AIM: To define the scope of combined therapy in patients having local Hodgkin's disease (LHD) with massive mediastinal lesion.
MATERIAL AND METHODS: From 1980 to 1997 the treatment was given to 75 LHD patients with massive lesion of the mediastinum (mediastinal-thoracic index, MTI, > or = 0.33). The patients have received combined chemoradiotherapy according to 3 cycle CVPP (cyclophosphamide 600 mg/m2, vinblastine 6 mg/m2, prokarbasin 100 mg/m2, prednisolone 40 mg/m2) with a 2-week interval + radiation to all the lymphatic collectors above the diaphragm with a single focal dose 2 Gy to total

We studied the usefulness of thoracoscopic surgery in the diagnosis and treatment of pleural and mediastinal lesions. 1. Thoracoscopic surgery was the best method for biopsy of pleural lesions or effusion-retaining disease. Thoracoscopic surgery allowed extensive intrathoracic inspection and collection of relatively large tissue samples, even though it is less invasive than thoracotomy. 2. Thoracoscopic biopsy was useful in the diagnosis of tumorous lesions of the anterior, middle, and posterior parts of the mediastinum. 3. Thoracoscopic surgery is indicated for treatment of the following five

We analysed the risk of myocardial infarctions in 339 patients with Hodgkin's disease treated with radiotherapy (rt) with or without chemotherapy. A total of 112 patients underwent cardiac testing with echocardiography, rest and exercise electrocardiogram and myocardial scintigraphy. Nearly all patients have been treated with < 2.0 Gy per fraction to the anterior cardiac region. A significantly increased risk of myocardial infarctions or of sudden death has been observed (10 patients). No cardia events have been observed in 215 non-smokers without hypertension and without coronary artery disea

The appearance of a mediastinal mass in a patient with Hodgkin's disease in remission frequently indicates recurrence. However, benign processes such as rebound hyperplasia of the thymus gland should be included in the differential diagnosis. The authors present a case in which an anterior mediastinal mass was revealed by a routine chest radiograph 3 months after completion of combined modality treatment with low-dose radiation and MOPP chemotherapy for cervical Hodgkin's disease, stage I, nodular sclerosis. As the patient was clinically well with no other laboratory evidence of disease, recur

The chest radiograms (CXR) of 102 patients with Hodgkin's disease presenting with mediastinal involvement at diagnosis were reviewed to assess the incidence and relevance of residual mediastinal abnormalities after therapy. All patients had completed planned treatment and had no evidence of persisting extramediastinum disease at restaging. Thirty-nine cases (38 per cent) showed residual mediastinal widening at the end of therapy (minimal changes in nine and measurable changes in 30 cases). Neither presenting features nor treatment modality correlated with residual mediastinal mass on chest X-r

The authors report a case of Castleman's disease in the mediastinum that was discovered in a young woman receiving high-dose steroids for oral pemphigus vulgaris. Computed tomography demonstrated that the mass was calcified and was markedly enhanced after intravenous injection of contrast material, a manifestation of the hyaline-vascular histologic variant. Arteriography proved invaluable in planning the surgical approach to this highly vascular mass. Because Castleman's disease is rare most radiologists are not familiar with the radiographic features and are unaware that profuse hemorrhage ma

INTRODUCTION: Descending Necrotizing Mediastinitis (DNM) is the fatal form of mediastinitis and mostly develops as a complication of peritonsillar abscesses or dental-odontogenic infections. The aim of this study is to evaluate clinical and surgical feature of the patients with DNM who were managed in our clinic.
METHODOLOGY: We retrospectively evaluated 13 consecutive patients with the diagnosis of DNM between February 2005 and February 2018. All of them had the typical physical appearance, history and radiological findings.
RESULTS: Ten (77%) patients were male, 3 (23%) patients were femal

RATIONALE: Mycobacterium kansasii usually causes chronic pulmonary infections in immunocompetent patients. In contrast, disseminated M. kansasii disease is commonly associated with advanced human immunodeficiency virus infection, but is reported infrequently in other immunocompromised patients.
OBJECTIVES: To identify common clinical manifestations and potential risk factors for M. kansasii infection in patients with GATA2 deficiency.
METHODS: We reviewed M. kansasii disease associated with GATA2 deficiency at one institution and disease associated with primary and other immunodeficiencies r

A 51-year-old female underwent resection of a solid lesion in the posterior mediastinum, preoperatively interpreted at imaging as thoracic schwannoma, requiring double sequential surgical procedure to be resected. The histologic examination of the resected mass diagnosed a hyaline-vascular Castleman's disease.

