Local lymph node micrometastasis in a patient with negative sentinel lymph node biopsies after lymphatic mapping with wide local excision of primary melanoma on the head/neck area.

Sentinel lymph node biopsy is widely accepted as a staging procedure for patients with cutaneous melanoma who are at risk for metastases. The complex lymphatic drainage pattern on the head and neck makes sentinel lymph node mapping more challenging compared with other sites. Here, we report a patient who underwent reexcision of the primary tumor site with lymphatic mapping on the right posterior auricular area. Four sentinel nodes were negative for melanoma metastasis; however, step sectioning of the wide local excision of the primary tumor site revealed a small lymph node with histopathologic

Diagnostic excision of the Rosenmüller's node. Screening for occult metastases before elective regional lymph node dissection in patients with lower limb melanoma?

Elective radical groin dissection was performed on 297 consecutive patients with high-risk melanoma of the leg, Anderson Stages I, IIA, IIIA. By separate histologic examination of the so-called "Rosenmüller's node," the other inguinal, and the external iliac lymph nodes, the diagnostic excision of the Rosenmüller's node was tested as a suitable mode of screening for metastases before a planned elective regional lymph node dissection. Eighty patients (27%) presented with what was histologically determined to be occult groin metastases. Rosenmüller's node was involved in 30 of these cases; in th

Comparative analysis on efficacy of lymph node sampling through whole node excision versus core needle biopsy: focusing on diagnostic yield for interpretation.

BACKGROUND: The interpretation of lymph node biopsy is influenced by factors inherent to the type of biopsy. In recent times, core needle biopsies (CNB) have increasingly been preferred over whole node excision (WNE) for sampling lymph nodes. PURPOSE: To compare the diagnostic efficacy of CNB and WNE in diagnosing lymph node pathologies and to evaluate the diagnostic challenges in interpreting CNB. MATERIAL AND METHODS: Lymph node biopsies (CNB and WNE) received over 5.5 years were analyzed using hematoxylin and eosin (H&E) stain and immunohistochemistry (IHC). The percentage of cases diagno

Number of Excised Lymph Nodes Has No Impact on Relapse and Survival in Patients With Stage III Melanoma.

BACKGROUND: Even though both the involvement of regional lymph nodes and the number of metastatic lymph nodes are regarded as major determinants of survival in cutaneous melanoma, the extent of node dissection has been analyzed as an independent prognostic indicator in only a few studies. This study aims to determine how the extent of lymph node excision (EN) might predict the disease relapse and survival in melanoma. METHODS: A total of 317 patients with stage III melanoma were included in the study and reviewed retrospectively. The patients were divided into 2 groups based on the number of

Survey of surgical management of malignant melanoma in Canada: optimal margins of excision and lymph-node dissection.

The feasibility of clinical trials involving the surgical management of primary melanoma was assessed through a survey of Canadian surgeons who treat this disease. Surgeons were polled as to their current practice regarding node dissection and margins of resection of the primary lesion. Their interest in randomized trials addressing these practices was also assessed. Two hundred and forty questionnaires were distributed to 20 centres across Canada. The total number of patients with melanoma seen annually by the 131 surgeons who responded was 790 - of the approximately 1000 new cases of melanom

Excision of mesenteric lymph nodes alters gut microbiota and impairs social dominance in adult mice.

INTRODUCTION: Mesenteric lymph nodes (MLNs) are central in immune anatomy. MLNs are associated with the composition of gut microbiota, affecting the central system and immune system. Gut microbiota was found to differ among individuals of different social hierarchies. Nowadays, excision of MLNs is more frequently involved in gastrointestinal surgery; however, the potential side effects of excision of MLNs on social dominance are still unknown. METHODS: MLNs were removed from male mice (7-8 weeks old). Four weeks after MLN removal, social dominance test was performed to investigate social domi

Wide local excision prior to sentinel lymph node biopsy for primary melanoma of the head and neck.

BACKGROUND: Previous wide local excision prior to sentinel lymph node biopsy (SLNB) may have the potential to disrupt lymphatic channels, thus incorrectly identifying the sentinel node. The purpose of this study was to investigate: (1) regional recurrence rates of prior wide local excision compared to other biopsy techniques and (2) survival outcomes in patients with melanoma of the head and neck. METHODS: Between the years 2000 and 2016, 391 cases were reviewed with a median follow-up time of 30 months in a large tertiary care center. Biopsy practices included shave, punch, wide local excisi

Caudal-to-cranial Approach in Laparoscopic Right Hemicolectomy with Complete Mesocolon Excision and D3 Lymph Node Dissection.

Laparoscopic right hemicolectomy (LRH) has been proven to be an effective and safe surgical approach. However, due to the numerous vessels involved in the right colon and the variability of individual anatomy, LRH still faces many challenges. In the early stages, two surgical approaches were reported in the literature: the lateral-to-medial and medial-to-lateral approaches. The lateral-to-medial approach is typically used in open surgery. Currently, with the development of technology, multiple surgical approaches for LRH are available. This paper reports on a caudal approach method, the caudal

Case report: incidental finding of an ectopic adrenal gland during sentinel lymph node evaluation following melanoma excision.

INTRODUCTION AND IMPORTANCE: Ectopic adrenal tissue is usually found in well-described locations, mainly along the embryological pathway of the adrenal glands. In some cases, fragments of this tissue are found in non-physiological anatomical locations. This discovery, often made by chance, raises questions about these abnormal locations and prompts discussion of etiopathological hypotheses, considering the literature. CASE PRESENTATION: A 71-year-old man presented with an unclassifiable acral invasive melanoma, according to Clark's staging system, located in the third interdigital space of th

Lymph node recurrence after right colon resection for cancer: evidence for the utilisation of complete mesocolic excision.

Complete mesocolic excision (CME) of colon cancer is a resection performed along embryological planes to include the completely intact mesentery surrounding the tumour with a high central vascular ligation. The aim is to remove all lymph nodes draining the cancer. Proponents of the technique cite the significantly decreased local recurrence and improved 5-year survival rates associated with CME versus conventional colectomy. Although increasingly performed in many centres, it has not yet gained widespread acceptance as it is technically more challenging and can incur an increased bleeding risk

Lymph node metastasis following chemoradiotherapy in advanced rectal cancer: ypT2-focused analyses of total mesorectal excision specimens.

BACKGROUND: Non-radical management is an option for good responders to neoadjuvant chemoradiotherapy in mid-to-low rectal cancer. This study aimed to analyze risk factors for lymph node metastasis in patients with ypT2 rectal cancer, exploring the possibility of non-radical management. METHODS: We included patients with ypT2 rectal cancer who received neoadjuvant chemoradiotherapy followed by total mesorectal excision between January 2004 and December 2022. Clinicopathological parameters were evaluated to identify risk factors for lymph node metastasis. RESULTS: Among the 198 patients, 158 (

[Experiences with intensive radiotherapy of prostatic cancer in conjunction with surgery (transurethral resection, pelvic lymph node excision)].

Since 1981 a curative radiation treatment was performed in 84 patients with prostatic carcinoma. Previously, in 37 cases a transurethral resection of bladder outlet obstruction was done and in 18 patients a pelvic lymph node dissection was performed, whereas 29 patients were without operative therapy. Mild side effects of radiation could observed in all 3 groups in nearly the same portion (59/56/65%). However, in the group with transurethral resection after follow-up of 4,4 years severe late complications were found (cystitis, incontinence, urethral fistula). Therefore, radiation treatment of
