Implications of 'low' maternal serum alpha-fetoprotein levels: are maternal age risk criteria obsolete?

An association has been reported between 'low' maternal serum alpha-fetoprotein (MSAFP) and fetal chromosome abnormalities, notably Down syndrome. We suggest the predictive value be used for genetic counselling when a 'low' MSAFP is found, and present an illustrative risk table. It can also be seen that normal MSAFP in a woman may lower her age-related risks below that previously defined as 'high risk'. However, until good estimates of sensitivity and specificity are available from prospective, population based studies, patients should be told that any risk estimates are rough approximations.

Severe maternal morbidity in patients of advanced maternal age with and without metabolic syndrome.

PURPOSE: To estimate the burden of metabolic syndrome (MetS) on severe maternal morbidity (SMM) in women of advanced maternal age (AMA), quantify age-related escalation in SMM risk, and evaluate sociodemographic influences. METHODS: We retrospectively conducted a cross-sectional, population-level analysis of the Nationwide Inpatient Sample (Q4 2015-2019), a discharge-level database, to identify delivery hospitalizations among patients aged ≥ 35 years. MetS was defined by ICD-10 codes indicating ≥ 3 of the following: hypertension, type 2 diabetes, obesity, hypertriglyceridemia, or low high-den

Maternal and perinatal complications according to maternal age: A systematic review and meta-analysis.

OBJECTIVE: To evaluate the risk levels for maternal and perinatal complications at > 40, > 45 and > 50 years old compared with younger controls. METHODS: Electronic databases were searched from their inception until March 2021. We included studies reporting pregnancy outcome in pregnant women aged 40, 45, and 50 years or older compared with controls at the time of delivery. Case reports and case series were excluded. The primary outcome was the incidence of stillbirth. Meta-analysis was performed using the random effects model of DerSimonian and Laird, to produce summary treatment effects in

The Vaginal Microbiota of Pregnant Women Varies with Gestational Age, Maternal Age, and Parity.

The composition of the vaginal microbiota is heavily influenced by pregnancy and may factor into pregnancy complications, including spontaneous preterm birth. However, results among studies have been inconsistent due, in part, to variation in sample sizes and ethnicity. Thus, an association between the vaginal microbiota and preterm labor continues to be debated. Yet, before assessing associations between the composition of the vaginal microbiota and preterm labor, a robust and in-depth characterization of the vaginal microbiota throughout pregnancy in the specific study population under inves

Maternal biological age assessed in early pregnancy is associated with gestational age at birth.

Maternal age is an established predictor of preterm birth independent of other recognized risk factors. The use of chronological age makes the assumption that individuals age at a similar rate. Therefore, it does not capture interindividual differences that may exist due to genetic background and environmental exposures. As a result, there is a need to identify biomarkers that more closely index the rate of cellular aging. One potential candidate is biological age (BA) estimated by the DNA methylome. This study investigated whether maternal BA, estimated in either early and/or late pregnancy,

Advanced maternal reproductive age elevates myopia risk in offspring.

AIMS: To evaluate the associations of parental reproductive age with the risk of myopia and high myopia in adult offspring. METHODS: This cross-sectional study included 14 044 UK Biobank participants. Myopia and high myopia were defined as a mean spherical equivalent (spherical power+1/2 cylindrical power) of both eyes ≤-0.5 dioptres (D), and ≤-6.0 D, respectively. Parental age was categorised as <25, 25-29, 30-34 and ≥35 years. Logistic regression was conducted after adjusting for age, sex, race, time spent outdoors, Townsend Index, income, education, body mass index, smoking and drinking. I

Mechanisms of oocyte aneuploidy associated with advanced maternal age.

It is well established that maternal age is associated with a rapid decline in the production of healthy and high-quality oocytes resulting in reduced fertility in women older than 35 years of age. In particular, chromosome segregation errors during meiotic divisions are increasingly common and lead to the production of oocytes with an incorrect number of chromosomes, a condition known as aneuploidy. When an aneuploid oocyte is fertilized by a sperm it gives rise to an aneuploid embryo that, except in rare situations, will result in a spontaneous abortion. As females advance in age, they are a

Outcomes of Women Delivering at Very Advanced Maternal Age.

BACKGROUND: The purpose of this study was to evaluate the maternal outcome in women with very advanced maternal age (VAMA) at childbirth (>45 years) compared to advanced maternal age (35-39 and 40-44 years). METHODS: Retrospective cohort study using the Texas Public Use Data File, years 2013-2014. Maternal age was a three-level variable: 35-39 (referent), 40-44, and 45-59 years (VAMA). Adjusted risk ratios (aRRs) for the two older age groups for various obstetrical and nonobstetrical complications were calculated from log-binomial regression models. RESULTS: The sample consisted of 96,879 de

Maternal Age and Contractility of Human Myometrium in Pregnancy.

There is controversy as to whether maternal age exerts an influence on the contractility of human myometrium in pregnancy. The aim of this study was to examine a series of functional contractile parameters of human myometrium in vitro, over a broad range of maternal ages. Myometrial tissue specimens were obtained at cesarean delivery from 32 women with maternal ages ranging from 28 to 52 years. Using in vitro recordings, a number of contractile parameters including maximal amplitude, mean contractile force, time to maximal amplitude, maximum rate of rise, and occurrence of simple and complex (

Maternal age and risk of labor and delivery complications.

We utilized an updated nationally representative database to examine associations between maternal age and prevalence of maternal morbidity during complications of labor and delivery. We used hospital inpatient billing data from the 2009 United States Nationwide Inpatient Sample, part of the Healthcare Cost and Utilization Project. To determine whether the likelihood that maternal morbidity during complications of labor and delivery differed among age groups, separate logistic regression models were run for each complication. Age was the main independent variable of interest. In analyses that

Teen maternal age and very preterm birth of twins.

BACKGROUND: As teen singleton pregnancy is associated with higher risks of adverse birth outcome, and twin pregnancy, regardless of maternal age, may result in poor outcome, teens pregnant with twins may represent a particularly vulnerable group. However, little has been documented regarding teen twin pregnancy outcome. OBJECTIVE: To characterize the risk of very preterm birth among teens having twins. DESIGN: Cross-sectional analysis of the US 1995-2000 Matched Multiple Birth Data Set. METHODS: We calculated the risk of very preterm birth (<33 weeks' gestation) for teen and young adult mot
