Comparison of Short-Term Outcomes Between Robot-Assisted and Conventional Mediastinoscopic Esophagectomies: A Retrospective Study Using Propensity Score Matching.

BACKGROUND: Mediastinoscopic esophagectomy (ME) is a minimally invasive surgery for esophageal cancer that avoids one-lung ventilation and chest wall trauma. However, a major challenge with this technique is the relatively high incidence of left recurrent laryngeal nerve palsy (RLNP). To address this, we introduced a transcervical mediastinoscopic and transabdominal robot-assisted esophagectomy (RA-ME) technique. The clinical significance of robotic assistance in ME was evaluated by comparing RA-ME and conventional ME (Co-ME), using propensity score matching (PSM) to control for baseline chara

Tracheal injury diagnosed by a sudden increase in end-tidal carbon dioxide levels during mediastinoscopic subtotal esophagectomy: a case report.

BACKGROUND: Mediastinoscopic surgery for esophageal cancer facilitates early postoperative recovery. However, it can occasionally cause serious complications. Here, we present the case of a patient with a tracheal injury diagnosed by a sudden increase in end-tidal carbon dioxide (EtCO2) during mediastinoscopic subtotal esophagectomy. CASE PRESENTATION: A 52-year-old man diagnosed with esophageal cancer was scheduled to undergo mediastinoscopic subtotal esophagectomy. During the mediastinoscopic procedure, the EtCO2 level suddenly increased above 200 mmHg, and the blood pressure dropped below

Bilateral transcervical mediastinoscopic-assisted transhiatal laparoscopic esophagectomy compared with thoracolaparoscopic esophagectomy for esophageal cancer: a propensity score-matched analysis.

BACKGROUND: Transcervical mediastinoscopic esophagectomy for esophageal and esophagogastric junction cancer is indicated in select institutions because of the complex surgical technique required and the unfamiliar surgical view compared with the standard transthoracic esophagectomy approach. This study was performed to compare the feasibility and efficacy of bilateral transcervical mediastinoscopic-assisted transhiatal laparoscopic esophagectomy (BTC-MATLE) with thoracolaparoscopic esophagectomy (TLE) for esophageal cancer. METHODS: This study involved 392 consecutive patients with esophageal

Minimally-invasive complete resection of intrathoracic mediastinal lesions with a Linder-Dahan spreadable-blade video mediastinoscope system: A report of two cases.

INTRODUCTION: Cervical mediastinoscopy can provide a minimally invasive access to the paratracheobronchial mediastinum within its reachable range, but its operability is substantially limited because of its small operative field, poor visualisation, and one-handed operation. PRESENTATION OF CASES: Patient 1, a 56-year-old woman, presented with a 22×17mm, non-symptomatic, (18)F-fluorodeoxy glucose (FDG)-avid, solid schwannoma originating from the vagus nerve trunk in the right upper paratracheal space. Patient 2, a 55-year-old man, presented with a 55×41mm cystic mass in the left upper paratra

Short- and Long-term Outcomes of Mediastinoscopic Radical Esophagectomy Compared with Open Esophagectomy for Esophageal Cancer: A Consecutive Single-Institution Cohort Study.

BACKGROUND: Mediastinoscopic esophagectomy has emerged as a non-transthoracic minimally invasive approach, but concerns remain regarding its oncologic adequacy and long-term outcomes. We evaluated short-term morbidity and long-term survival after mediastinoscopic versus open esophagectomy for esophageal cancer. METHODS: From an institutional esophagectomy database comprising 330 patients, 116 consecutive patients with pathological stage I-III esophageal cancer who underwent curative esophagectomy between 2007 and 2025 in the modern neoadjuvant era were analyzed. Data were collected retrospect

Predictive Factors for Mediastinal Lymph Node Metastases in Patients with Non-small Cell Lung Cancer, as Detected by Video-assisted Mediastinoscopic Lymphadenectomy.

