Histologic changes in the anterior mallear ligament and the head of the malleus in otosclerosis.

OBJECTIVE: To determine ossicular and anterior mallear ligament (AML) changes in otosclerosis. STUDY DESIGN AND SETTING: Hyalinization of AML was graded as follows: none, patchy, or diffuse in 95 temporal bones (TBs) with otosclerosis: 52 with stapedial fixation (SF); 43 without fixation (NSF); and 52 age-matched controls. Fixation of the head of the malleus was noted. RESULTS: Hyalinization with SF was 17 none, 23 patchy, and 12 diffuse; with NSF it was 16 none, 20 patchy, and 7 diffuse; and in controls, 23 none, 24 patchy, and 5 diffuse. There was no significant difference in hyalinization

[An unusual case: fracture of the handle of the malleus due to finger manipulation].

An isolated fracture of the handle of the malleus is a rare entity in otorhinolaryngology and manifests clinically as acute-onset unilateral hearing loss. Several factors may cause this injury, including acute barotraumatic pressure changes or traumatic events. Various therapeutic approaches such as tympanoplasty, autologous graft, or application of bone cement are discussed. We report the case of a 46-year-old female patient who developed acute hearing loss in her left ear after finger manipulation. Clinical evaluation revealed axial displacement of the handle of the malleus and audiometry in

Linear transmission between malleus and stapes in cases with incus necrosis.

OBJECTIVE: The outcome of cartilage interposition ossiculoplasty was assessed in cases of incus necrosis after posterior malleus repositioning in the plane of the stapes, in terms of hearing gain after ossicular reconstruction. METHODS: A retrospective observational study was conducted of 30 patients admitted to an Ain Shams University hospital from March 2021 to September 2021. All patients with ossicular disruption due to chronic suppurative otitis media and hearing loss of more than 40 dB were included in the study. Pure tone audiometry was conducted for each patient after three months, si

An Endoscopic Procedure for the Simultaneous Treatment of both Otosclerosis and Malleus Fixation.

To report the case of the simultaneous treatment of otosclerosis and malleus fixation through an entirely endoscopic transcanal approach. A targeted transcanalar epitympanotomy with annular bony ridge conservation was planned preoperatively, with 3D CT localization of the fixed part of the malleus head. The upper part of the malleus head and the superior ossified ligament of the malleus were drilled. A 0.6 mm stapedotomy was performed and a piston inserted. The patient's recovery was uneventful, with closure of the air-bone gap on her postoperative audiogram, associated with an overclosure in

Bilateral Fracture of the Handle Malleus: A Case Report and Review of the Literature.

Malleus fracture is a rare condition. Usually, the handle of the malleus is involved, and we do not find reports in the literature of this condition in the bilateral presentation. It is present as sudden conductive hearing loss commonly after digital manipulation of the external auditory canal. The diagnosis is based principally on clinical examination by otomicroscopy and audiometry. Cone-beam computed tomography emerging as a powerful tool in the field of otolaryngology, especially for explorations of paranasal sinuses and temporal bone, due to imaging with a high resolution and few artifact

Use of the malleus handle as a landmark for localizing the round window membrane.

Localization of the round window membrane is desirable in order to allow a more direct application of medication to the inner ear. A new procedure has been developed to deliver direct, near-continuous inner ear perfusion to the round window with the Silverstein MicroWick. In this office procedure, the wick is inserted through a tympanostomy tube into the round window niche. Accurate localization of the round window is a necessary component of this procedure. In an effort to ascertain the precise location of the round window, we examined 25 cadaveric human temporal bones and measured the distan

Recognition of malleus handle fracture in the differential diagnosis of otologic trauma.

We have had the opportunity to care for four patients who experienced barotrauma or implosive trauma to their ears. The differential diagnosis in these cases included ossicular injuries as well as possible perilymph fistulae. In each of these cases there was a small conductive hearing loss which fluctuated and a high compliance tympanogram. Careful otoscopy with the aid of pneumomassage revealed an isolated malleus handle (manubrium) fracture. Exploration of one ear was performed to rule out perilymph fistula and the diagnosis of malleus handle fracture was confirmed. Recognition of this injur

Repair of a malleus-handle fracture using calcium phosphate bone cement.

We report a case of a 60-year-old woman who reported a popping sound in her left ear, induced by changes in pressure, since she pulled her index finger out of her ear canal after accidentally inserting it while washing her hair. Otoscopic examination revealed a wrinkled tympanic membrane and irregular malleus handle. An isolated malleus-handle fracture resulting from barotrauma was suspected. The fracture was repaired surgically using calcium phosphate bone cement. Postoperatively, the problem disappeared and her hearing recovered. This case is the first to demonstrate the efficacy of calcium

Chondroma of the bony external auditory canal attached to the short process of the malleus.

A chondroma located in the external auditory canal is a rare finding. Here, we report the case of a 12-year-old girl who attended our clinic with complaints of a rustling sound in her left ear. Otoscopic examination revealed a tiny bone-like tumor protruding from the anterior wall of the bony external auditory canal, attached to the short process of the malleus. An audiogram revealed a conductive hearing loss in her left ear, although she did not notice it. After excision of the tumor through the ear canal, her complaints disappeared and her hearing recovered. Histologically, the tumor proved
