Fluconazole is an antifungal commonly used to treat Coccidioides immitis, but this medication has a number of side effects including fatigue, rash, headache, dizziness, nausea, vomiting, diarrhea, and elevated liver enzymes. We are unaware of any cases of retinal toxicity related to fluconazole. In this case we present a 76 year old woman with longstanding Coccidioides treated with high dose fluconazole. She becomes symptomatic with fluconazole toxicity and subsequently develops bilateral cystoid macular edema. As her dose of fluconazole is decreased and she is transitioned to voriconazole the

Cystoid macular edema (CME) is a common problem after cataract extraction. It can occur after uncomplicated surgery in patients with otherwise healthy eyes, after complicated surgery, or after surgery in patients with ocular diseases such as uveitis or diabetic retinopathy. Usually vision loss from cystoid macular edema is temporary and responds to treatment with topical anti-inflammatory medications. However, some cases respond poorly to conservative treatment and may develop permanent visual loss. A review of the medical literature was performed for all articles published in English between

PURPOSE: To ascertain the association between the improvement of diabetic macular edema and increased visual acuity after pars plana vitrectomy.
METHODS: From January 1994 to December 1996 we prospectively studied 18 patients (18 eyes, 7 women and 11 men, mean age 52 years, range 37-68) with type II diabetes and clinically significant macular edema. One group was composed of 9 patients presenting diffuse macular edema (DME); a second group with 9 patients presented cystoid macular edema (CME). All patients underwent pars plana vitrectomy.
RESULTS: Preoperative Snellen visual acuity was 20/14

The incidence of clinically significant cystoid macular edema (CME) following cataract surgery has risen sharply with the widespread utilization of intraocular lenses. To evaluate the factors contributing to the incidence of CME, the senior author has divided 1808 of his own cataract cases into three groups according to procedures used: 1) intracapsular cataract extraction (ICCE) with no lens implantation; 2) ICCE with implantation of an iris-supported lens; and 3) extracapsular cataract extraction (ECCE) with implantation of a posterior chamber lens. The incidence of CME in these three groups

We reviewed 224 cases that had undergone secondary lens implantation. Cystoid macular edema occurred in 16 (7.1%) cases; edema was transient in 14 (6.3%) cases and permanent in two (0.8%) cases. Three factors seem to affect the incidence of CME: time lapse between cataract extraction and lens implantation; a positive preoperative fluorescein angiogram; and vitreous loss during lens implantation--if implantation is performed within a year of cataract extraction. Two eyes with pre-existing CME and a functional vision of 20/100 or better deteriorated to 20/200 or less after secondary lens implant

Aphakic penetrating keratoplasty (APKP) and combined penetrating keratoplasty with lens extraction (CPKP) are highly successful surgical procedures, with clear corneas occurring in over 90% of cases. However, macular problems tend to develop or appear postoperatively, and visual acuity is disappointing, principally because of either cystoid macular edema (CME) or pre-existing macular degeneration. To determine whether vitreous manipulation at the time of surgery alters the macular outcome, 132 eyes were studied prospectively. Eighty-four of these had pre-existing aphakia and all underwent ante

In 650 consecutive Binkhorst lens implants, secondary membrane surgery was required more than eight times as frequently after an extracapsular than after an intracapsular cataract extraction. The visual acuity results after discussion were uniformly good. Transpars plana vitrectomy was reserved for more severe cases. The pupils were cleared, but the visual acuity results were mixed. A fluorescein angiographic study revealed little difference in the incidence of cystoid macular edema in implant and nonimplant cases. There was a lower incidence after extracapsular than intracapsular extractions

This presentation recounts the original description of CME and the more recent developments associated with intravenous fluorescein angiography in its study and diagnosis. The work of previous authors to show the incidence of CME and its possible prognosis are reviewed. Our findings in a retrospective study agree with the other authors and show that there is no particular increase in CME following intraocular lens implantation regardless of the type of surgical procedure used. It is established that vitreous loss does increase the incidence of CME. A prospective study with routine intravenous

PURPOSE: To describe a case of bilateral cystoid macular edema (CME) that developed following intracameral travoprost implantation, which then resolved after explantation, and to review the association of CME with prostaglandin analogues (PGA).
OBSERVATIONS: The patient developed CME after intracameral travoprost implantation in each eye. An extensive work up including blood work and aqueous biopsy did not reveal other inflammatory etiologies. Explantation of the travoprost led to resolution of the CME in each eye, suggestive of a temporal association of the implant and the CME.

