Comparing Receipt of Prescribed Post-acute Home Health Care Between Medicare Advantage and Traditional Medicare Beneficiaries: an Observational Study.

BACKGROUND: Medicare Advantage (MA) covers more than 1/3rd of all Medicare beneficiaries. MA plans are required to provide the same benefits as Traditional Medicare (TM), but can impose utilization management tools to control costs. OBJECTIVE: To assess differences between TM and MA enrollees in the probability of receiving prescribed post-acute home health (HH) care and to describe MA plan characteristics associated with HH receipt. DESIGN: Retrospective cross-sectional analysis of claims data, HH patient assessment data, and MA plan data from 2011 to 2017. PARTICIPANTS: Medicare beneficia

Home-Based Medical Care in Traditional Medicare vs. Medicare Advantage Among Older Adults With Alzheimer's Disease and Related Dementias.

BACKGROUND: As Alzheimer's disease and related dementias (ADRD) progress, many people become homebound and depend on home-based medical care (HBMC), but differences in HBMC, telemedicine, and office visits between Traditional Medicare (TM) and Medicare Advantage (MA) plans are not well understood. METHODS: We analyzed 2021-2022 Medicare claims and encounter data to categorize 2022 outpatient visits for community-living TM and MA enrollees with ADRD as HBMC, telemedicine, or office-based. We summarized HBMC recipient characteristics by plan type and used Poisson regression with inverse weights

Impact of the 2023 Centers for Medicare & Medicaid Services policy change on carotid artery stenting use among Medicare beneficiaries.

OBJECTIVE: On 11 October 2023, the Centers for Medicare & Medicaid Services expanded the indications for carotid artery stenting to include standard risk patients with symptomatic ≥50% and asymptomatic ≥70% carotid artery stenosis, providing surgeons an option between carotid endarterectomy and carotid artery stenting. This decision followed arguments both for and against the change by various professional societies, including opposition by the Society for Vascular Surgery. We sought to examine the impact of the change in policy with use of carotid artery stenting versus carotid endarterectomy

Use of real-world evidence in the Medicare Drug Price Negotiation Program: A checklist for the Centers for Medicare and Medicaid Services and manufacturers.

Under the Inflation Reduction Act's (IRA's) - Medicare Drug Price Negotiation Program, the Centers for Medicare & Medicaid Services' (CMS's) "maximum fair price" must be informed by evidence on factors such as therapeutic advance of the selected drug compared with its alternative, comparative effectiveness across clinical and patient-reported outcomes, the impact on specific populations, and the ability to address unmet medical needs. This paper describes how real-world evidence could improve CMS decision-making and creates a best practices checklist to help CMS evaluate the quality of any man

Same-Day Sinus and Brain CT Imaging in the Medicare Population: Are Practice Patterns Changing in Association with Medicare Policy Initiatives?

BACKGROUND AND PURPOSE: Monitoring the frequency of same-day sinus and brain CT (Outpatient Measure 14, "OP-14") is part of a recent large Centers for Medicare and Medicaid Services hospital outpatient quality initiative to improve imaging efficiency. This study investigates patient-level claims data in the Medicare population focusing on where same-day sinus and brain CT imaging is performed and how the frequency of same-day studies changed with time before and during OP-14 measure program implementation. MATERIALS AND METHODS: Research Identifiable Files were used to identify all sinus and

Association of Medicare Part D Benzodiazepine Coverage Expansion With Changes in Fall-Related Injuries and Overdoses Among Medicare Advantage Beneficiaries.

IMPORTANCE: Benzodiazepines, which are associated with safety-related harms for older adults, were not covered when the US Medicare Part D prescription drug benefit began. Coverage was extended to benzodiazepines in 2013. OBJECTIVE: To examine whether the expansion of benzodiazepine coverage among Medicare Advantage (MA) beneficiaries was associated with increases in fall-related injuries or overdoses among older adults. DESIGN, SETTING, AND PARTICIPANTS: This ecological study used interrupted time-series with comparison-series analyses of MA claims data from 4 635 312 age-eligible MA benefi

Transcatheter Aortic Valve Replacement (TAVR) in Medicare Advantage and Medicare Fee-for-Service: Differences in Patient Characteristics, Utilization, and Outcomes.

