The low indexes of metabolism intervention trial (LIMIT): design and baseline data of a randomized controlled clinical trial to evaluate how alerting primary care teams to low metabolic values, could affect the health of patients aged 75 or older.

BACKGROUND: Too-low body mass index (BMI), HbA1c% or cholesterol levels predicts poor survival. This study investigates whether e-mails about these low values, improve health of people older than 75 years. METHODS: LIMIT - an open label randomized trial - compares usual care to the addition of an e-mail which alerts the family physicians and nurses to low metabolic indexes of a specific patient and advises on nutritional and medical changes. PARTICIPANTS: Clalit Health Services (CHS) patients in the Northern and Southern Districts, aged ≥75 years with any of the following inclusion criteria:

Decrease of red blood cell filterability seen in intensive care I. The correlation of low erythrocyte filterability with mortality and its return to normal values in critically ill patients under parenteral nutrition.

Damage to the internal viscosity of red cell is correlated with changes in the surface area to volume ratio and to prelytic phenomena. Considerable changes in shape were noticed in red cells from shocked dogs and some critically ill patients in blood smear or fresh preparation, or after in vitro treatment with colloidal plasma substitutes or glucose, and so erythrocyte filterability was investigated in critically ill patients, given or not given parenteral nutrition. The results revealed an increase of the erythrocyte filtration time (EFT 1/2) in more than 50% of the critically ill patients an

A qualitative interview study of Australian physicians on defensive practice and low value care: "it's easier to talk about our fear of lawyers than to talk about our fear of looking bad in front of each other".

BACKGROUND: Defensive practice occurs when physicians provide services, such as tests, treatments and referrals, mainly to reduce their perceived legal or reputational risks, rather than to advance patient care. This behaviour is counter to physicians' ethical responsibilities, yet is widely reported in surveys of doctors in various countries. There is a lack of qualitative research on the drivers of defensive practice, which is needed to inform strategies to prevent this ethically problematic behaviour. METHODS: A qualitative interview study investigated the views and experiences of physicia

Trends in Low-Value Health Service Use and Spending in the US Medicare Fee-for-Service Program, 2014-2018.

IMPORTANCE: Low-value care, defined as care offering no net benefit in specific clinical scenarios, is associated with harmful outcomes in patients and wasteful spending. Despite a national education campaign and increasing attention on reducing health care waste, recent trends in low-value care delivery remain unknown. OBJECTIVE: To assess national trends in low-value care use and spending. DESIGN, SETTING, AND PARTICIPANTS: In this cross-sectional study, analyses of low-value care use and spending from 2014 to 2018 were conducted using 100% Medicare fee-for-service enrollment and claims da

De-implementing spinal injections in pursuit of value-based care: Rethinking pain relief.

Despite guideline recommendations to the contrary, patients experiencing low back pain are commonly exposed to low-value care. Spinal injections for chronic low back pain are a key example of this disconnect. Their persistence highlights the broader challenge of removing low-value practices in pain care. Low-value interventions rarely fade with the introduction of new treatments; rather, their continued use reflects entrenched reimbursement incentives, provider training norms, and patient expectations. This Focus Article applies implementation science concepts to examine spinal injections as a

Implications of the CMS Ambulatory Specialty Model for Low Back Pain for Interventional Pain Management: The Good, The Bad, And The Ugly.

Since the enactment of the Affordable Care Act (ACA) in 2010, the Centers for Medicare & Medicaid Services (CMS) have embraced value-based care as the organizing principle of national payment reforms. Yet, after more than a decade, the value-based movement has, at best, generated modest returns and at worst increased costs on patients and negatively impacted access. Pursuant to its authority under the ACA, CMS proposed the Ambulatory Specialty Model (ASM) in the CY 2026 Physician Fee Schedule rulemaking and subsequently finalized the model, establishing the agency's first mandatory alternative

Value, Access, and High-Quality Medical Oncology Care: Can We Have It All?

Health systems, oncologists, and patients face a crisis in value of cancer care that has several components: interpretation of meaningful benefit, cost, and affordability that limits access to treatment and effective delivery of evidence-based care. Value in health care has several definitions but can be defined as benefit/cost, which can be explicitly measured using cost-effectiveness analyses and used to shape coverage decisions for health systems. In addition to drug costs, costs can also include physical costs (ie, clinical toxicities and harm), productivity losses (from inability to work

Financial Waste is Patient Harm: Reducing Wasteful Healthcare Spending Increases Safety and Health.

