Effectiveness of artemether-lumefantrine for treating uncomplicated malaria in low- and high-transmission areas of Ghana.

BACKGROUND: Artemisinin-based combination therapy (ACT) has been effective in the supervised treatment of uncomplicated malaria in Ghana. Since ACT usage is primarily unsupervised, this study aimed to determine the effectiveness of artemether-lumefantrine (AL) for treating malaria patients in two transmission settings in Ghana. METHODS: Eighty-four individuals with uncomplicated Plasmodium falciparum malaria were recruited from Lekma Hospital (LH) in Accra (low-transmission area; N = 28), southern Ghana, and King's Medical Centre (KMC) in Kumbungu (high-transmission area; N = 56), northern Gh

Efficacy and safety of artemether-lumefantrine for the treatment of uncomplicated falciparum malaria in mainland Tanzania, 2018.

BACKGROUND: The use of artemisinin-based combination therapy (ACT) is recommended by the World Health Organization for the treatment of uncomplicated falciparum malaria. Artemether-lumefantrine (AL) is the most widely adopted first-line ACT for uncomplicated malaria in sub-Saharan Africa (SSA), including mainland Tanzania, where it was introduced in December 2006. The WHO recommends regular assessment to monitor the efficacy of the first-line treatment specifically considering that artemisinin partial resistance was reported in Greater Mekong sub-region and has been confirmed in East Africa (R

In Vivo Efficacy and Parasite Clearance of Artesunate + Sulfadoxine-Pyrimethamine Versus Artemether-Lumefantrine in Mali.

Although artemisinin resistance has yet to be reported in Africa, surveillance of the efficacy of artemisinin-based combination therapies (ACTs) is warranted. Here, the efficacy of artesunate + sulfadoxine-pyrimethamine (AS + SP) and artemether-lumefantrine (AL) was evaluated in Mali. Randomized open-label comparative in vivo assay of AS + SP versus AL were carried out using the 28-day follow-up World Health Organization protocol. Patients with uncomplicated falciparum malaria and at least 6 months of age were recruited between October 2010 and January 2014. A subset of these patients was sele

Artesunate/Amodiaquine Versus Artemether/Lumefantrine for the Treatment of Uncomplicated Malaria in Uganda: A Randomized Trial.

BACKGROUND: In treating malaria in Uganda, artemether-lumefantrine (AL) has been associated with a lower risk of recurrent parasitemia, compared with artesunate-amodiaquine (AS/AQ), but changing treatment practices may have altered parasite susceptibility. METHODS: We enrolled 602 children aged 6-59 months with uncomplicated falciparum malaria from 3 health centers in 2013-2014 and randomly assigned them to receive treatment with AS/AQ or AL. Primary outcomes were risks of recurrent parasitemia within 28 days, with or without adjustment to distinguish recrudescence from new infection. Drug sa

[The management of therapeutic failure in a falciparum malaria patient under oral arthemether-lumefantrine therapy].

Malaria is a parasitic infection characterized by anemia, splenomegaly and periodic fever. This infection has a tendency to cause serious complications. Falciparum malaria could occur in our country as an imported case due to increasing intercontinental travel opportunities. The World Health Organisation (WHO) recommends arthemether combination treatment as a first line choice. Here we report a Turkish case admitted to the hospital with high fever, sweating and fatigue. He had been in Uganda for 6 months without prophylaxis. Plasmodium falciparum with an intense parasitic load was diagnosed. W

Plasmodium falciparum and Schistosoma mansoni coinfection and the side benefit of artemether-lumefantrine in malaria patients.

INTRODUCTION: The distribution of both malaria and schistosomiasis exhibits a large geographical overlap in tropical environments, particularly in sub-Saharan Africa. This part of the world currently harbours more than 85% of the estimated global burden of these diseases. Studies showed that artemisinin derivatives used for the treatment of malaria also have an antischistosomal effect. This study aimed to investigate the extent of malaria-schistosomiasis co-infection and the antischistosomal effect of artemether-lumefantrine when administered to treat falciparum malaria in Kemise, Northeast Et

Experimental Evolution of Reduced Susceptibility to Artemether-Lumefantrine in Plasmodium berghei Following Serial Drug Pressure.

Malaria remains a major global health problem, with uneven progress and the greatest disease burden clustered in sub-Saharan Africa. Antimalarial drug resistance continues to undermine malaria control efforts, with widespread resistance reported in Plasmodium falciparum, including reduced susceptibility to artemisinin-based combination therapies (ACTs). However, despite increasing concerns over the long-term effectiveness of ACTs, the processes through which resistance to artemether-lumefantrine (AL) partner drugs evolves and contributes to resistance against individual drug components remain

Lumefantrine quantification on pre-treated filter paper dried blood spots for clinical field trials.

Artemether-lumefantrine is the most widely used antimalarial treatment for uncomplicated Plasmodium falciparum malaria. Malaria predominantly affects populations in rural areas of tropical and subtropical regions. Drug resistance is a growing concern, and monitoring antimalarial efficacy remains crucial for detecting changes in treatment performance and tracking potential resistance trends. In this study, we present an efficient method for collecting blood samples in resource-limited settings, using dried blood spots on pre-treated filter paper, which enhances the stability of lumefantrine. Dose-Optimization of a Novel Co-Formulated Triple Combination Antimalarial Therapy: Artemether-Lumefantrine-Amodiaquine.

Artemisinin-based combination therapy (ACT) is the first-line therapy for uncomplicated falciparum malaria, but artemisinin resistance in Asia and now sub-Saharan Africa is threatening our ability to control and eliminate malaria. Triple-ACTs have emerged as a viable alternative treatment to combat declining ACT efficacy due to drug-resistant malaria. In this study, we developed and evaluated an optimal fixed-dose regimen of artemether-lumefantrine-amodiaquine through population pharmacokinetic modeling and simulation. Three published population-based pharmacometric models and two large cohort

In vitro evaluation of ganaplacide/lumefantrine combination against Plasmodium falciparum in a context of artemisinin resistance.

BACKGROUND: Ganaplacide, also known as KAF156, is among the new antimalarial drug candidates that have successfully reached Phase III clinical trials, and is proposed in combination with lumefantrine. This combination could replace the current front-line artemisinin-based combination therapies (ACTs) in case of Plasmodium falciparum resistance to both artemisinins and partner drugs. Indeed, the African continent, where the malaria burden is the highest, is currently experiencing worrying multiple emergences and spread of artemisinin resistance, which urges for the exploration of the antiparasi

India Needs to Consider Planning a Change to Artemether-Lumefantrine to Treat Plasmodium falciparum Malaria.

As the malaria elimination target draws closer for India, it must be ensured that the country's policies, strategies, and tools remain effective. Artemisinin-based combination therapies are the mainstay of Plasmodium falciparum malaria management. India has a differential standard therapy for uncomplicated falciparum malaria in the form of artemether-lumefantrine in its northeastern states and artesunate + sulfadoxine-pyrimethamine in the rest of the country. The clinical failure of artesunate + sulfadoxine-pyrimethamine in the northeast regions was attributed primarily to parasite resistance