We report the case of a 61-year-old patient with relapsing-remitting multiple sclerosis (RRMS) who developed asymptomatic macular edema (ME) after initiation of ozanimod, a sphingosine-1-phosphate receptor (S1PR) modulator. The patient had a history of completely resolved central serous choroidopathy (CSC) in the right eye. Following a recent clinical worsening and a new brain lesion, ozanimod was started after appropriate screening, including ophthalmological evaluation. Three months into treatment, an OCT performed as part of routine monitoring revealed ME in the contralateral (left) eye, de

An 11-year-old girl with a history of neutropenia, developmental delay, hypotonia, and intellectual disability was diagnosed with Cohen syndrome after genetic testing discovered homozygous mutation in the VPS13B gene. She was referred to a retinal specialist with a chief complaint of decreased peripheral vision. On examination, decreased visual acuity, pigmentary changes, and nonleaking cystoid macular edema were present in both eyes.

PURPOSE: Fingolimod is among the first oral disease-modifying agents for the treatment of relapsing-remitting multiple sclerosis (MS). Despite its favorable safety profile, fingolimod may cause macular edema, a significant adverse event, which occurs within the first 4 months of therapy. Macular edema usually resolves upon discontinuation of fingolimod; however, the time required for resolution of this condition is unknown.
OBSERVATIONS: A 42-year-old white male with a history of relapsing-remitting MS presented with blurring of vision in his left eye 24 h after the first dose of fingolimod.

We believe that this is the first report of cystoid macular edema following corneal-relaxing incisions. Both cases we describe occurred in pseudophakic eyes, one of which contained an ultraviolet light-absorbing intraocular lens. One case was complicated by a corneal perforation and postoperative inflammation. The other case had no apparent intraoperative or postoperative complications other than the cystoid macular edema. In both cases, the cystoid macular edema resolved after the institution of medical treatment, and visual acuity returned to preoperative levels.

Early cystoid macular edema causing some loss of central vision in an eye with an 8-year history of limbal melanoma was studied histopathologically. A typical combination of slight chronic uveitis, vitreoretinal traction, parafoveal separation of posterior vitreous and inner-limiting membrane, swelling with distinct cystoid changes in Henle's fiber layer, and foldlike detachment of foveal cones is described and discussed.

A foveola with signs of atrophy and cystoid edema in an eye with a history of contusion, laceration, and hemorrhage exhibits a selective attraction of blood-filled macrophages. This important observation allows for the conclusion that the avascular foveola can leak into the inner eye like a blood vessel and chemotactically attract vitreous macrophages under these conditions.

A woman developed an inner lamellar macular hole and cellophane maculopathy as a complication of chronic cystoid macular edema following cataract extraction. Fluorescein angiography revealed a cystoid pattern of fluorescence in the paracentral area but no evidence of staining in the region of the hole. Following death, histopathologic examination of both eyes confirmed the clinical observations. Biomicroscopic examination and fluorescein angiography are of value in differentiating an inner lamellar macular hole from a full-thickness macular hole and from a pseudomacular hole caused by spontane

BACKGROUND: Here we report two patients who developed an atypical macular hole (MH) during the treatment course for diabetic macular edema (DME).
CASE PRESENTATIONS: Patient 1 was a 73-year-old male. Optical coherence tomography (OCT) revealed perifoveal retinoschisis (RS) in addition to cystoid macular edema and serous retinal detachment (SRD) in his left eye, and that an MH had developed during the clinical course. A convex surface was formed at the MH margin toward the vitreous cavity, and granular shadows were observed in the fluid cuff. Intraoperative findings revealed a thin epiretinal