Wasteful healthcare spending has more than economic costs-there are clinical costs as well. Waste increases out-of-pocket costs, delays necessary care, and worsens outcomes. We argue that, while most quality frameworks do not consider this patient harm, they should. Financial waste is clinically on par with medical errors and adverse events, as overtreatment and low-value care injure patients, care coordination failures lead to preventable complications and death, pricing failures cause delayed and complicated care, and low-value procedures cause complications. We reviewed the literature to ti

Clinical and Demographic Factors Linked to Low-Value Emergency Department Visits in Pediatric Patients With Spina Bifida.

PURPOSE: Identifying factors associated with emergency visits that could be delivered at lower cost sites may help guide population health strategies for pediatric patients with spina bifida. MATERIALS AND METHODS: Emergency department encounters (2016-2023) by patients with spina bifida (younger than 18 years) in the Pediatric Health Information System were identified. Absence of clinical and imaging charges was defined as low-value emergency visit. We used a control population of patients (younger than 18 years) with obstructive/reflux uropathy who presented for emergency department encount

High-volume endocrine surgeons perform thyroid surgery at decreased cost despite increased case relative value units.

BACKGROUND: Healthcare systems are transitioning to value-based payment models based on analysis of quality over cost. To gain an understanding of the relationship between surgeon volume and health care costs, we compared the direct costs of thyroidectomy performed by dedicated high-volume endocrine surgeons and low-volume surgeons within a large health care system. METHODS: We evaluated all thyroid surgeries performed within a single billing year at a single health care system. We defined high-volume surgeons as those who treated >50 thyroid cases yearly and compared them to low-volume surge

Understanding High Utilization of Unscheduled Care in Pregnant Women of Low Socioeconomic Status.

BACKGROUND: Pregnant high utilizers of unscheduled care may be at particular risk for poor perinatal outcomes, but the drivers of this association have not been explored from the patient perspective. PURPOSE: We sought to understand maternal preference for unscheduled hospital-based obstetric care to inform interventions and improve value of publicly funded care during pregnancy. METHODS: We conducted a comparative qualitative analysis of in-depth semistructured interviews. Low-income pregnant women presenting to an inner city hospital-based obstetric triage unit were purposively sampled, ca

Should we focus on sustainable thromboprophylaxis and avoid overuse of low-molecular-weight heparins?

Low-molecular-weight heparins (LMWHs) have become a near-routine prescription for venous thromboembolism (VTE) prevention in hospitalized patients in several parts of the world. Their use may be reinforced by embedded order sets and 'opt-out' defaults. However, the absolute clinical benefit of universal pharmacologic thromboprophylaxis for many low-risk medical inpatients appears small. Symptomatic VTE rates are low, mortality benefits with LMWH use are not clearly demonstrated, and any reductions in thrombotic events may be offset by bleeding complications. Several analyses suggest that a sub

Identifying Strategies to Reduce Low-Value Preoperative Testing for Low-Risk Procedures: a Qualitative Study of Facilities with High or Recently Improved Levels of Testing.

BACKGROUND: Healthcare agencies and perioperative professional organizations recommend avoiding preoperative screening tests for low-risk surgical procedures. However, low-value preoperative tests are still commonly ordered even for generally healthy patients and active strategies to reduce this testing have not been adequately described. OBJECTIVE: We sought to learn from hospitals with either high levels of testing or that had recently reduced use of low-value screening tests (aka "delta sites") about reasons for testing and active deimplementation strategies they used to effectively improv

High-value care for critically ill oncohematological patients: what do we know thus far?

The number of patients with cancer requiring intensive care unit admission is increasing around the world. The improvement in the pathophysiological understanding of this group of patients, as well as the increasingly better and more targeted treatment options for their underlying disease, has led to a significant increase in their survival over the past three decades. Within the organizational concepts, it is necessary to know what adds value in the care of critical oncohematological patients. Practices in medicine that do not benefit patients and possibly cause harm are called low-value prac
