Intraoperative ultrasonography-guided surgery: An effective modality for breast conservation after neo-adjuvant chemotherapy.

Margin status is one of the significant prognostic factors for recurrence in breast-conserving surgery (BCS). The issue that merits consideration for oncologic safety and cost-effectiveness about the modalities to assure clear margins at initial surgical intervention remains controversial after neo-adjuvant chemotherapy (NAC). The presented study aimed to assess the impact of intraoperative ultrasound (IOUS)-guided surgery on accurate localization of tumor site, adequacy of excision with clear margins, and healthy tissue sacrifice in BCS after NAC. Patients who had IOUS-guided BCS ater NAC wer

Sexuality of women after mastectomy.

Changes in sexual functioning, attitudes and sexual behaviour in local women after mastectomy and conservative breast surgery for breast cancer were assessed in this study. Given the social bearing, only married women agreed to be entered into the study. The complexities of variables other than breast loss, contributing to sexual difficulties were recognized and assessed. Through questionnaires designed for local women, a significant proportion of mastectomy women were found to have disturbances in at least one area in their sexual functioning after surgery. A significant number experienced ch

Impact of Surgical Approach on Survival and Recurrence in Borderline and Malignant Phyllodes Tumours: A Systematic Review and Meta-Analysis.

BACKGROUND: Phyllodes tumours of the breast exhibit a spectrum of biologic behaviour, with a predisposition to recur locally and metastasize distantly in their malignant form. There are limited data regarding the optimal surgical approach. OBJECTIVES: Our systematic review and meta-analysis aimed to determine the impact of breast conserving surgery (BCS) compared to mastectomy on overall survival (OS), disease-free survival (DFS), and local recurrence (LR) for patients with borderline and malignant phyllodes tumours to guide treatment planning. DATA SOURCES: We identified relevant studies fr

Locoregional recurrence and mortality after breast cancer surgery in patients aged 85 and older.

PURPOSE: Life expectancy is increasing and the number of patients age ≥ 85 with breast cancer (BC) may be on the rise. We report descriptive statistics regarding surgical management and outcomes in this cohort. METHODS: Patients ≥ 85 years surgically treated for a primary or in-breast tumor recurrence (IBTR) were identified through Oncoshare, a database that links Stanford Healthcare and Sutter Health electronic medical records with the California Cancer Registry. RESULTS: From 2010 to 2019, 91 patients, 65 (71%) aged 85-89 and 26 (29%) ≥ 90 + years were identified. 29.2% were screen-detecte

The impact of breast reconstruction compared with no reconstruction on breast cancer-related lymphedema: A systematic review and meta-analysis.

INTRODUCTION: Breast cancer-related lymphedema affects 21.9% of patients. The role of breast reconstruction in lymphedema prevention remains unclear. This review aimed to assess the relative risk of breast cancer-related lymphedema after breast reconstruction compared with mastectomy and breast-conserving surgery, which has thus far been inconclusively assessed. METHODS: We conducted a systematic review and meta-analysis following Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines. PubMed, EMBASE, Cochrane Central, and gray literature searches identified studies rep

Rural-urban differences in secular trends of locoregional treatment for ductal carcinoma in situ: A patterns of care analysis.

PRECIS: Omission of PORT following BCS remains high among rural patients despite evidence that PORT leads to a significant reduction in the risk of local recurrence. Further research is needed to examine the impact of rural residence on treatment choices and develop methods to ensure equitable care among all breast cancer patients. BACKGROUND: Despite national guidelines, debate exists among clinicians regarding the optimal approach to treatment for patients diagnosed with ductal carcinoma in situ (DCIS). While regional variation in practice patterns has been well documented, population-based

Breast Biopsy During Post-treatment Surveillance of Screen-Detected Breast Cancer Patients Yields High Rates of Benign Findings.

BACKGROUND: The incidence of breast biopsy following treatment for breast cancer is not well-characterized. We sought to determine the frequency and outcomes of breast biopsy and the need for subsequent surgery in patients treated with breast-conserving surgery (BCS). METHODS: Using a prospective database, we identified patients in Alberta, Canada, treated with BCS for screen-detected breast cancer or ductal carcinoma in situ (DCIS) from 2010 to 2014. Post-treatment breast procedures were identified from physician claims data. Multivariable analysis was performed to identify factors associate

Adoption of SSO-ASTRO Margin Guidelines for Ductal Carcinoma in Situ: What Is the Impact on Use of Additional Surgery?

BACKGROUND: Historically, more than one-third of patients with ductal carcinoma in situ (DCIS) treated with breast-conserving surgery (BCS) underwent additional surgery. The SSO-ASTRO guidelines advise 2 mm margins for patients with DCIS having BCS and whole-breast radiation (WBRT). Here we examine guideline impact on additional surgery and factors associated with re-excision. PATIENTS AND METHODS: Patients treated with BCS for pure DCIS from August 2015 to January 2018 were identified. Guidelines were adopted on September 1, 2016, and all patients had separately submitted cavity-shave margin

Management of chylous leakage after breast surgery: report of four cases.

Chylous leakage is a rare occurrence after breast surgery. Trauma to the thoracic duct and its main distribution may be the main cause of chylous leakage, but the cause of such an injury during breast surgery is unknown. We herein report the first case of chylous leakage from many minimal lymph ducts in a skin flap. In all, four cases of chylous leakage were observed out of 1 096 breast cancer patients who underwent axillary lymph node dissection. Of these four cases, three cases were successfully treated conservatively; however, surgical exploration was performed in the fourth case because of

The role of radiologists in the era of oncoplasty.

