Journal of cranio-maxillo-facial surgery : official publication of the European Association for Cranio-Maxillo-Facial Surgery2016eng
The aim of this study was to evaluate the outcomes of coronectomy as an alternative surgical procedure to complete removal of the impacted mandibular third molar in patients with a suspected close relationship between the tooth root(s) and the mandibular canal. A total of 151 patients underwent coronectomy and were followed up with clinical examinations and panoramic radiographs for a minimum of 6 months after surgery. None of the patients exhibited inferior alveolar nerve injury. Eruption of the retained root(s) was more frequent in younger patients (18-35 years). Thirty-six patients (23.8%)

OBJECTIVES: This study evaluated the relationship of the lingual nerve with the adjacent anatomical structures of the mandibular third molar region, influencing the dentist to be aware of the variability of these relationships.
MATERIAL AND METHODS: Samples of 24 human corpse half-heads were selected and divided according with the presence or absence of the mandibular third molars. The lingual nerve (LN) was explored, showing its run from the oblique line until its crossing with the submandibular gland duct. The measurements along the LN and the adjacent anatomical structures were taken at th

OBJECTIVE: This study reports the signs and symptoms that are the features of trigeminal nerve injuries caused by local anaesthesia (LA).
METHODS: Thirty-three patients with nerve injury following LA were assessed. All data were analysed using the SPSS statistical programme and Microsoft Excel.
RESULTS: Lingual nerve injury (LNI; n = 16) and inferior alveolar nerve injury (IANI; n = 17) patients were studied. LNI were more likely to be permanent. Neuropathy was demonstrable in all patients with varying degrees of paraesthesia, dysaesthesia (in the form of burning pain) allodynia and hyperalg

The capacity of 4 patients who had previously experienced trauma to their mandibular nerves to distinguish opposing directions of tactile motion over the distribution of the mental nerve was compared to that of 8 neurologically normal adults. Brushing stimuli were delivered to the perioral region and were precisely controlled for their velocity, the length of skin traversed, the width of skin contacted, and the orientation and direction of motion. A temporal, 2-alternative, forced choice method was used to obtain estimates of directional sensitivity, d'. It was discovered that impairment in cu

In a patient with developmental facial asymmetry resulting from unilateral mandibular hyperplasia, irregular height of the mandibular body was strongly manifested on the affected side. Since a large portion of the inferior border of the mandible predicted to be cut off contained almost the whole of the lower alveolar nerve, transfer of the latter was employed. The nerve was surgically isolated along its course, and secured by being lifted while horizontal osteotomy of the mandibular body was performed. The nerve was accommodated in new groove created at a safe distance from the roots of the lo

Herpes zoster is a disease caused by the reactivation of dormant varicella zoster virus present in the sensory root ganglion. It presents with a vesicular rash on an erythematous base similar to that seen in classical varicella, however, with only a single dermatomal distribution. The rash is usually seen throughout the affected dermatome as the dorsal root ganglia for each dermatome are clustered together. We present a case of an otherwise healthy male who developed a vesicular rash confined to the distribution of the posterior division of the mandibular nerve. Though the entire mandibular ne

The purpose of this study was to perform a review of the literature related to the anatomy of the mandibular lingula and to provide information to improve the execution of effective anaesthesia of the inferior alveolar nerve in clinical practices of the mandible. Prognathic mandibles have a lingula that is located more posterior and superior than non-prognathic mandibles. Most of the lingulae are located above the occlusal plane, but it is occasionally located at or below the occlusal plane. The lingula of the triangular shape was generally located slightly more superior and posterior than oth

The approximity of the inferior alveolar nerve (IAN) to the roots of lower third molars (M3) is a risk factor for the occurrence of nerve damage and subsequent sensory disturbances of the lower lip and chin following the removal of third molars. To assess this risk, the identification of M3 and IAN on dental panoramic radiographs (OPG) is mandatory. In this study, we developed and validated an automated approach, based on deep-learning, to detect and segment the M3 and IAN on OPGs. As a reference, M3s and IAN were segmented manually on 81 OPGs. A deep-learning approach based on U-net was appli

Iatrogenic injury of the inferior alveolar or lingual nerves frequently leads to legal actions for damage and compensation for personal suffering. The masseter inhibitory reflex (MIR) is the most used neurophysiological tool for the functional assessment of the trigeminal mandibular division. Aiming at measuring the MIR sensitivity and specificity, we recorded this reflex after mental and tongue stimulations in a controlled, blinded study in 160 consecutive patients with sensory disturbances following dental procedures. The MIR latency was longer on the affected than the contralateral side (P

BACKGROUND: Marginal mandibular nerve injuries are more likely to be symptomatic than other facial nerve injuries following facelift procedures. The marginal mandibular nerve courses over the facial artery in the region of the mandible. The nerve is most susceptible to injury in this location because it lies superficial to the anterior facial artery.
OBJECTIVES: The authors describe the location of the marginal mandibular nerve based on superficial anatomic landmarks as it crosses the facial artery above the mandibular border, in order to help surgeons avoid injury to this nerve during faceli

BACKGROUND: The inferior alveolar nerve block (IANB) is commonly used in dentistry for pain management before and after dental procedures. Traditional nerve block techniques can sometimes be associated with complications such as anesthetic failure and nerve damage. Ultrasound-guided (USG) nerve blocks have emerged as a promising alternative. This systematic review aimed to evaluate the effectiveness of USG mandibular nerve blocks in pain control in adults.
METHODS: A systematic search of English literature was conducted using the PubMed, Web of Science, Scopus, Google Scholar, and Cochrane Li

INTRODUCTION: Mandibular lymphomas are rare and most often revealed by painless swelling. The authors report the case of a mandibular lymphoma revealed by an isolated lesion of the inferior alveolar nerve evolving for eight months.
CASE REPORT: A 41-year-old male patient was followed for left mandibular pain, with progressive hypoesthesia of the left inferior alveolar nerve. The radiological assessments remained normal during eight months. Then a vestibular tumor developed in front of tooth 34. The biopsy revealed a B-cell lymphoma. No other localization was found. The patient was in complete

Endosseous mandibular implant placement can result in injuries to the peripheral branches of the trigeminal nerve even with the most careful preoperative planning and intraoperative technique. In the past, many patients have been discouraged from seeking repair for such injuries because of the unreliability of the techniques for correcting the injury. It is now possible to perform microneurosurgical repair of such injuries. If the repair is done in a timely fashion, sensation can be improved or restored and painful nerve dysesthesia can be relieved. This article reviews the different types of

BACKGROUND: Placement of mandibular endosseous implants can result in damage to the lingual nerve, the inferior alveolar nerve or both nerves. All dentists who place mandibular implants should be aware of the appropriate early management of these injuries, as well as the appropriate time to refer patients with these injuries to a microneurosurgeon.
OVERVIEW: The lingual nerve is less likely to undergo spontaneous regeneration than is the inferior alveolar nerve, which is protected within the inferior alveolar canal. Since the inferior alveolar canal can be seen on most panoramic radiographs a
