Guest Editorial: Two hits and four factors affecting the development of, or resistance to, transfusion-associated graft-versus-host disease.

Transfusion-associated graft-versus-host disease (TA-GVHD), a rare but usually fatal adverse effect of transfusion, is hypothesized here as a two-hit phenomenon: 1) predisposing "cytokine weather" irrespective of immunocompetence, plus 2) consequence of a (partial or total) one-way (donor to recipient) HLA match of viable donor T lymphocytes with proliferation potential. Since the introduction of irradiation, universal leukoreduction with third or later generation filters, and avoidance of transfusion from blood relatives, TA-GVHD has been successfully prevented in developed countries. However

Human anti-lymphoma responses generated in vitro and in vivo following sensitization with allogeneic leukocytes.

Peripheral blood lymphocytes (PBL) from a patient with poorly differentiated lymphocyte lymphoma (PDLL), after stimulation for 7 days with X-irradiated allogeneic lymphocytes pooled from three or ten donors (poolx), were cytotoxic for autologous lymphoma cells. Some clones lytic for autologous lymphoma cells, that were derived from this patient's pool-stimulated cells, resembled cytotoxic T lymphocytes (CTL), while other clones resembled natural killer (NK)-like cells in that they also lysed NK-sensitive HLA-negative K562 cells. In a second patient with more advanced PDLL, PDL cultured with T-

-318C/T polymorphism of the CTLA-4 gene is an independent risk factor for RBC alloimmunization among sickle cell disease patients.

Cytotoxic T-lymphocyte-associated antigen 4 (CTLA-4) molecule is expressed on T-lymphocyte membrane and negatively influences the antigen-presenting process. Reduced expression of CTLA-4 due to gene polymorphisms is associated with increased risk of autoimmune disorders, whose physiopathology is similar to that of post-transfusion red blood cell (RBC) alloimmunization. Our goal was to evaluate if polymorphisms of CTLA-4 gene that affect protein expression are associated with RBC alloimmunization. This was a case-control study in which 134 sickle cell disease (SCD) patients and 253 non-SCD pati

Passenger lymphocyte syndrome with severe hemolytic anemia due to an anti-Jk(a) after allogeneic PBPC transplantation.

BACKGROUND: After allogeneic peripheral blood progenitor cell (PBPC) transplantation, a patient developed a severe hemolytic transfusion reaction due to passenger lymphocyte syndrome. CASE REPORT: A 50-year-old woman with secondary acute myeloid leukemia transforming from a myelodysplastic syndrome received an ABO-compatible PBPC graft from her HLA-identical sister. For prophylaxis of GVHD, the patient was treated with cyclosporine and methotrexate. Eighteen days after the transplant, the patient experienced a severe hemolytic transfusion reaction due to an alloantibody (anti-Jk(a)) produced

Induction of antiidiotypic antibodies by blood transfusions. Characterization of T cell alloantigen-specific receptors by sera from transfused mice.

Balb/c (H-2d) mice were transfused weekly with 0.1 ml of whole blood from C3H (H-2k) mice. One week after 3 blood transfusions (BT), the mice were bled and the sera collected and pooled. The 3BT serum was absorbed twice with C3H lymphocytes and IgG isolated by ion-exchange chromatography. Balb/c anti-C3H, Balb/c anti-Balb/c, and Balb/c anti-SJL (H-2s) lymphocytes were generated in the mixed lymphocyte cultures and metabolically labeled with 35S-methionine. Cell lysates were prepared from labeled lymphocytes and precleared by absorption with normal mouse serum. Immunoprecipitation was carried o

Transfusion-associated graft-versus-host disease in an immunocompetent individual.

Transfusion-associated graft-versus-host disease (TA-GVHD) is a fatal complication that occurs in both immunocompromised and immunocompetent individuals following blood transfusion. It is characterised by donor lymphocyte attacks on recipient tissues. We present a case of TA-GVHD in an immunocompetent man in his 70s who developed fever, diarrhoea, rash and pancytopenia after non-irradiated blood transfusion from an unrelated donor. Despite aggressive treatment, the patient's condition deteriorated leading to his death, highlighting the challenges in recognising and managing TA-GVHD. This case

[HLA and transfusion].

Using published data, the role of the HLA system for transfusion of cellular blood components is described. Transfusion-associated immunomodulation and the causative role of HLA in other side effects of transfusion therapy are discussed, using review articles and original papers from the German and English literature as sources. The HLA system has important implications for transfusion therapy. Sensibilization of recipients with consecutive formation of HLA antibodies impairs the support of thrombocytopenic patients and may cause febrile transfusion reactions. HLA antibody formation in donors

Single dose of 5 Gy can damage erythrocytes and consequently induces lymphocyte depletion in spleen and circulating blood.

PURPOSE: Radiation-induced lymphopenia (RIL) has been well known to associate with poor patient prognosis. Based on interesting findings in our previous study, we hypothesize that a low dose of 5-Gy can damage erythrocytes, which indirectly depletes lymphocytes in spleen, and may contribute to RIL. This study aimed to verify the hypothesis. METHODS: Rats were divided into non-irradiated and 5Gy-irradiated groups. Peripheral blood was collected. Erythrocytes were isolated, irradiated for the irradiated group, and transfused back into the same rat for both groups. Peripheral blood samples were

Immunologic evaluation of children with homozygous beta-thalassemia treated with desferrioxamine.

