Near miss maternal morbidity.

Audit of severe maternal morbidity is a potent tool in determining standards of maternity care. This study determines the incidence of severe acute maternal morbidity in our population, identifies the underlying organ dysfunction and associated obstetric risk factors, and compares them to published international reports. Over a 5 year period, 1999-2003, data were collected prospectively from patients with severe acute maternal morbidity. There were 36,802 women who delivered infants weighing more than 500 g over the 5 years with 53 cases of severe maternal morbidity. There were two indirect ma

[Preventive check-ups of pregnant women in Denmark. Common ailments in pregnancy].

The study was conducted in order to describe the extent and content of advice on common ailments in pregnancy given by doctor or midwife during prenatal visits and to describe the frequency of ailments in the period before the visits. The design was a nationwide cross-sectional study based on questionnaires completed by pregnant women who had seen a general practitioner (GP), midwife or hospital doctor for prenatal care. Ailments and advice in connection with one specific visit were reported. The questionnaires were completed by 517 women after a prenatal visit to their GP (92% of eligible), b

[Importance of maternity routines in the success of maternal breast feeding].

Many of the problems that are associated with breast-feeding appear to be caused by various postnatal care procedures that take place in the majority of our hospitals. In order to identify these obstacles, a survey was carried out on a sample of 400 mothers whose children were born in Móstoles in 1989. The results were as follows: 1) The average duration of breast-feeding was 2.43 months (SD = 2.19 months). 2) The lower the baby's birth weight and the longer that he was kept in the hospital, the shorter the time that he was breast-fed. Two basic procedures in the hospital "norms" appear to con

Caesarean section for the dead baby--an unhappy reality.

Caesarean section for the dead baby is sometimes still needed to prevent maternal complications. The objective of the study is to critically analyse the characteristics of the mother and indications for the operation in women who delivered stillbirths following caesarean section. The study period covered a 2 years span from January, 2003 to December, 2004. During this time 121 mothers (study group) underwent caesarean section for the dead baby representing 1.41% of all caesarean section operations done in the hospital. The study group was compared to the overall caesaren section done during th

Smoking and adverse maternal and child health outcomes in Brazil.

INTRODUCTION: Numerous studies from high-income countries document the causal relationship between cigarette smoking during pregnancy and adverse maternal and child health (MCH) outcomes. Less research has been conducted in low and middle income countries, but a burgeoning literature can be found for Brazil. METHODS: We review Brazilian studies of the prevalence of maternal smoking, the relative risk of smoking-attributable adverse MCH outcomes, and present new estimates for these outcomes, using the attributable fraction method. RESULTS: We found that Brazilian studies of the relative risks

Challenges of dealing with migrant brick kiln workers.

Brick kilns in rural India exemplify extreme poverty and exploitation, as seasonal migrant labourers work long hours for minimal pay, living in cramped, poorly ventilated dwellings. Children often begin working at a young age, with irregular or no schooling. Access to healthcare is severely limited, with maternal care, immunisation, and disease screening being sporadic and follow-up rare. Poor nutrition and widespread substance use further compromise health. Lacking identification or permanent addresses, these families are excluded from social welfare schemes. This situation reflects a profoun

[Indicators of maternal and infant health: implications of the 10th revision of the International Classification of Diseases].

Since the end of the 1940s, when the World Health Organization assumed responsibility for the decennial revisions of the Classification of Causes of Death, the Classification came to include diseases and definitions of use in vital statistics, resulting in the Sixth International Classification of Diseases (ICD-6). The most recent revision of this work, the International Statistical Classification of Diseases and Related Health Problems (ICD-10), is more significantly different than any revision since ICD-6, especially in the area of maternal and child health. Among the changes introduced in I

Mandated supporting, not reporting: Reimagining maternal health care for women who use drugs.

This study examines how carceral logics shape maternal healthcare for pregnant women who use drugs. Through qualitative interviews with 21 women and 14 providers in Florida, we explore how punitive surveillance, via mandatory reporting and child welfare investigations, disrupts prenatal care and exacerbates stigma. We find that actual and anticipated involvement of child welfare services (CWS) inadvertently operates as an extension of poverty governance. Participants described deep fear of CWS, particularly newborn removal, as a central reason for avoiding prenatal care. Many reported dehumani

The Fourth Trimester of Pregnancy: Committing to Maternal Health and Well-Being Postpartum.