New morphological and functional breast imaging techniques, progress in neoadjuvant treatments, and the development of new surgical techniques that include oncoplastic surgery, have made it possible to perform breast-conserving surgery in patients with large multifocal or multicentric breast tumours, where previously mastectomy would have been recommended. This allows for better aesthetic results, adequate oncological treatment, and improved quality of life. As a result, oncoplastic surgery has increased in local breast cancer treatment with radiologists playing a significant role in its plann

Local flaps for partial breast reconstruction.

INTRODUCTION: Breast-conserving surgery (BCS) is often the first-line treatment for early-stage invasive breast cancer. Many studies claim that survival is non-inferior to radical treatments in selected cases. Although BCS preserves some breast tissue and often the nipple-areola complex, asymmetry and distortion of the breast contour may occur and reconstruction is sometimes required. The authors' aim is to conduct a review of main breast reconstruction options with local flaps after partial mastectomy. EVIDENCE ACQUISITION: A literature review was conducted on PubMed using the keywords: "par

Can we improve breast cancer management using an image-guided histopathology workup supported by larger histopathology sections?

PURPOSE: Breast radiologists examine the entire breast in full-size images, while breast pathologists examine small tissue samples at high magnification. The diagnostic information from these complementary imaging approaches can be difficult to integrate for a more clinically relevant evaluation of malignancies spanning several centimetres. We have explored the advantages and disadvantages of imaging guided larger section pathology techniques compared with the standard 2 × 2.5 cm. small section technique. METHODS: We compared the ability of conventional small section histopathology with large

Usefulness of near-infrared fluorescence for identifying the segmental bronchus in minimally invasive segmentectomy.

Minimally invasive anatomical sublobar resections have gained relevance in recent years mainly due to advances in imaging techniques, screening programs and the increase in second neoplasms. Accurate identification of the segmental or subsegmental bronchus is vital to guarantee optimal results in segmentectomies and subsegmentectomies. Given the complexity and the possibility of anatomical variations, several authors have published different methods to identify the target bronchus. However, these methods have certain limitations. This article describes a new rapid and effective technique, with

Vacuum intraoperative specimen mammography: A novel technique.

OBJECTIVE: Intraoperative specimen mammography (ISM) is a diffuse technique that allows surgeons to check specimens immediately after lumpectomy. Although the specimen is slightly compressed, the radiological image can be distorted by tissue overlap, and this may affect the evaluation of tumour borders, resulting in extension of the lumpectomy. As ISM may be less precise due to inadequate compression, a vacuum effect was applied to the specimen to increase the precision of margin detection. STUDY DESIGN: This study was conducted at St. Anna Hospital Breast Unit, Turin, Italy. Women who underw

Partial mastectomy reconstruction.

The treatment of breast cancer is an evolving field. Different modalities are continuously being developed to maximize patient survival while minimizing the treatment's morbidity. Currently, the two main options for the management of primary breast cancer are total mastectomy and partial mastectomy with radiation. Although partial mastectomies (lumpectomy or quadrantectomy) conserve the nipple and areola complex and native breast tissue, asymmetry and distortion of the breast can still occur. Many methods of reconstruction have been described. The early and long-term effects of radiation also

Male breast hemangioma--a rare entity: a case report and review of the literature.

We report the case of a 77-year-old man with a very rare breast tumor: hemangioma. The clinical examination found a large (6 cm diameter) lump and mammography revealed a well-defined high-density lesion with lobulated contour. Simple mastectomy was performed. The histology confirmed the diagnosis of benign hemangioma. There has been no local recurrence after 9 years of follow-up. There is very little literature on this type of tumor in men. Generally hemangiomas are large and ultrasound and magnetic resonance imaging (MRI) are useful for the diagnosis. The important differential diagnosis with

Three-dimensional histological electrophoresis with fluorescence labeling for detection of malignant tissue contours in precise conserving surgery.

BACKGROUND: Breast-conserving surgery (BCS) is the primary surgical approach for patients with breast cancer. The accurate determination of surgical margins during BCS is critical for patient prognosis; however, time constraints and limitations in current pathological techniques often prevent pathologists from performing this assessment intraoperatively. The inability to reliably assess margins during surgery can lead to incomplete tumor removal and the need for additional surgeries. The challenge remains to develop a real-time, reliable method for margin assessment during surgery to improve p

Secondary Breast Angiosarcoma After a Primary Diagnosis of Breast Cancer: A Retrospective Analysis of the Surveillance, Epidemiology, and End Results (SEER) Database.

OBJECTIVES: Angiosarcoma is a rare complication of breast-conserving therapy. This study evaluated the change in incidence between 1992 and 2016 of secondary breast angiosarcoma (SBA) in patients with a history of breast cancer and the impact of management strategies for the original breast carcinoma on angiosarcoma treatment. METHODS: Breast cancer and angiosarcoma cases were abstracted from the Surveillance, Epidemiology, and End Result (SEER) database. SBAs were defined as angiosarcomas located in the breast occurring after a prior breast cancer diagnosis. Primary breast angiosarcomas (PBA