In 15 children with thalassemia major (age 4-17 years) a detailed analysis of different immune functions was performed: phagocyte function, specific cellular immunity, humoral defense system. All patients had been subjected to a desferrioxamine therapy and a high transfusion regimen. Examination of neutrophil function included adherence, random migration, chemotaxis, killing of Escherichi coli and production of superoxide radical; these neutrophil functions were shown to be normal. In addition, lymphocyte proliferation in response to different lectins (phytohemagglutinin, concanavalin A, pokew

Effect of donor-specific blood transfusions on allotransplanted teeth in a monkey model: Histoquantification of periodontal healing.

BACKGROUND/AIMS: Pre-transplant blood transfusions have previously shown a positive effect on organ allograft survival in humans and various animal species. The aims of this study were, first, to evaluate the effect of pre-transplant donor-specific blood transfusions on periodontal healing of fully developed allotransplanted teeth in monkeys; and second, to investigate the immune response against donor antigens and to determine a possible correlation between alloimmune reactions and histopathological signs of rejection. MATERIAL AND METHODS: Twenty monkeys (Cercopithecus aethiops) were divide

Acute haemolytic reaction due to anti-Wb: a case report.

A 73-year-old Greek woman presented with symptomatic anaemia requiring red cell transfusion in the setting of progressive chronic lymphocytic leukaemia (CLL). Based on a negative antibody screen, two units of red blood cells (RBCs) were provided for transfusion. During the transfusion, the patient developed an acute haemolytic transfusion reaction (HTR), but recovered with supportive measures. Subsequent antibody investigation confirmed that the patient had an anti-Wb antibody and that the implicated RBC unit was Wb-positive. This is the first report of an anti-Wb causing a clinically signific

The Impact of Intraoperative Blood Loss on the Survival After Laparoscopic Surgery for Colorectal Cancer.

BACKGROUND/AIM: Blood transfusion and a large amount of intraoperative blood loss (IBL) have been reported to have a negative impact on long-term survival via immunosuppression. In recent years, thanks to the spread of laparoscopic surgery and the development of surgical devices, the average amount of IBL has decreased, as has the need for perioperative blood transfusion. Under such conditions, the prognostic significance of the amount of IBL is unclear. The aim of this study was to assess the impact of the amount of IBL on long-term survival. PATIENTS AND METHODS: A total of 277 patients who

Lymphocyte subpopulation abnormalities in sickle cell anemia: a distinctive pattern from that of AIDS.

In this study, we evaluated lymphocyte subpopulations in 23 adults with SCA. When compared to controls, SCA patients had higher lymphocyte counts with normal numbers of T101+ cells (T-lymphocytes) and T4+ cells. T8+ cells were significantly increased in SCA patients in comparison to controls (1684 +/- 243 vs 980 +/- 367, p less than .001). This increment was largely dependent on a T101-, T8+ cell population. The SCA patients as a group had significantly decreased T4/T8 ratio (p less than .0001). The SCA patients with history of blood transfusions had higher T4+ cells numbers and higher T4/T8 r

Interactions of cardiopulmonary bypass and erythrocyte transfusion in the pathogenesis of pulmonary dysfunction in Swine.

BACKGROUND: Allogeneic erythrocyte transfusion in cardiac surgical patients is associated with a fourfold increase in pulmonary complications. Our understanding of the processes underlying these observations is poor and there is no experimental model of transfusion-related acute lung injury that shows homology to cardiac surgical patients. Our objective was to develop a novel swine recovery model to determine how two clinical risk factors, allogenic erythrocyte transfusion and cardiopulmonary bypass, interact in the genesis of postcardiac surgery acute lung injury. METHODS: Thirty-six pigs we

Investigation of the mechanism of active enhancement of renal allograft survival by blood transfusion.

Transfusion with donor blood before transplantation can prolong the survival of renal allografts in the rat. The phenomenon is donor specific. We have investigated the effect of blood transfusion on both lymphocyte proliferation and the generation of donor-specific cytotoxic cells. In the first instance the kinetics of the proliferative responses of lymph node, spleen and thoracic duct lymph (TDL) leucocytes (harvested 7 days after transfusion of donor-specific blood, third-party allogeneic blood, syngeneic blood or no treatment) to lymph node stimulator cells were determined in a one-way mixe

Association of acute normovolemic hemodilution with hematologic inflammatory indices and early clinical outcomes after coronary artery bypass grafting.

BACKGROUND AND OBJECTIVES: Acute normovolemic hemodilution (ANH) is an established patient blood management strategy in cardiac surgery, but its effects on postoperative inflammatory biomarkers remain unclear. In this study, we evaluate the association of ANH with inflammatory indices, lactate-to-albumin ratio, transfusion requirements, and early postoperative outcomes after coronary artery bypass grafting (CABG). MATERIALS AND METHODS: This retrospective, single-center study included 366 patients who underwent elective isolated CABG with cardiopulmonary bypass. Patients were assigned to an A

Platelet transfusion therapy for alloimmunized patients: selective mismatching for HLA B12, an antigen with variable expression on platelets.

There can be wide variation in the expression of the HLA B12 antigen and its "splits," B44 and 45, on the platelets and lymphocytes from the same individual. One hundred sixty-two single donor platelet transfusions mismatched only for this antigen group were administered to 54 alloimmunized patients who were refractory to random donor platelets. Satisfactory increments (one-hour post-transfusion corrected-count increment [CCI] greater than 7,500) were seen following 111/162 transfusions (69%). In 31 patients (57%), all transfusions (n = 85) produced CCI greater than 7,500, and 76% of patients
