[Mediastinal localisation of a thoracic duct lymphocele--account of a surgical case].

INTRODUCTION: Lymphocele of the thoracic duct is a rare pathological entity occurring as a consequence of deterioration and cystic dilation wall of this vessel. It may be of congenital or degenerative origin. OBSERVATION: We report the case of a 47 year old patient, operated on for a symptomatic cyst localised by CT scan to the inferoposterior mediastinum. At postero-lateral thoracotomy it was identified as a lymphocele and completely resected. Chemical analysis of the liquid and histopathology study of the cystic wall confirmed the diagnosis. The patient's post operative course was uneventfu

[Axillary lymphoceles after breast cancer surgery. Pathogeny, prevention].

We report a retrospective series of 126 patients seen in 1992 for conservative surgery (group T) or Patey procedure (group P) for breast cancer. We looked for factors predictive of axillary lymphocele and their prevention. Axillary lymphadenectomy performed in patients undergoing surgery for breast cancer leads to a certain degree of morbidity including the development of lymphoceles. The mean number of lymph nodes removed was 16 in group T and 19 in group P. Mean lymphorrhea was 741 cm3. Two factors were significantly implicated in the pathogenesis of postoperative lymphorrhea : the number of

[CT in the diagnosis and treatment of lymphoceles following gynecologic cancer surgery].

The staging of gynecologic cancers requires the knowledge of lymph node status and thus pelvic and/or lumbo-aortic lymphadenectomy remains, to date, a widely used procedure. Lymphoceles are a frequent complication of surgical lymph node dissection. They are lymph collections in the retroperitoneum following the continuous drainage of afferent lymph vessels. To assess the incidence of this complication, its CT features and the role of diagnostic imaging to treat lymphoceles, 140 patients were retrospectively evaluated. Forty of them had undergone pelvic and/or lumbo-aortic lymphadenectomy for p

High-Volume LVA-New Surgical Technique for Treatment of Lymphoceles in the Groin.

Background: Groin lymphoceles are common postoperative complications after vascular interventions that can be difficult to treat, especially in recurrent or complex cases. While lymphovenous anastomosis (LVA) is a potential, minimally invasive option, its limited flow capacity may not provide sufficient drainage in large lymphoceles. We present a novel high-volume LVA technique that uses larger veins to directly drain the lymphocele cavity. Methods: Five patients with six groin lymphoceles, all previously treated unsuccessfully with conventional methods (mean 3.3 surgeries), underwent high-vol

Lymphoceles complicating arterial reconstructions of the lower limbs: outpatient conservative management.

BACKGROUND: Treatment of lymphoceles complicating vascular procedures is controversial. The purpose of this study was to evaluate the practicability, effectiveness and safety of conservative management on an outpatient basis. METHODS: Medical records were reviewed for 23 postoperative lymphoceles (in 18 patients) seen at our institution between 1986 and 1999. Diagnosis was made by physical examination and needle aspiration of fluid collection; bacterial cultures were obtained in all. Ultrasonography was performed in all patients, lymphoscintigraphy (99mTc HSA) in 11, angio-TC in 2 cases, MRI

Afferent lymphovenous anastomosis for treating refractory high-output lymphocele following groin node dissection.

Groin node dissection (GND) following uro-gynaecological cancer surgery has a very high incidence of lymphatic complications. These patients often suffer from prolonged wound healing due to high-output lymphatic discharge. Repeated debridement, sclerotherapy, embolisation are among the commonly used alternatives. These approaches tend to scar and obliterate the lymphatic system, leading to secondary lymphoedema. In recent years, there has been an exponential rise in the popularity and success of lymphovenous anastomosis as a prophylactic surgery following nodal dissection. It has been well-docu

Supermicrosurgical lymphatic venous anastomosis for intractable lymphocele after great saphenous vein harvesting graft.

Lymphoceles result from either trauma to the lymphatic vessels or after vein graft harvest, which occurs in 10% to 16% of patients. When a lymphocele persists despite conservative treatment, patients can experience undue distress. We have reported the case of successful treatment using lymphatic venous anastomosis (LVA) of an intractable lymphocele that had been refractory to conservative treatment, including stretch bandaging, drainage, and local injection for 2 years after great saphenous vein harvest. The lymphocele resolved shortly after the LVA without any adverse effects. LVA can be a us

Prophylaxis of lymphocele formation after kidney transplantation via peritoneal fenestration: a systematic review.