PURPOSE: Accurate mediastinal staging of non-small cell lung cancer is essential for guiding curative treatment strategies, particularly regarding the indication for neoadjuvant therapy. This study evaluated pretherapeutic predictive factors for mediastinal lymph node metastases based on routinely collected clinico-pathological data. PATIENTS AND METHODS: We retrospectively analysed data from all patients who underwent video-assisted mediastinoscopic lymphadenectomy for mediastinal staging before curatively-intended treatment during a 12-year period. Logistic regression and ROC analyses were

[Long‒Term Complete Response after Salvage Radiotherapy with Nivolumab Following Mediastinoscopic Esophagectomy in a High‒Risk Elderly Patient with Advanced Esophageal Cancer].

Neoadjuvant chemotherapy followed by radical esophagectomy is the standard treatment for advanced esophageal cancer in Japanese treatment guideline. However, in elderly patients with dysphagia and impaired organ function, preoperative chemotherapy may worsen their condition. We report here a case of a 79‒year‒old malnourished man with advanced esophageal squamous cell carcinoma and EGJ adenocarcinoma presenting with severe dysphagia. Given his age, frailty, and high‒risk of systemic decline, initial mediastinoscopic esophagectomy was selected over neoadjuvant therapy. Pathology showed a positi

Efficacy comparison of transcervical video-assisted mediastinoscopic lymphadenectomy combined with left transthoracic esophagectomy versus right transthoracic esophagectomy for esophageal cancer treatment.

BACKGROUND: This study aimed to propose a new surgical strategy, i.e., the transcervical video-assisted mediastinoscopic lymphadenectomy (VAMLA) with esophagectomy via the left transthoracic approach for patients with esophageal cancer (EC), and to compare the outcomes with those of esophagectomy via the right thoracic approach. METHODS: From December 2014 to March 2016, 49 cases were enrolled in this non-randomized concurrent control study. Twenty-eight patients with EC who underwent transcervical VAMLA with esophagectomy via the left transthoracic approach were assigned into the study group

Video-assisted mediastinoscopic lymphadenectomy combined with minimally invasive pulmonary resection for left-sided lung cancer: feasibility and clinical impacts on surgical outcomes†.

OBJECTIVES: Although video-assisted mediastinoscopic lymphadenectomy (VAMLA) has greatly increased the accuracy of mediastinal staging, its clinical value as a therapeutic tool for complete mediastinal lymph node dissection in the treatment of left-sided lung cancer is not well elucidated. METHODS: We identified the consecutive 649 patients with left-sided lung cancer undergoing minimally invasive pulmonary resection between July 2002 and June 2013. Among them, 225 patients underwent VAMLA combined with pulmonary resection (VAMLA + VATS group), while the remaining 424 patients underwent VATS

Mediastinal staging with video-assisted mediastinoscopic lymphadenectomy after endobronchial ultrasound-guided transbronchial needle aspiration: real-world evidence in 228 patients.

BACKGROUND: Accurate staging of mediastinal lymph nodes in patients with non-small-cell lung cancer (NSCLC) is essential to determine further management. Current guidelines recommend confirmatory mediastinoscopy for patients with suspicious mediastinal lymph node metastases in image-based staging but negative findings on endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA). In clinical practice, adherence to these recommendations is low and many physicians omit invasive surgical staging. This study aimed to assess the results of subsequent video-assisted mediastinoscopi

Anatomical study of the left superior mediastinal lymphatics for tracheal branches of left recurrent laryngeal nerve-preserving mediastinoscope-assisted surgery in esophageal cancer.

PURPOSE: Curative treatment of esophageal cancer requires meticulous superior mediastinal lymphadenectomy, in addition to esophagectomy, because superior mediastinal lymph node metastases are common in esophageal cancer. When preserving the tracheal branches of the left recurrent laryngeal nerve (RLN), good anatomical understanding is required for confirmation of the positional relationships between the courses of lymphatic vessels, lymph node distribution, and the left RLN and its tracheal branches. We performed a detailed anatomical examination of these relationships. METHODS: Macroscopic a

Detection of disseminated tumour cells in mediastinoscopic lymph node biopsies and endobronchial ultrasonography-guided transbronchial needle aspiration in patients with suspected lung cancer.