BACKGROUND: Over half of Medicare beneficiaries are enrolled in privately administered Medicare Advantage (MA) plans, and aortic stenosis predominantly affects older patients. In this study, we evaluated whether differences in care delivery between MA and fee-for-service (FFS) Medicare contribute to cardiovascular outcomes after transcatheter aortic valve replacement (TAVR). METHODS: Medicare patients who underwent TAVR from January 1, 2018, to December 31, 2022, were eligible for cohort inclusion. TAVR utilization rates in MA and FFS were calculated as the age- and sex-standardized number of

Total Hip Arthroplasty and the Medicare Inpatient-Only List: An Analysis of Complications in Medicare-Aged Patients Undergoing Outpatient Surgery.

BACKGROUND: The Centers for Medicare and Medicaid Services has solicited public comments for the 2019 Proposed Rule to remove total hip arthroplasty (THA) from the inpatient-only list. Concerns exist regarding the safety of discharging higher risk Medicare patients as an outpatient and whether hospitals may still be reimbursed for an inpatient procedure. The purpose of this study is to determine whether Medicare-aged patients undergoing outpatient THA have higher complication rates than patients who underwent inpatient THA. We also sought to identify characteristics of Medicare-aged patients t

Medicare program; payment for kidneys sent to foreign countries or transplanted in patients other than Medicare beneficiaries--HCFA. Final rule.

These final regulations exclude from Medicare payments made to organ procurement organizations the costs associated with kidneys sent to foreign countries or transplanted in patients other than Medicare beneficiaries. In addition to reducing Medicare expenditures by eliminating Medicare subsidization of the costs of kidneys sent to foreign countries or transplanted in patients other than Medicare beneficiaries, we intend these regulations to increase the availability of kidneys to Medicare beneficiaries who are suitable transplant candidates. This could result in medical and social benefits fo

Geographic variation in access to Medicare-subsidised psychotherapy.

OBJECTIVE: Within Australia, evidence is limited regarding how access to mental health care varies across geographical regions, and which areas experience the lowest levels of service use. This study aimed to provide new population-level evidence on the geographic variation in the use of Medicare-subsidised psychotherapy over the past decade and identify regions with the lowest service use. METHODS: We used administrative data from the Medicare Benefits Schedule to identify 2.7 million adults aged 25-54 years across mainland Australian states with a new mental health treatment plan from a gen

Frailty in prodromal Parkinson's disease in Medicare beneficiaries.

INTRODUCTION: Frailty, a common syndrome of aging associated with a decline in physiological reserve, places individuals with Parkinson's disease (PD) at a greater risk of poor health outcomes. We hypothesized that frailty would also be greater in prodromal PD patients compared to the general population and, thus, could be part of the prodromal phase of PD. METHODS: We conducted a population-based case-control study of incident PD cases (N = 82,560) and controls (N = 96,482) ages 70-90 identified in 2009. Using 2004-2009 Medicare data, we examined frailty in five individual 12-month time wind

Medicare billing and utilization trends in vascular surgery.

OBJECTIVE: Within the past decade, Medicare Part B reimbursements for various surgical procedures have been declining, whereas health care expenses continue to increase. As a result, hospitals may increase service charges to offset losses in revenue, which may disproportionately affect underinsured patients. Our analysis aimed to characterize Medicare billing and utilization trends across common vascular surgical procedures. METHODS: The 2017 to 2021 Medicare Physician and Other Practitioners by Provider and Service dataset was queried for Current Procedural Terminology (CPT) codes for common

Trends in Medicare Payments for Beneficiaries With Aortic Stenosis.

Background Aortic stenosis (AS) is the most common form of valvular heart disease with an increasing prevalence. Management of AS has changed dramatically with the introduction of transcatheter aortic valve replacement (AVR). The shift in management of AS, combined with an aging population, may increase the cost of patients with AS in the US health care system. Methods and Results We performed a retrospective cohort study, using inpatient, carrier, and outpatient data from a 20% Medicare fee-for-service beneficiaries' sample from 2008 to 2019 and included beneficiaries, aged ≥65 years. We iden

The introduction of generic risperidone in Medicare Part D.

OBJECTIVES: The introduction of generic second-generation antipsychotics (SGAs), starting with risperidone in July 2008, could reduce antipsychotic spending and cost-related use barriers. This study examines associations between generic risperidone use and spending and adherence after introduction among Medicare Advantage (MA) beneficiaries. STUDY DESIGN: Historic cohort study. METHODS: The study included MA beneficiaries receiving SGA treatment prior to July 2008. We examined antipsychotic spending using linear models, adherence (proportion of days covered ≥ 80%) using logistic models, and
