Impact of using dosing criteria to refine pharmacogenomic clinical decision support for tricyclic antidepressants.

PURPOSE: To evaluate the impact of dose-specific logic for tricyclic antidepressant (TCA) pharmacogenomics (PGx) clinical decision support (CDS). We aimed to provide guidance in an area with limited supporting literature, ensure optimal dosing through CDS, and limit alert fatigue. The primary outcome was the reduction in alerts prescribers encountered, while the secondary outcome included an analysis across specialties. METHODS: A retrospective chart review was conducted to examine TCA PGx CDS before and after implementation of dosing criteria for alerts. Data were abstracted from the electro

[Telemonitoring in continuous positive airway pressure-treated patients with obstructive sleep apnoea syndrome: An algorithm proposal].

Most of the continuous positive airway pressure (CPAP) devices currently in use allow telemonitoring of observance, leaks and the apnoea-hypopnoea index (AHI). La Société française de recherche et de médecine du sommeil (SFRMS) and La Société de pneumologie de langue française (SPLF) workgroup offer to CPAP prescribers and to home care providers a scientific document which has the following purposes: to underline the relevance of the telemonitoring of leaks and the AHI, to define alert thresholds, to describe the principal mechanisms generating excessive leaks and high AHI, and to propose a di

Effect of an electronic ordering system on adherence to the American College of Cardiology/American Heart Association guidelines for cardiac monitoring.

INTRODUCTION: Telemetry monitoring is often overused in the inpatient setting. This has led to overcrowding of telemetry beds, increased wait times in the emergency department, and inefficient allocation of hospital resources. The American College of Cardiology/American Heart Association (ACC/AHA) guidelines exist to guide appropriate utilization of cardiac monitoring. We sought to investigate the effect of the institution of an electronic ordering system (EOS) on adherence to guideline-based telemetry use. METHODS: Telemetry bed utilization was followed prospectively before and after institu

Effect of a patient-specific QTc alert on decreasing prescribing of QTc-prolonging medications in outpatients.

PURPOSE: Previous studies have shown decreased exposure to QTc-prolonging medications through utilization of a clinical decision support (CDS) alert in the electronic health record (EHR) in hospitalized patients. Minimization of risk factors, including exposure to QTc-prolonging medications, is important to decrease the risk of adverse outcomes, including ventricular arrhythmias. This study evaluated the effect of a patient-specific prolonged QTc EHR alert in nonhospitalized patients. METHODS: This was a single-center, retrospective, quasi-experimental pre-post analysis of an interruptive ale

Consecutive national surveys of ABO-incompatible blood transfusion in Japan.

BACKGROUND AND OBJECTIVES: Morbidity and mortality from ABO-incompatible transfusion persist as consequences of human error. Even so, insufficient attention has been given to improving transfusion safety within the hospital. MATERIALS AND METHODS: National surveys of ABO-incompatible blood transfusions were conducted by the Japanese Society of Blood Transfusion, with support from the Ministry of Health, Labor and Welfare. Surveys concluded in 2000 and 2005 analysed ABO-incompatible transfusion data from the previous 5 years (January 1995 to December 1999 and January 2000 to December 2004, res

The effect of a computerized best practice alert system in an outpatient setting on metabolic monitoring in patients on second-generation antipsychotics.

WHAT IS KNOWN AND OBJECTIVE: Metabolic syndrome is a well-documented adverse effect of second-generation antipsychotics (SGAs). Patients with metabolic syndrome are at an increased risk of potentially fatal cardiovascular events, including myocardial infarction and stroke. This elevated risk prompted the creation of a national guideline on metabolic monitoring for patients on SGAs in 2004. However, monitoring practices remained low at our clinic. To address this concern, a clinical decision support system was developed to alert providers of monitoring requirements. The purpose of this study is

Is health information technology associated with patient safety in the United States?

(1) Estimates using national data provide tentative evidence of a positive relationship between health information technology (HIT) and clinical quality. Electronic medical records (EMRs) are associated with a statistically significant aversion of two post-operative infections per year at the average U.S. acute care hospital. (2) The study found no significant relationships for two other types of HIT - nurse charting and picture archiving communication systems (PACS) - or for two other measures of patient safety - post-operative hemorrhages/hematomas and post-operative pulmonary embolism/deep

Standardization of intrapartum management and impact on adverse outcomes.

Patient safety is currently a critical focus within obstetrics. Standardization of intrapartum management, in particular with regard to fetal heart rate monitoring assessment and management, is an important effort to improve obstetrical adverse outcomes. This study explores various strategies to implement standardization of assessment and management. Such measures can impact significantly perinatal outcomes and safety culture. Investigation of the impact of standardizing intrapartum management remains an important area of further study.

Impact of clinical decision support preventing the use of QT-prolonging medications for patients at risk for torsade de pointes.

We developed and implemented a 'CPOE-QT Alert' system, that is, clinical decision support integrated in the computerized physician order entry system (CPOE), in 2011. The system identifies any attempts to order medications with risk of torsade de pointes (TdP) for patients with a history of significant QT prolongation (QTc ≥500 ms) and alerts the provider entering the order. We assessed its impact by comparing orders and subsequent medication administration before and after activation of the system. We found a significant decrease in the proportion of completed order per ordering attempt after

Antiemetic prescribing practices using a computerized physician order entry system.

PURPOSE: Adherence to guideline-consistent chemotherapy-induced nausea and vomiting (CINV) prophylaxis is suboptimal. The primary aim of this study was to evaluate the magnitude of compliance to institutional guideline-directed antiemetic prophylaxis using a computerized physician order entry system at a single tertiary care institution. A nurse survey was also performed to evaluate how oncology practices, within a cooperative group, managed clinician orders for the prevention of CINV. METHODS: The electronic medical records of 100 consecutive patients were evaluated. The primary endpoint was
