Evaluation of the three-dimensional soft tissue changes after anterior segmental maxillary osteotomy.

The aim of this study was to assess the three-dimensional soft tissue changes following anterior segmental maxillary osteotomy, in terms of magnitude and direction in correlation to hard tissue changes, using cone beam computed tomography (CBCT). The study included 6 patients (age range 17-35 years) suffering from dental maxillary protrusion that required surgical correction by anterior segmental maxillary osteotomy. For each patient, preoperative and postoperative CBCT, photographs, and orthodontic casts were taken before and 6 months after operation to analyse soft tissue changes, in terms o

Treatment of ankylosed maxillary central incisors by segmental osteotomy with autogenous bone graft.

This case report describes the treatment of a 16-year-old girl with ankylosed maxillary central incisors that were noticeably infraoccluded and labially displaced. We performed a segmental osteotomy with an autogenous bone graft in a single-stage surgery to align and level the ankylosed teeth. The dento-osseous segment was successfully repositioned with satisfactory periodontal results.

Craniomaxillofacial Fibrous Dysplasia: Conservative Treatment and Maxillary Osteotomy Using the Schuchardt-Kufner Technique.

Fibrous dysplasia (FD) is a disturbance of the mesenchymal tissue that accounts for 2.5% of all bone tumors and more than 7% of nonmalignant bone tumors. In the craniomaxillofacial region, FD affects the calvaria, skull base, zygoma, and jaws, the prevalent site being the maxilla (50% of cases). Therapy for craniomaxillofacial FD is surgical. The goals of surgery are to prevent functional disorders and restore facial symmetry, volume, and contour. In this article, we present a case of a young female patient affected by right orbital-zygomatic-maxillary FD. She had developed facial asymmetry an

Palatal window osteotomy technique improves maxillary sinus augmentation in previously insufficient augmentation case.

INTRODUCTION: Perforation of the Schneiderian membrane is the most common complication in sinus floor augmentation (SFA). When volume of grafting is qualified to prevent enlargement of the membrane perforation, lack of bone volume may occur in optimal site. CASE PRESENTATION: SFA was performed in sites #24 to 26 in a 63-year-old male. However, a 10-mm size perforation of the Schneiderian membrane occurred in site #26. Although the sinus cavity was grafted with deproteinized bovine bone mineral (DBBM) after repair of membrane perforation, insufficient bone formation was observed on palatal and

Tooth pulp changes following Le Fort I maxillary osteotomy in a primate model.

Le Fort I maxillary 'down-fracture' osteotomy with 8 mm advancement was performed in 15 adult rhesus monkeys. Forty-five tooth pulps were examined histologically at intervals from 0 to 24 weeks after surgery. Cell degeneration occurred in 31% of pulps examined, necrosis in 16% of pulps and osteo-dentine was found in 7% of pulps. Almost half of the teeth examined (47%) showed marked cellular changes, more frequently found in posterior teeth. Other features noted were inflammation (13%) and reactive dentine in pulps (24%). Axons degenerated initially but recovered by 24 weeks. It is concluded th

Surgical-orthodontic correction of maxillary retrusion by Le Fort I osteotomy and proplast.

The combined efforts of the orthodontist and surgeon are necessary for successful treatment of most patients with maxillary retrusion. Both the aesthetic and functional manifestations of this dento-facial deformity can usually be effectively treated by maxillary advancement, augmentation of the midfacial region and orthodontics. Treatment planning, pre- and post-surgical orthodontic treatment principles, and surgical considerations in treating 25 adults with maxillary retrusion are described and illustrated. Surgical advancement of the maxilla by Le Fort I osteotomy, proper alignment of the te

Partial maxillary osteotomy following an unsuccessful forced eruption of an impacted maxillary canine: 10 year follow-up. Review and case report.

The maxillary canines are amongst the most frequently impacted teeth, second only to the third molars. Several conservative orthodontic and surgical techniques are available to position the teeth properly in the dental arch, even in severe cases. However, when an extraction is necessary, it often leaves a critical alveolar defect of difficult management. The authors present the technique of partial maxillary osteotomy, in which a dento-alveolar segment is moved mesially, hence closing the remaining space, allowing for the formation of healthy periodontium and resulting in an adequate functiona

Surgically assisted maxillary expansion: Influence of piezosurgery on the complications linked to the midline osteotomy.

Transverse maxillary deficiency requires surgical maxillary expansion when the midpalatal suture is closed. The midline osteotomy of the maxilla can lead to significant dental, gingival and bone complications. Technically, this osteotomy is usually performed using osteotomes, a burr or an oscillating saw, but is increasingly being replaced by piezosurgery. There are no published studies on the impact of piezoelectric devices on complications. This study tries to evaluate complications after median inter-incisors osteotomy (gingival recession, tooth loss, bone loss) with piezosurgery, and then

Are Vowels Normalized After Maxillary Osteotomy? An Acoustic Study in Cleft Lip and Palate.

BACKGROUND: Maxillary hypoplasia is a common skeletal condition in cleft lip and palate (CLP). Maxillary osteotomy is typically used to reposition the maxilla in CLP with maxillary hypoplasia. Previous studies have suggested that vowel articulations are adjusted postsurgically due to altered vocal tract configuration and articulatory reorganization. This acoustic study aims to investigate whether vowels are normalized postoperatively and to explore the nature of articulatory reorganization. METHODS AND PROCEDURES: A prospective study was conducted to examine the vowel production of a group of

Management of ankylosed maxillary canine with single-tooth osteotomy in conjunction with orthognathic surgery.

Ankylosis of teeth is the abnormal adherence of alveolar bone to dentin or cementum. Ankylosis of a submerged maxillary canine can be challenging when striving for an optimal occlusal and esthetic treatment outcome with orthognathic surgery. If an ankylosed tooth does not respond to orthodontic forces, surgical procedures may be indicated to facilitate movement of the tooth to the correct position including a single-tooth segmental osteotomy to reposition the alveolar bone including the ankylosed tooth. The objective of this case report is to describe the treatment of a patient with an ankylos

Maxillary sinus augmentation with a sonic handpiece for the osteotomy of the lateral window: a clinical report.

Several techniques have been proposed to manage patients with insufficient bone height for implant placement in the posterior maxilla. The lateral approach to sinus elevation is a successful procedure, with percentages of success close to 100%. Unfortunately, a frequent complication encountered during sinus elevation procedure is perforation of the Schneiderian membrane. In this clinical report, the authors present the application of a diamond coated sonic tip and an air-driven sonic instrument, commonly used in prosthodontics for the preparation of the lateral window osteotomy during sinus au
