INTRODUCTION: Median nerve ultrasound shows increased cross-sectional area (CSA) in carpal tunnel syndrome (CTS) and diabetic peripheral neuropathy (PN). The role of ultrasound in diagnosing CTS superimposed on diabetic PN is unknown. The objective of this study is to evaluate ultrasound for diagnosis of CTS in diabetic PN.
METHODS: Prospective recruitment of diabetics with electrodiagnostically proven PN, subdivided into cases (with CTS) or controls (without CTS). The gold standard for CTS was clinical diagnosis. NCS were correlated with blinded median nerve CSA ultrasound measurements.

The diagnosis of median nerve compression neuropathy at the carpal tunnel is usually confirmed by clinical electrophysiology. The classical findings of a significantly slowed median nerve conduction velocity for both sensory and motor fibres, with a prolonged distal motor latency and a reduced amplitude compared to age-related norms are unambiguous, but these criteria are often present only in part. In such cases another quantitative indicator of compression neuropathy would be extremely helpful. The present study aimed to test whether measurement of warm and cold sensory acuity in cases of pu

Background: Clenched fist syndrome is a rare disorder, often attributed to a conversion disorder without anatomic basis. Here, we review the literature surrounding clenched fist syndrome and challenge the assumption it is always psychiatric in origin, via description of a case of clenched fist syndrome responsive to surgical nerve decompression. Methods: An unusual case of clenched fist syndrome is reviewed and discussed. Results: A child presenting with clenched fist syndrome failed conservative measures consisting of formal hand therapy, multidisciplinary pain management, and psychiatric tre

A 55-year-old woman presented with an 18-month history of intermittent left-hand paresthesias affecting her first, second, and third digits. A small, immobile, nontender midline mass on the volar aspect of the left wrist was also present. Evaluation included normal routine nerve conduction studies and needle electromyography. An ultrasound was performed in the electromyography laboratory, verifying median nerve involvement of the mass within the proximal carpal tunnel. This finding guided the electromyographer to perform the unconventional technique of sensory nerve short segmental stimulation

Ten cases of acute carpal tunnel syndrome (ACTS) and six cases of nerve contusion were identified in patients with acute median neuropathy associated with blunt wrist trauma. The patients with ACTS initially had normal sensation and subsequently developed objective sensory loss (2-point discrimination greater than 15 mm) in the median nerve distribution associated with severe wrist pain. Patients with nerve contusion injuries had immediate sensory loss and symptoms were nonprogressive. Wick catheter measurements of the carpal canal pressure were used in seven patients to help distinguish ACTS

Two clinical and four surgical steps can be taken to return gliding and nutritional balance to the median nerve after revision surgery for a recurrent median nerve neuropathy: Step One (Clinical): An accurate diagnosis of the median nerve traction neuropathy. Consider brachial plexopathy and terminal neuropathies. Step Two (Surgical): Comprehensive exploration and meso-epineurolysis of the median nerve from the radial remnants of the divided TCL and fibrosed radial and ulnar bursae by resection of the bursae and flexor tenosynovectomy. Step Three (Surgical): Fasciotomy of the volar carpal liga

BACKGROUND: Myofascial pain syndrome may coexist with other musculoskeletal conditions, including entrapment neuropathies. The relationship between myofascial dysfunction and entrapment neuropathy remains poorly understood, and the evaluation and management of patients presenting with overlapping symptomatology can be challenging.
CASE REPORT: We report on the use of trigger point injections (TPIs) in a 49-year-old woman with median and ulnar entrapment neuropathies and concurrent myofascial pain. The patient underwent a series of 8 TPI sessions targeting muscles of the shoulder, arm, and han

This study aimed at identifying the anatomic structures which may be responsible for entrapment neuropathies of the median nerve. Thirty upper extremities of 15 formalin fixed adult cadavers were dissected from the axilla to the distal forearm under Zeiss Opmi 9--FC microscope. We encountered seven different anatomic structures that may compress the median nerve. These structures are the brachialis muscle, Struther's ligament, the bicipital aponeurosis, pronator teres, flexor digitorum superficialis, the accessory head of plexor pollicis longus (Gantzer's muscle) and vascular structures. The s

