Sclerosing lobular hyperplasia is a uncommon benign fibroproliferative lesion of the breast similar to a fibroadenoma. It usually presents as a mass in the outer quadrant of the breast of younger women. The authors report an atypical case of sclerosing lobular hyperplasia that presented as bilateral breast masses in an elderly woman after a reduction mammaplasty. Breast masses that develop after reduction are often a result of fat necrosis, internal scarring, or organizing hematomas. Those that do not resolve warrant open biopsy because they may represent malignant or benign neoplasms.

PURPOSE: Reduction mammaplasty is one of the most common procedures performed by plastic surgeons. Previous studies demonstrated that most plastic surgeons do not require preoperative mammography prior to reduction mammaplasty. The incidental discovery of malignant or high-risk lesions in breast reduction specimens may preclude the possibility of breast-conserving surgery. The clinical features of the toxic shock syndrome are well established. It is rapidly developing disease and may be lethal if not recognized and treated in time. Once the syndrome was associated with the use of tampon by menstruating women. Recently this syndrome has been reported as due to Staphylococcus aureus infection. A severe case of toxic shock syndrome in a 18-years-old patient after reduction mammoplasty is reported, and the importance of an early diagnosis is emphasized.

Vertical reduction mammaplasty using a superomedial pedicle is a well-accepted technique giving good results in mild to moderate breast hypertrophy. A 30-year-old female with mammarial hypotrophy was diagnosed with non-bullous congenital ichthyosiform erythroderma (NBCIE) with dermatologic evaluation. Her mammarial hypotrophy and ptosis was corrected with a breast implant. Although there is a tendency to skin infections and increased skin fragility in the ichthyotic patients, breast augmentation with silicone prosthesis is well-tolerated by the patient.

A series of patients who had biopsies of breast nodules during augmentation mammaplasty is reviewed. We conclude that such procedures performed for discrete lesions yield valuable information. In this series, the biopsies did not result in an increased rate of complications.