The postpartum period is a time of significant challenge and need as women adapt to hormonal and physical changes, recover from delivery, experience shifting family responsibilities, and endure sleep deprivation, all while caring for and nourishing their newborn. It is also a period of significant maternal health risk. Recent data on U.S. maternal mortality indicate a shift in the timing of maternal deaths over the past 10 years, with the majority of maternal deaths now occurring postpartum, from one day to one year after delivery. Postpartum care also marks a period of transition, as women sh

Experiences with maternal and perinatal death reviews in the UK--the MBRRACE-UK programme.

Established in 1952, the programme of surveillance and Confidential Enquiries into Maternal Deaths in the UK is the longest running such programme worldwide. Although more recently instituted, surveillance and confidential enquiries into perinatal deaths are also now well established nationally. Recent changes to funding and commissioning of the Enquiries have enabled both a reinvigoration of the processes and improvements to the methodology with an increased frequency of future reporting. Close engagement with stakeholders and a regulator requirement for doctors to participate have both suppo

The use of continuous surveys to generate and continuously report high quality timely maternal and newborn health data at the district level in Tanzania and Uganda.

BACKGROUND: The lack of high quality timely data for evidence-informed decision making at the district level presents a challenge to improving maternal and newborn survival in low income settings. To address this problem, the EQUIP project (Expanded Quality Management using Information Power) implemented a continuous household and health facility survey for continuous feedback of data in two districts each in Tanzania and Uganda as part of a quality improvement innovation for mothers and newborns. METHODS: Within EQUIP, continuous survey data were used for quality improvement (intervention di

Maternal mortality in Italy, 1980-1996.

OBJECTIVE: To establish the extent of maternal mortality in Italy in between 1980 and 1996 in order to compare it with the international data. STUDY DESIGN: We conducted a retrospective study on maternal deaths in Italy from 1980 to 1996. Data have been collected by Italian Statistic Institute (ISTAT). We calculated both the maternal mortality rates and the percentages of causes of death in the whole period, according to WHO definitions. RESULTS: The data confirmed the trends of the previous decade: maternal mortality rates have decreased from 13.25 (1980) to 3.78 (1996) for 100000 live birt

Families with young children in homeless shelters: Developmental contexts of multisystem risks and resources.

We apply a multisystem perspective to three aims relevant to resilience for young children in emergency and transitional homeless shelters. We consider profiles of risks and resources before shelter, early childhood program enrollment during shelter, and the likelihood of returning to shelter or having a subsequent child welfare placement. We used longitudinal, city-wide data from multiple sources integrated at the individual level across the lifespan for 8 birth cohorts. Young children (N = 1,281) stayed in family shelters during an 18-month period during a multisystem intervention. Risk fact

Risk of Child Welfare System Involvement Among Mothers With Substance Use Disorder.

INTRODUCTION: This paper aimed to examine the factors associated with child welfare system involvement among mothers receiving public substance use treatment services. METHODS: This was a cohort study using 2015-2023 linked data from Allegheny County, Pennsylvania, on residents' interactions across public service systems. Independent variables, measured within 3 months of study inclusion, included family characteristics, public benefit receipt, jail release, and substance use disorder diagnoses. The data, captured over the subsequent 21 months, included any child welfare interaction, active c

Development and validation of a vitamin D status prediction model in Danish pregnant women: a study of the Danish National Birth Cohort.

Vitamin D has been hypothesized to reduce risk of pregnancy complications such as preeclampsia, gestational diabetes mellitus, and preterm delivery. However, many of these outcomes are rare and require a large sample size to study, representing a challenge for cohorts with a limited number of preserved samples. The aims of this study were to (1) identify predictors of serum 25-hydroxy-vitamin D (25(OH)D) among pregnant women in a subsample (N = 1494) of the Danish National Birth Cohort (DNBC) and (2) develop and validate a score predicting 25(OH)D-status in order to explore associations betwee
