cross-sectional survey design was used, including both quantitative and open-ended questions. Data were collected from 165 nurses (59% response rate) at nine LTC homes in southern Ontario, Canada. The majority (85%) felt that the medication administration system was adequate to help them manage residents' pain and 98% felt comfortable administering narcotics. In deciding to administer a narcotic, nurses were influenced by pain as

Interventions for improving mealtime experiences in long-term care.

Poor food intake in residents living in long-term care (LTC) homes is a common problem. The mealtime experience is known to be important in the multifactorial causes of food intake. Diverse interventions have been developed, implemented, and/or evaluated to improve the mealtime experience in LTC; it is possible that multicomponent interventions will have a greater benefit than single activities. To identify the range of feasible and potentially useful interventions for including in a multicomponent intervention, this scoping review identified and summarized 58 studies that described and/or eva
