Lymphangioma secondary to irradiation after mastectomy.

Lymphangioma is a rare benign disease of the lymphatic vessels. Typically, they are primary conditions but may be acquired secondarily, such as those caused by irradiation during radiotherapy for the treatment of breast cancer. The local lymphatic obstruction provoked by irradiation causes the appearance of asymptomatic hyaline vesicles on the irradiated skin. The present report describes a 78-year-old female patient, who initially presented hyaline vesicles that progressed into multiple papules with serous exudation of a yellowish and odorless secretion on the area of chronic radiodermitis in

Update on Post-mastectomy Lymphedema Management.

Lymphedema is a chronic, progressive condition caused by an imbalance of lymphatic flow. Upper extremity lymphedema has been reported in 16-40% of breast cancer patients following axillary lymph node dissection. Furthermore, lymphedema following sentinel lymph node biopsy alone has been reported in 3.5% of patients. While the disease process is not new, there has been significant progress in the surgical care of lymphedema that can offer alternatives and improvements in management. The purpose of this review is to provide a comprehensive update and overview of the current advances and surgical

Optimizing the total skin-sparing mastectomy.

HYPOTHESIS: Dissection of subnipple tissue to spare the entire skin envelope of the breast (total skin-sparing mastectomy) is a feasible option in appropriately selected patients and yields an excellent final cosmetic outcome. DESIGN: Prospective surgical technique outcomes study. SETTING: University-based breast care referral center. PATIENTS: Total skin-sparing mastectomy with preservation of the nipple-areola complex was performed in 64 breasts in 43 women. Indications for total skin-sparing mastectomy included prophylaxis (n = 29), invasive carcinoma (n = 24), and ductal carcinoma in si

Completion mastectomy after breast conserving surgery.

BACKGROUND: Breast conserving surgery (BCS) is increasingly offered to suitable patients diagnosed with early stage breast cancer. Occasionally the pathological margins on specimens following BCS are positive. The objective of this study is to assess the proportion of patients within our unit who required completion mastectomy after BCS and to determine if predictive factors could be identified to assist the breast surgeon identifying those patients at risk of positive margins following BCS. METHODS: All patients diagnosed with breast cancer between 2001 and 2005 were reviewed. Patients under

Stewart-Treves syndrome: lymphangiosarcoma following mastectomy.

Lymphangiosarcoma (LAS) is an aggressive, malignant vascular tumor following long-lasting chronic lymphedema. Patients with LAS demonstrate a history of breast cancer treated by radical mastectomy in the majority of patients. In the 1960s the incidence of LAS in patients with a 5-year survival after radical mastectomy varied from 0.07 to 0.45%. Today, due to changes in the operative techniques of breast cancer, less chronic lymphedema is seen with only a scant number of LAS patients. The etiology of this enigmatic tumor is not yet completely understood. Histologically, LAS arises from vascular

Bilateral prophylactic mastectomy: issues and concerns.

At present, the care of women at increased risk of developing breast cancer poses a clinical dilemma and remains an area of controversy. A number of investigators have addressed the pros and cons of prophylactic mastectomy versus close follow-up, utilizing annual mammography, semiannual or even more frequent physical examinations of the breast, and proficient monthly breast self-examinations. Recent efforts to isolate a gene (BRCA1) on chromosome 17q12-21 raise additional concerns about the management of women testing positive for BRCA1 mutations. These women are estimated to have an 85% lifet

Gynecomastia. Treatment by liposuction subcutaneous mastectomy.

Gynecomastia is a common finding in both adolescents and mature adult males. The previous surgical techniques of direct subcutaneous mastectomy were associated with high frequency of complications and unsatisfactory aesthetic results. Application of liposuction techniques to the prepectoral region has provided an effective and safe technique for reliably contouring and reducing the prepectoral region. The technique described in this article includes proper placement of the incision, the injection of low-dose Xylocaine and epinephrine, and the utilization of three different types of cannulas to

[Stewart-Treves syndrome: post-mastectomy angiosarcoma].

A case is reported of angiosarcoma occuring in the chronic-lymphedematous right arm of a 56 year-old woman 9 years after radical mastectomy for cancer of the right breast. This is a characteristic example of Stewart-Treves syndrome (angiosarcoma in chronic lymphedema). The patient died within the year from multiple metastases. About 200 such cases are reported in the literature: the prognosis is bad, the best therapy reported being amputation of the extremity. Early diagnosis is essential through frequent checks for chronic lymphedema, which is seemingly the crucial etiologic factor in the occ

Subcutaneous mastectomy. The excess skin problem.

Subcantaneous mastectomy through a lateral approach, with preservation of the nipple and areola on a dermal pedicle, removing the excess skin at the time of aubautaneous mastectomy, is a safe procedure which results in esthetically acceptable breasts. The surgical approach greatly facilitates the removal of the entire glandular portion of the breast. The need for a second surgical procedure is eliminated. Lateral biopsy scars can be reinforced by the dermal sling support, thereby decreasing the chances of exposure of the implant. The implant is also successfully and easily held in position by

Long-term outcomes of skin-sparing mastectomy and nipple-sparing mastectomy versus traditional mastectomy in breast cancer: a case-control study based on preoperative ultrasound and clinical indicators.