Mediastinal granulomatous disease with resulting fibrosis is known to cause several serious complications, including pulmonary artery occlusion, bronchial stenosis, superior vena cava syndrome, and constrictive pericarditis. This process is typically the result of an excessive fibrogenic response to the antigen of the fungus Histoplasma capsulatum. We present a case of a 20-year-old man who had massive bilateral chylothoraxes develop as a complication of mediastinal granulomatous disease. Aggressive surgical therapy was necessary to treat this potentially life-threatening condition with an exc

A 35-year-old woman presented with symptoms of pulmonary embolism and thyrotoxicosis. Computed tomography of the chest ruled out pulmonary embolism but showed an incidental mediastinal mass. Graves' disease was confirmed and the mediastinal mass was ascribed to associating thymic hyperplasia. Treatment of Graves' thyrotoxicosis resulted in a spontaneous shrinkage of the mediastinal mass. Although it is relatively unknown, thymic hyperplasia is a normal and reversible manifestation of Graves' disease.

Castleman's disease, defined as angiofollicular lymphoid hyperplasia, is a rare lymphoproliferative disorder, which usually occurs in the chest. The tumor is often asymptomatic, but it can cause nonspecific thoracic symptoms such as cough and dyspnea. Surgical removal is curative and malignant transformation has not been described. We report an unusual case of Castleman's disease localized in the posterior mediastinum and bordering the chest wall, and review the relevant literature.

PURPOSE: To evaluate the risk of cardiac lesions after conventionally fractionated irradiation (Rt) of the mediastine with or without chemotherapy (Ct) in patients with Hodgkin's disease (HD) and to relate them to known cardiovascular risk factors.
PATIENTS AND METHODS: Between 1964 and 1992, 352 (total group) patients with HD were treated with curative intention using Rt with or without Ct including the mediastine and had a follow-up of at least 1 year. More than 96% of the patients had a complete follow-up. One hundred forty-four patients (64% of the living patients, heart study group) have

A residual mass after treatment of Hodgkin's disease points to the diagnosis of fibrotic residue or of persistent disease. Radiological and radionuclide imaging, although helpful, cannot differentiate between both conditions with absolute certainty. Thus, residual masses are followed up: stable or regressive masses strengthen the hypothesis of fibrotic residue, while enlarging masses evoke relapsing disease. We report on a patient with an enlarging residual mass after completion of therapy, which proved to be a benign thymic hyperplasia after histological analysis. Previously reported similar

Hodgkin's disease is highly curable today. Radiotherapy (RT) is the treatment of choice in the early stages. A mantle field is often used in the RT of Hodgkin's disease, and the technique and dosimetry are quite complex. We used computerized tomography (CT)-based dosimetry to determine doses delivered to different mediastinal nodes with the commonly used technique in Hodgkin's disease that was originally described by Kaplan. We used dose-volume histograms to determine doses to various groups of nodes in nine patients. Significant inhomogeneity (30%, 30%, 35%, 35%, 30%, 40%, 35%, 35%, and 30% i

Castleman's disease or pseudo-tumoral lymphoid hyperplasia is a rare entity. We report a case of Castleman's tumour in the antero-superior mediastinum which was revealed by a radiological examination in a young woman of 26. We also report immunohistochemical studies. There are two clinical presentations of Castleman's disease; a localised form (mediastinal or extrathoracic) and the multicentric variety. The localised form may be asymptomatic or can be accompanied by general features of inflammatory disease. Surgical excision of these localised forms both confirms the diagnosis and assures a cu
