Medicaid Expansion and Payday Lending: Evidence from Louisiana.

Medicaid eligibility expansions improve financial well-being, but their effects on non-traditional credit use remain understudied. We examined changes in payday loan use among individuals gaining coverage through Louisiana's 2016 Medicaid expansion, compared with individuals in high-uninsurance ZIP Codes in Southern non-expansion states. Gaining Medicaid coverage was associated with lower payday loan origination amounts and a sharp decline in the share of loans sent to collection agencies. However, we found no effect on the number of loans per borrower, or the share of loans charged off. Our f

Medicaid and health resilience: Evidence from TennCare disenrollment.

Does public health insurance mitigate the functional consequences of adverse health shocks? I investigate this question by exploiting variation induced by Tennessee's 2005 Medicaid disenrollment, which abruptly terminated coverage for approximately 170,000 childless adults. Using Behavioral Risk Factor Surveillance System (BRFSS) data from 1997 to 2010 and a difference-in-differences design, I estimate Intent-to-Treat effects. I first document that the reform significantly increased the uninsured rate and increased the share of Tennessean childless adults reporting "poor" health status. I also

First-trimester preexisting diabetes screening in Medicaid beneficiaries.

INTRODUCTION: Patients with public insurance have higher rates of pregestational diabetes mellitus (DM) and are less likely to receive preconception care than those with private insurance. First trimester DM screening rates among high-risk publicly insured pregnant individuals in the United States are unknown. This study assessed first-trimester DM screening rates in this population. We hypothesized that less than half of publicly insured pregnant patients with at least one DM risk factor were screened in the first trimester. We also examined the utilization of different DM screening tests and

Stimulant Overdose Prediction Model for Medicaid-Insured Persons.

IMPORTANCE: Overdoses involving methamphetamines and cocaine have increased in recent years. Identification of individuals at highest risk could facilitate the implementation of evidence-based interventions to reduce overdose risk. OBJECTIVE: To develop and internally validate a model that predicts hospitalization or emergency department (ED) treatment for stimulant-involved overdose among the Medicaid-insured population. DESIGN, SETTING, AND PARTICIPANTS: This was a retrospective case-cohort study using Medicaid claims data from 2016 to 2019 (development) and 2020 (validation) for all Medic

Medicaid Spending on Antiretrovirals From 2007 Through 2019.

BACKGROUND: Antiretroviral (ARV) medications to treat human immunodeficiency virus (HIV) are a major contributor to Medicaid prescription drug spending. Despite having been used for over 3 decades, the first generic ARVs only recently became available, and many newer versions continue to be sold at high prices despite within-class competition. We estimated Medicaid spending on ARVs from 2007 through 2019. METHODS: Using public Medicaid State Drug Utilization data, we identified trends in ARV spending and use from 2007 through 2019. We estimated net spending and average prices (spending per 30

Hepatitis C Treatment Initiation Among US Medicaid Enrollees.

IMPORTANCE: Direct-acting antiviral (DAA) treatment for hepatitis C virus (HCV) infection is highly effective but remains underused. Understanding disparities in the delivery of DAAs is important for HCV elimination planning and designing interventions to promote equitable treatment. OBJECTIVE: To examine variations in the receipt of DAA in the 6 months following a new HCV diagnosis. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used national Medicaid claims from 2017 to 2019 from 50 states, Washington DC, and Puerto Rico. Individuals aged 18 to 64 years with a new diagn

Affordable Care Act Medicaid expansions and maternal morbidity.

In this paper, we test whether the Affordable Care Act Medicaid expansions are associated with maternal morbidity. The ACA expansions may have affected maternal morbidity by increasing pre-conception access to health care, and by improving the quality of delivery care, through enhancing hospitals' financial positions. We use difference-in-difference models in conjunction with event studies. Data come from individual-level birth certificates and state-level hospital discharge data. The results show little evidence that the expansions are associated with overall maternal morbidity or indicators

Drive Times to Methadone Treatment among Medicaid Patients.

OBJECTIVE: To describe round-trip drive times and travel distance to methadone clinics among Medicaid enrollees in West Virginia, testing for differences between those in rural versus urban areas. FINDINGS: In this cross-sectional analysis of West Virginia Medicaid enrollees' claims from 2018-2019, methadone recipients on average traveled almost an hour round-trip to receive their daily treatment. The travel burden was substantially higher among those in more rural areas. Meaning. These findings indicate that it may be difficult for patients to adhere to their methadone treatment regimen, per

Reducing Medicaid Coverage Gaps for Youth During Reentry.