BACKGROUND AND OBJECTIVE: Nipple-sparing mastectomy (NSM) and skin-sparing mastectomy (SSM) are recognized for their aesthetic benefits in breast cancer patients. However, detailed evaluations with large samples of their long-term oncological effectiveness are limited. This study aims to compare the long-term oncologic outcomes of NSM/SSM and traditional mastectomy (TM) in patients with stage I-III breast cancer and to identify influential preoperative factors. METHODS: Among the 12,802 breast cancer patients who underwent surgery from 2009 to 2022 in West China Hospital of Sichuan University

A Comparison of Modified Radical Mastectomy, Breast Conservation Therapy, and Nipple-sparing Mastectomy With Immediate Implant Reconstruction.

BACKGROUND: This study evaluated and compared the perioperative outcomes and quality of life associated with modified radical mastectomy (MRM), breast-conserving surgery (BCS), and nipple- and areola-sparing subcutaneous mastectomy with immediate breast reconstruction (NSM-IBR). METHODS: This retrospective cohort study included 105 patients with breast cancer, stratified into MRM (n = 35), BCS (n = 35), and NSM-IBR (n = 35) groups. The perioperative outcomes (operative times, intraoperative blood loss, and recent complications) were subsequently compared across the 3 groups. Differences in pa

Comparison of patient characteristics and outcomes of contralateral prophylactic mastectomy and unilateral total mastectomy in breast cancer patients.

BACKGROUND: There has been an increasing trend toward contralateral prophylactic mastectomy (CPM) in the management of breast cancer (BCa). This study's objective was to compare clinicopathologic characteristics of BCa patients who elected CPM to those who elected unilateral total mastectomy (UTM) and to determine whether CPM improved survival. METHODS: Comparison was performed on 355 patients with stage 0-III BCa matched by age and stage who underwent mastectomy from 1995 to 2008: 177 patients had CPM; 178 patients had UTM. Clinicopathological characteristics and survival outcomes were analy

Contralateral Risk Reduction Mastectomy in Patients With Unilateral Breast Cancer Scheduled for Mastectomy: A Multidisciplinary Survey; Physicians' Perspective.

INTRODUCTION: The number of contralateral risk reduction mastectomies (RRM) continues to rise. Despite the existence of many guidelines, there is no general consensus on which patients should be considered for RRM. This survey was distributed among various breast specialists with the goal of evaluating current practices, perspectives, and attitudes towards RRM. METHODS: An English-language survey containing ten questions was designed and distributed electronically to members of the Senologic International Society (SIS). The findings of this survey align with existing research and also provide

Stewart Treves Syndrome in a Woman with Mastectomy.

Stewart Treves Syndrome is an angiosarcoma generally seen long time after radical mastectomy in breast carcinoma patients in chronic lymphoedema area. It's an angiosarcoma developed on a long standing lymphoedema, following a radical mastectomy. An 86-year-old woman was referred to our Dermatology outpatient clinic which developed a giant erythematous, oedematous, purplish lobulated plaque on her right forearm anteromedially with a few satellite erythematous papulonodules on her arm. The pathology revealed spindle-shaped tumour cells invading dermis with vascular differentiation into the subcu

Evolution of radical mastectomy for breast cancer.

Surgical treatment of breast cancer has been marked by a constant evolution since the Halsted radical mastectomy described in the late 19th century has become the current standard Madden radical mastectomy, a breast surgery that involves the ablation of tissue with the axillary lymphatic preserving both pectoral muscles. The purpose of this paper was to present the stages that have marked the evolution of this intervention and to provide an overview of the way breast cancer has been understood and treated in the last century.

Prophylactic mastectomy for the prevention of breast cancer.

BACKGROUND: Recent progress in understanding the genetic basis of breast cancer has increased interest in prophylactic mastectomy (PM) as a method of preventing breast cancer. OBJECTIVES: (i) To determine whether prophylactic mastectomy reduces death rates from any cause in women who have never had breast cancer and in women who have a history of breast cancer in one breast, and (ii) to examine the effect of prophylactic mastectomy on other endpoints, including breast cancer incidence, breast cancer mortality, disease-free survival, physical morbidity, and psychosocial outcomes. Complications of mastectomy and their relationship to biopsy technique.

BACKGROUND: Wound complication rates after mastectomy are associated with several factors, but little information is available correlating biopsy technique with the development of postmastectomy wound complications. Fine-needle aspiration (FNA) biopsy is an accurate method to establish a diagnosis, but it is unknown whether this approach has an impact on complications after mastectomy. METHODS: Charts of 283 patients undergoing 289 mastectomies were reviewed to investigate any association between biopsy technique and postmastectomy complications. RESULTS: The diagnosis of breast cancer was m

Predictors of and satisfaction with bilateral prophylactic mastectomy.

BACKGROUND: Women with a first-degree relative with breast cancer are at increased risk of developing this disease. The optimal medical management of these women is unclear, with options including close breast cancer screening, bilateral prophylactic mastectomy, or participation in chemoprevention trials. Among women who undergo prophylactic bilateral mastectomy, very little is known about satisfaction with this surgery. Also, we know very little about variables related to prophylactic mastectomy decision making. METHODS: Participants were women at increased risk of breast cancer due to famil