Lymphocele formation after kidney transplantation is a frequent complication which causes pain, secondary graft loss, rehospitalizations and reoperations. Therefore, prophylaxis of lymphocele formation is of utmost importance. To assess the effectiveness of peritoneal fenestration in renal transplantation to prevent lymphocele development. A systematic literature search was conducted combined with hand-searches on lymphocele prevention following renal transplantation using peritoneal fenestration. A qualitative and quantitative analysis of included trials was conducted. We identified three tri

Predictors of symptomatic lymphocele after radical prostatectomy and bilateral pelvic lymph node dissection.

OBJECTIVES: Lymphocele is the most common complication of pelvic lymphadenectomy (PLND). We sought to determine predictors of symptomatic lymphocele after radical prostatectomy (RP) and PLND, and in particular, to determine if the number of drains placed represents an independent predictor. METHODS: Between January 1999 and June 2007, 4173 consecutive patients underwent bilateral PLND at the time of either open or laparoscopic RP. Lymphoceles were identified in patients undergoing imaging as a result of symptoms suspicious for lymphocele, such as fever, abdominal pain or lower extremity swell

Retroperitoneal laparoscopic management of a lymphocele after abdominal aortic surgery: a case report.

A retroperitoneal lymphocele is a rare complication of abdominal aortic surgery. We present a case of 77-year-old man who developed a retroperitoneal lymphocele 14 days after undergoing graft replacement for an abdominal aortic aneurysm. Paracentesis showed a white and turbid fluid that was determined to be chyle. Conservative therapy, including percutaneous drainage, fasting, and total parenteral nutrition, was unsuccessful. Retroperitoneal laparoscopic ligation of the leaking lymphatics was performed on postoperative day 33. The postoperative course was satisfactory. The laparoscopic approac

Late perirenal lymphocele causing ureteral and arterial obstruction in renal transplant patients.

The 3 cases reported herein reveal the problem of chronic lymphoceles in the transplant patient. Surgical correction of the obstruction to the renal artery or ureter is greatly facilitated by the recognition of this clinical entity and the use of intraoperative radiographic studies to distinguish these pseudoencapsulated cysts from the true renal pelvis. The use of sterile milk aids in the demonstration of the anastomotic patency of the watertight closure.

[Chylou Peritonitis Secondary to a Lymphocele of the Thoracic Duct due to Blunt Trauma].

Chylous peritonitis, which is caused by the accumulation of chyle in the abdominal cavity, is a rare cause of acute surgical abdomen. It has a milky appearance due to its high triglyceride content. It can originate from different mechanisms, including congenital malformations and injury or obstruction of the lymphatic ducts. Due to its low incidence, there is no clear diagnostic or therapeutic approach, adding further complexity to the condition. We present a clinical case of a 37-year-old female patient with a history of blunt cervical trauma that progressed to abdominal distension and perito

Microsurgical Lymphovenous Anastomosis for Pelvic Lymphoceles after Gynecological Cancer Surgery.

BACKGROUND: Pelvic lymphoceles are the most common complications after pelvic lymphadenectomy. Microsurgical procedures have attracted attention as an alternative treatment for lymphoceles. Here, we report six cases of refractory lymphoceles that were successfully treated using lymphovenous anastomosis (LVA). METHODS: Six patients underwent surgery for gynecological cancers and developed pelvic lymphoceles, which did not respond to conventional treatment. We mainly performed LVA on the ipsilateral lower limbs, although some procedures were also performed on the contralateral limbs. The change

Bilateral abdominopelvic lymphoceles secondary to huge uterine leiomyomas: A case report.

Pelvic lymphocele secondary to uterine leiomyoma has not been previously reported. We report a case of abdominopelvic lymphocele associated with huge uterine fibroids which was managed conservatively. A 39-year-old unmarried lady presented with pressure symptoms in pelvis was diagnosed to have a huge uterine leiomyomas occupying the entire abdomen. Magnetic resonance imaging of pelvis and abdomen demonstrated multiple uterine fibroids. In addition, bilateral cystic structures were seen in the pelvis with extension to the para-colic gutters. During myomectomy, bilateral abdominopelvic lymphocel

A Breast Reconstruction Using a Breast Prosthesis and Capsular Flap for a Lymphocele Patient.

We encountered a very rare condition where the patient had a lymphocele under the skin envelope of the breast following mastectomy during the course of breast reconstruction with a tissue expander. The incidence rate of axillary lymphoceles is reported as 2.2-50% in breast cancer patients, but there have been no reports mentioning lymphoceles under the breast skin during the course of breast reconstruction with a prosthesis. The patient had a lymphocele in the lower lateral part of the breast following mastectomy and had multiple cellulitis-like inflammations. These inflammations were treated