PURPOSE: Ultrasound-guided transbronchial needle aspiration of mediastinal lymph nodes (EBUS-TBNA) is apparently more accurate for cancer diagnosis than standard transbronchial needle aspiration (TBNA), but it is less sensitive than mediastinoscopy. The detection of disseminated tumour cells in transbronchial needle aspiration and mediastinoscopic biopsies could improve staging and might be helpful concerning indications for neoadjuvant regimen. The goal of this study was to develop a quantitative method for the detection of disseminated tumour cells (DTCs) in lymph node samples from patients

Portrayal of video-assisted mediastinoscopic lymphadenectomy's range subsequent to its simultaneous use with uniportal VAT-lobectomy for left-sided NSCLC: a case-based perspective.

BACKGROUND: Video-assisted mediastinoscopic lymphadenectomy (VAMLA) is the most precise approach combining staging and therapeutic interventions in non-small cell lung cancer (NSCLC). In the case of left-sided NSCLC, the likelihood of mediastinal lymph node metastases depends on the involvement of the left lung regional lymphatic network. As such, it appears obvious - at least for selected patients with mediastinal staging by either PET-CT or EBUS-TBNA ± EUS-FNA and with cN ≤ 2 - to merge VAMLA and left-sided video-assisted thoracoscopic (VAT) lobectomy for a single-stage therapeutical procedu

A feasibility study of mediastinoscopic radical esophagectomy for thoracic esophageal cancer from the viewpoint of the dissected mediastinal lymph nodes validated with thoracoscopic procedure: a prospective clinical trial.

PURPOSE: A prospective trial evaluated the feasibility and safety of "mediastinoscopic esophagectomy with lymph node dissection" (MELD). METHODS: Eligible patients had thoracic esophageal squamous cell carcinoma, excluding T4, a bulky primary lesion or distant metastasis. Ten patients were enrolled and treated between September 2015 and March 2018. Additionally, to verify the integrity of the mediastinal lymph node dissection, thoracoscopic observation and lymph node dissection were followed. The primary end point was the integrity of mediastinal lymph node dissection. The secondary end point

How I do it: The Hi-UP approach-single-port da Vinci SP-assisted mediastinoscopic subtotal esophagectomy with a transhiatal-dominant strategy in the abdominal and cervical phases.

Mediastinoscopic esophagectomy for thoracic esophageal cancer may reduce pulmonary morbidity; however, upper mediastinal lymphadenectomy, particularly along the left recurrent laryngeal nerve (station 106recL), remains technically challenging. We developed a transhiatal-dominant mediastinoscopic subtotal esophagectomy using the da Vinci SP system for both the abdominal and cervical phases. After completing lower and mid-mediastinal lymphadenectomy while preserving the mediastinal pleura, transhiatal dissection is advanced beyond the carina into the upper mediastinum to perform 106recL lymphade

Transient unilateral hypoperfusion of the lung following mediastinoscopy.

Two cases of pulmonary hypoperfusion occurring after mediastinoscopic examination were demonstrated on lung scans. In one case, this finding on the ninth day required a pulmonary angiographic study that yielded normal findings. Repeat lung scans were normal. We propose that localized mediastinal edema or bleeding after mediastinoscopic examination can produce defects of hypoperfusion, and we urge caution in the interpretation of lung scans up to nine days after mediastinoscopic examination.

Mediastinoscopy in the treatment of mediastinal cysts.

OBJECTIVE: Primary cysts constitute 25% of all masses in the mediastinum. Because radiological investigations are often inconclusive, many adults require mediastinoscopy, thoracotomy, video-assisted thoracic surgery, or computed tomography-guided transbronchial, transesophageal, or transcutaneous aspiration to confirm the cystic nature of these lesions. Minimally invasive procedures fail when the cyst contents are gelatinous and mucoid (failure to aspirate) or when the cyst wall continues to secrete fluid. Though Pursel reported mediastinoscopic extirpation of benign cysts 35 years ago, it rem