OBJECTIVE: The aim of this study is to elucidate the significance of electrophysiological data of the palmar cutaneous branch of the median nerve (PCBm) in the diagnosis entrapment of the PCBm concomitant with carpal tunnel syndrome (CTS).
MATERIALS AND METHODS: Clinical and electrophysiological studies were conducted for abnormal sensation of the thenar eminence in eight CTS patients. Sensory nerve action potentials (SNAPs) of the PCBm were recorded from bipolar surface electrodes over the forearm using an orthodromic method. The preoperative SNAP evaluation was compared with the operative f

The neurovascular bundle anatomy in the upper arm displays changing relationships of nerve and vascular structures along short segments. Fibrous tissues segregate these elements into enclosed compartments allowing for specific patterns of injury. We report a patient with a iatrogenic brachial artery injury in this region who featured combined median and MAC neuropathies, which were consistent with complete axonotmesis on neurophysiological assessment. Increased intracompartmental pressure may have led to nerve injury either thorough an ischemic mechanism or to focal compression. Recognition of

This retrospective pilot study was undertaken to help determine the usefulness of measuring sensory nerve action potential and mixed nerve action potential temporal dispersion in median neuropathy at the wrist (MNW; i.e., carpal tunnel syndrome). The records were reviewed for 34 patients who were referred to an electrodiagnostic medicine laboratory with normal antidromic median sensory nerve action potential (recording from the index finger), median transcarpal mixed nerve action potential, and ulnar transcarpal mixed nerve action potential peak distal latencies (NO group) and 29 patients with

The authors measured the carpal tunnel and intraneural pressures with a soft catheter in fifteen cases of leprous neuritis of the median nerve at the wrist. In the carpal tunnel, the pressure was about 40 mm of mercury, while within the nerve, the pressure was about 25 mm of mercury. In five healthy male volunteers, the same pressures were measured: the results were much lower (2 to 4 mm of mercury). The authors call attention to the high pressures found in acute leprous neuritis. They conclude that the sooner a surgical procedure (careful neurolysis) is performed, the better the results, befo

INTRODUCTION: Digital neuropathy is a pure sensory neuropathy of a digital nerve. It may be caused by acute or chronic local trauma or pressure, or accompany systemic illnesses such as rheumatoid disease, leprosy, Raynaud disease, dysproteinemia, or diabetes mellitus. We describe an extraordinary case of digital neuropathy of the median and ulnar nerves caused by Dupuytren contracture.
CASE REPORT: A 56-year-old right-handed man was presented with numbness and tingling of the little finger of the right and ring finger of the left hand. The clinical and EMG findings in this patient were consis

BACKGROUND: Nerve conduction (NC) tests, using rigid cut-offs separating normal from abnormal test values, are commonly used to confirm median neuropathy at the wrist (MNW). The authors studied patients with clinically defined mild MNW and a normal median distal motor latency to determine 1) how much sensory or mixed NC test results increase (or decrease) the probability of MNW and 2) the NC test values required to confirm (or exclude) MNW for the range of pretest probabilities of MNW.
METHODS: Palmar, digit 4 (D4), and digit 2 (D2) median NC tests were reviewed in 125 hands with mild carpal

Spontaneous anterior interosseous nerve (AIN) palsy develops following the resolution of nerve pain, which may be considered as distal neuralgic amyotrophy. NA is assumed to have a complex etiology, but an autoimmune mechanism is likely involved. However, precise assessment of the lesion is challenging. We examined five consecutive patients with suspected spontaneous AIN palsy using ultrasonography. On electromyography, all patients exhibited denervation potentials in the muscles, not only in the AIN territory, but also in the proximal median nerve territory (e.g., the flexor carpi radialis or

We report a case of heterotopic ossification formation 6 years after a revision carpal tunnel release in a 46-year-old woman, causing new-onset mixed ulnar and median nerve compression symptoms. The patient underwent excision of the heterotopic ossification mass along with decompression of the median and ulnar nerves, and postoperative radiation. Four years after treatment, the patient was completely asymptomatic with full range of motion in her hand and wrist.

BACKGROUND: Carpal tunnel syndrome (CTS) can be treated with open carpal tunnel release (OCTR) or endoscopic carpal tunnel release (ECTR). Our goal was to evaluate the safety and efficacy of ECTR versus OCTR in patients with severe CTS. We hypothesized that ECTR would be as safe and effective as OCTR in these patients.
METHODS: This was a retrospective cohort study of patients with severe CTS who underwent ECTR or OCTR by E. G. Deune between 2001 and 2014. Variables were patient age, sex, relevant medical history, alcohol and tobacco use, and preoperative electromyography and physical examina