Although many justice-involved youth (JIY) rely on Medicaid, due to the federal "inmate exclusion" Medicaid is often suspended or terminated upon youth's intake to detention, which can lead to coverage gaps at release. We interviewed 28 experts on Medicaid and the justice system and conducted thematic analysis to identify solutions for reducing Medicaid coverage gaps during reentry. Participants viewed coverage gaps during reentry as a significant public health problem to which JIY are especially vulnerable. Recommended solutions for reducing coverage gaps for JIY included (a) leave Medicaid a

Patterns of cervical cancer screening among Medicaid beneficiaries.

OBJECTIVE: Cervical cancer screening guidelines have evolved over time with the incorporation of human papillomavirus (HPV) testing along with cytology. Current screening guidelines recommend cytological screening every 3 years or HPV testing with or without cytology every 5 years in women age 30-65 years. We examined the use of cervical cancer screening among average-risk Medicaid beneficiaries. DESIGN: Retrospective cohort study. POPULATION: Women age 30-64 years at average risk for cervical cancer who underwent cervical cancer screening with cytology, co-testing or primary HPV testing fro

Changes in Preventative Health Care After Medicaid Expansion.

BACKGROUND: Medicaid expansion substantially increased health insurance coverage, but its effect on the delivery of preventative health care is unclear. OBJECTIVE: The objective of this study was to assess the impact of Medicaid expansion on the receipt of 15 different measures of preventive care including cancer screening, cardiovascular risk reduction, diabetes care, and other primary care measures. RESEARCH DESIGN: We performed serial cross-sectional analysis of Behavioral Risk Factor Surveillance System (BRFSS) survey data from 2012 to 2017. We used a quasi-experimental design with diffe

Medicaid Coverage of Sexually Transmitted Disease Service Visits.

INTRODUCTION: Chlamydia and gonorrhea are the most commonly reported notifiable infections in the U.S., with direct medical costs for the treatment of these infections exceeding $700 million annually. Medicaid currently covers approximately 80 million low-income Americans, including a high percentage of racial and ethnic minorities. Studies have shown that racial and ethnic minority populations, particularly those with low SES, are at an increased risk of acquiring a sexually transmitted disease. Therefore, as Medicaid expands, there will likely be a greater demand for sexually transmitted dis

Health Reform, Medicaid Expansions, and Women's Cancer Screening.

BACKGROUND: Health reform, including Medicaid expansion, is increasing insurance coverage and financial access to breast and cervical cancer screening for low-income women, although services for low-income uninsured women are still needed. METHODS: American Community Survey and administrative data about Medicaid and health insurance enrollment are used to estimate the number of low-income women who will be uninsured in 2017, focusing on the age ranges 21 to 64, 40 to 64, and 50 to 64. RESULTS: Assuming that 29 states expand Medicaid (as of June 2015), the national percentage of low-income wo

Provider-delivered tobacco dependence treatment to Medicaid smokers.

INTRODUCTION: Smoking prevalence is 49% among Medicaid enrollees in Ohio. The objective of this pilot project was to test a comprehensive tobacco dependence treatment program targeting rural Medicaid-enrolled smokers for both physician-level and smoker-level outcomes. METHODS: Using a group-randomized trial design, intervention group physicians (n = 4) were exposed to systems-level changes in their clinics, and smokers in these clinics were offered 12 weeks of telephone cessation counseling. Control group physicians (n = 4) were given the clinician's version of the U.S. Public Health Serivce

Experiences of Mothers Following the Medicaid Postpartum Extension.

IMPORTANCE: The American Rescue Plan Act of 2021 extended postpartum coverage for women eligible for Medicaid for Pregnant Women from 60 days to 12 months. Understanding how mothers experienced the postpartum extension is vital in evaluating this policy change and informing policymakers on how policies aimed at one family member can improve outcomes for the family unit. OBJECTIVE: To describe the experiences of postpartum mothers following the Medicaid postpartum coverage extension in North Carolina from 60 days to 12 months. Structural determinants of Medicaid enrollment in late life.

OBJECTIVES: The objective of the study was to examine whether two state-level measures of policy context and racialized public opinion are differentially associated with Medicaid enrollment among adults aged 50 and above across social locations. METHODS: Multilevel logistic regression models were estimated to assess structural and individual factors associated with Medicaid enrollment among adults aged 50 or above using data from 10 waves of the Health and Retirement Study in combination with a Medicaid generosity index (MGI) and a racial resentment (RR) score. Age- and race-stratified models

Ensuring Integrity of the Medicaid Drug Rebate Program.

The Medicaid Drug Rebate Program (MDRP) is a federal-state partnership program that aims to control prescription drug costs through contractual agreements enabling manufacturers to secure Medicaid coverage for their drugs by paying rebates to state Medicaid agencies. All states participate in MDRP, helping Medicaid beneficiaries obtain affordable access to nearly all Food and Drug Administration-approved outpatient drugs. Although the program has achieved significant cost savings for Medicaid, notable False Claims Act violations in recent years have called into question the program's operation
