The immotile-cilia syndrome compared to other obstructive lung diseases: a clue to their pathogenesis.

Twentyfour adults, aged 19-47 years, with congenitally nonfunctioning cilia were investigated by clinical examination, radiography, spirometry, and in most cases by tracheobronchial clearance measurements and ultrastructural examination of respiratory tract cilia and/or sperm tails. They all suffered from chronic bronchitis, rhinitis and sinusitis, and most of them had bronchiectasis and a history of otitis. Most had spirometrically overt obstructive lung disease and four radiological signs of emphysema. Clinical profile, lung function and mucociliary clearance data were compared to correspond

Integrating the overlap of obstructive lung disease and obstructive sleep apnoea: OLDOSA syndrome.

Obstructive lung diseases (OLD) such as asthma and chronic obstructive pulmonary disease (COPD) are very prevalent conditions. Disease phenotypes (e.g. chronic bronchitis, emphysema, etc.) often overlap, and significant confusion exists about their optimal nosologic characterization. Obstructive sleep apnoea (OSA) is also a common condition that features bidirectional interactions with OLD. OSA appears to be more commonly seen in patients with OLD, perhaps as a result of shared risk factors, for example obesity, smoking, increased airway resistance, local and systemic inflammation, anti-inflam

Impact of obstructive lung disease on survival in patients with rheumatoid arthritis related interstitial lung disease.

Chronic obstructive pulmonary disease (COPD) and asthma are common comorbidities in rheumatoid arthritis related interstitial lung disease (RA-ILD), yet their impact on health outcomes is poorly understood. Using health services data from Ontario, Canada, we aimed to evaluate the effect of COPD, asthma and COPD/asthma overlap (ACO) on survival in patients with RA-ILD. Among 6470 RA-ILD patients 4437 (68.6%) had obstructive lung disease. COPD and ACO were associated with significantly increased mortality compared to RA-ILD with asthma and RA-ILD alone. These results highlight the burden and del

Untreated Obstructive Sleep Apnea in Interstitial Lung Disease and Impact on Interstitial Lung Disease Outcomes.

Subjects with interstitial lung disease (ILD) often suffer from nocturnal cough, insomnia, and poor sleep quality. Subjects with ILD and obstructive sleep apnea (OSA) seem to have relatively mild symptoms from sleep fragmentation compared to subjects with only ILD. The overlap of ILD, OSA, and sleeping hypoxemia may be associated with poor outcome, even though there is no agreement on which sleep parameter is mostly associated with worsening ILD prognosis. Randomized controlled trials are needed to understand when positive airway pressure (PAP) treatment is required in subjects with ILD and OS

[Anesthetic Management for Patients with Chronic Obstructive Lung Disease].

The -most common cause of COPD is cigarette smoking. We use mMRD (Modified British Medical Research Council), CAT (COPD Assessment Test) and GOLD classification of airflow limitation, to evaluate severity of patients with COPD before surgery and create plans to manage their anesthesia. Known COPD is an important patient-related risk factor for postoperative pulmonary complications. Relative risks of postoperative pulmonary complications have ranged from 2.7 to 6.0. Cessation of smoking for four to eight weeks prior to surgery decreases risk of postoperative pulmonary complications. Preoperativ

Management of chronic obstructive pulmonary disease beyond the lungs.

Chronic obstructive pulmonary disease (COPD) is an umbrella term that covers many clinical subtypes with clearly different pulmonary and extra-pulmonary characteristics, but with persistent airflow limitation in common. This insight has led to the development of a more personalised approach in bronchodilator therapy, prevention of exacerbations, and advanced treatments (such as non-invasive ventilation and lung volume reduction techniques). However, systemic manifestations and comorbidities of COPD also contribute to different clinical phenotypes and warrant an individualised approach as part

Chronic obstructive lung disease: from structure to pathology.

Chronic obstructive pulmonary disease (COPD) is a complex disease characterized by progressive and irreversible airway obstruction and ubiquitous chronic inflammation in the lung. It is the end result of accumulated damage to many parts of the lung by many types of noxious agents, a destructive exhaustion of the lung reserve. Recent increases in the incidence of COPD have occurred mainly in older age groups, in non-smokers, and in females. Pathologically, COPD is characterized by chronic inflammation which perpetuates and accumulates locally. Discrete areas interact with each other, in the loc

Progressive obstructive lung disease associated with microscopic polyangiitis.

Small airway involvement and progressive severe airflow obstruction are unexpected features in patients with microscopic polyangiitis. We report the case of a patient with microscopic polyangiitis and circulating anti-neutrophil cytoplasmic antibodies (ANCA), who developed pulmonary hyperinflation and airflow obstruction over a 7-yr period. Systemic manifestations of this vasculitis improved under corticosteriods and cyclophosphamid therapy, a treatment that did not influence either the very high level of anti-myeloperoxidase antibodies or the ventilatory impairment. Small airway involvement w

[Acetazolamide in hypercapnic chronic obstructive lung disease--a renaissance?].

The use of acetazolamide, a carbonic anhydrase inhibitor, in chronic obstructive pulmonary disease (COPD) remains controversial. A substantial improvement in blood gas values has been documented, with correction of metabolic alkalosis in COPD, in hypoxemic sleep apnea at high altitudes and in acute mountain sickness. This randomized, double-blind study examined the short and long term effects of acetazolamide (2 X 250 mg) on 14 patients with hypoxemia, hypercapnia and metabolic alkalosis (paO2 49 +/- 5.2 mm Hg, paCO2 50 +/- 3.6 mm Hg, base excess + 5.7 +/- 2.3). [Lung Volume Reduction for Chronic Obstructive Pulmonary Disease].

Chronic obstructive pulmonary disease (COPD) is increasing with population aging and currently ranks as the third leading cause of death worldwide, accounting for approximately three million deaths annually. COPD is characterized by irreversible airflow obstruction, mainly caused by reduced lung elastic recoil due to peripheral airway lesions and emphysema. This condition results in lung hyperinflation, causing dyspnea, reduced exercise capacity, decreased physical activity, and an increased risk of heart failure. For patients with severe COPD unresponsive to medical therapies, surgical interv

Complex obstructive lung disease - A diagnostic and management conundrum.

Rheumatoid arthritis (RA) is a common autoimmune disease most well-known for its inflammatory, destructive polyarthropathy. Extraarticular manifestations of the disease may involve the respiratory system, including interstitial lung disease, pleural disease, pulmonary vascular abnormalities, and airways disease. Smoking is highly prevalent in the RA population, and may even have a synergistic effect in disease development and progression. In the diagnosis of pulmonary disease, this presents a unique diagnostic and therapeutic challenge. We present a case of a woman in her 50s who presented for

Non-invasive biomarkers in exacerbations of obstructive lung disease.

BACKGROUND AND OBJECTIVE: Current methods of diagnosing exacerbations of asthma and chronic obstructive pulmonary disease (COPD) shed little light on their aetiology or pathophysiology. This study aimed to define the inflammatory biomarker profile of subjects with obstructive lung disease and to compare these with control subjects also with respiratory infections, using exhaled breath condensate (EBC) and induced sputum biomarker analysis. METHODS: EBC, induced sputum and C-reactive protein were collected from subjects with exacerbations of asthma (n = 28), exacerbations of COPD (n = 29) and

[Care of patients with exacerbation of chronic obstructive lung disease].

Exacerbations of chronic obstructive pulmonary disease are a common cause of hospital admission. They have a profound effect on the quality of life, disease progression and healthcare costs. The central event is amplified inflammation with many biomarkers who are not in clinical use. Severe dyspnea and large volume of purulent sputum are typical clinical presentation of exacerbation due to viral or bacterial infections. Bronchodilators and corticosteroids are the mainstay of pharmacologic therapy of exacerbation, antibiotics are useful in the presence of purulent sputum and other signs of resp

Identification of exacerbations in obstructive lung disease through biomarkers.

Inflammation has been identified as an important factor for disease exacerbation in obstructive lung disease. In this study, we used neutrophil and eosinophil counts as biomarkers for exacerbation in obstructive lung disease. We conducted a case-control study within a cohort of patients frequenting an outpatient clinic of Respiratory Medicine using data from the Utrecht Patient Oriented Database (UPOD). Cases were patients with a hospital admission for obstructive lung disease in 2005. For each case, one control patient was sampled from the same study base. We identified 143 cases (118 patient

Obstructive lung disease in children with mild to severe BPD.

BACKGROUND: Bronchopulmonary dysplasia (BPD) is a common cause of respiratory insufficiency in children born very premature. OBJECTIVES: The purpose of this study was to examine the impact of the severity of BPD on pulmonary morbidity at school age, as measured by conventional spirometry and impulse oscillometry. We also studied the association between changes in lung function and structural changes in the lungs of children with BPD via High-Resolution Computed Tomography (HRCT). Finally we studied the prevalence of atopy associated with BPD. METHODS: We studied 60 very low birth weight (VLB

Conventional chest physical therapy for obstructive lung disease.

Chest physical therapy (CPT) is a widely used intervention for patients with airway diseases. The main goal is to facilitate secretion transport and thereby decrease secretion retention in the airways. Historically, conventional CPT has consisted of a combination of forced expirations (directed cough or huff), postural drainage, percussion, and/or shaking. CPT improves mucus transport, but it is not entirely clear which groups of patients benefit from which CPT modalities. In general, the patients who benefit most from CPT are those with airways disease and objective signs of secretion retenti

Chronic obstructive pulmonary disease: effects beyond the lungs.

PURPOSE OF REVIEW: This article reviews the systemic effects of chronic obstructive pulmonary disease in general and the musculoskeletal effects and cachexia in particular. RECENT FINDINGS: Shift in muscle fiber type from I to II, reduction of capillary/mitochondria ratio, reduction of oxidative enzymes and oxidative capacity, alteration in amino acid profile, changes in respiratory muscle dynamics, activation of the ubiquitin-proteosome pathway with loss of contractile proteins are some of the factors found to cause muscle dysfunction in chronic obstructive pulmonary disease.
