Outpatient chemotherapy drug costs and expensive chemotherapy drug use in 340B and Non-340B hospitals: an observational study.

BACKGROUND: The 340B Drug Pricing Program has been controversial since its inception in 1992, a major criticism being that 340B hospitals use more outpatient drugs, and more expensive drugs, because of financial incentives to "make money" through the program. The goal of this study was to determine whether characteristics of patients treated at 340B hospitals, and affiliation of hospitals with NCI-designated cancer centers, would explain higher Part B drug costs and use of more expensive chemotherapy drugs. METHODS: This is an observational study using data from SEER-Medicare and 340B entity

Recent Trends in Imaging Use in Hospital Settings: Implications for Future Planning.

PURPOSE: To compare trends in utilization rates of imaging in the three hospital-based settings where imaging is conducted. METHODS: The nationwide Medicare Part B databases for 2004-2014 were used. All discretionary noninvasive diagnostic imaging (NDI) CPT codes were selected and grouped by modality. Procedure volumes of each code were available from the databases and converted to utilization rates per 1,000 Medicare enrollees. Medicare's place-of-service codes were used to identify imaging examinations done in hospital inpatients, hospital outpatient departments (HOPDs), and emergency depar

Increased Medicare hospital spending is associated with reduced acute aortic dissection mortality in the United States.

BACKGROUND: In recent decades, national acute aortic dissection (AAD) mortality rates have decreased, whereas Medicare expenditures have risen, raising questions about the value of care. We investigated the longitudinal association between Medicare expenditures (both hospital and physician) and age-adjusted AAD mortality at a population level, accounting for geographic variation. METHODS: We conducted a retrospective, longitudinal ecological study using publicly available, aggregate data from 1999 to 2020. The primary outcome was the annual state-level, age-adjusted AAD mortality rate from th

Diabetes white paper: Defining the delivery of nutrition services in Medicare medical nutrition therapy vs Medicare diabetes self-management training programs.

Registered dietitians (RDs) have a defined and unique role in care for patients with diabetes that differs depending on whether the service is for medical nutrition therapy (MNT) or part of a diabetes self-management training (DSMT) program (DSMT and diabetes self-management education [DSME] are used interchangeably in this article). The purpose of this article is to describe the current regulatory and practice framework that supports nutrition care under Medicare Part B for people with diabetes. A description of MNT and DSMT provided under Medicare Part B is included. The role of RDs and othe

Utilization Patterns and Costs of Ocular Amniotic Membrane Grafts in the Medicare Population.

PURPOSE: Recent investigations from the Office of the Inspector General and The New York Times have raised concerns about the costs to Medicare of skin substitutes, documenting pervasive fraudulent billing practices outside of ophthalmology. These investigations have not covered the use of tissue in eye care, despite similar reimbursement incentives for the use of sutureless ocular amniotic membrane grafts (AMGs). We examine the volume, cost, and clinical indications associated with sutureless AMGs in eye care. DESIGN: Retrospective cohort study of fee-for-service Medicare patients who receiv

National trends in lower extremity bypass surgery, endovascular interventions, and major amputations.

INTRODUCTION: Advances in endovascular interventions have expanded the options available for the invasive treatment of lower extremity peripheral arterial disease (PAD). Whether endovascular interventions substitute for conventional bypass surgery or are simply additive has not been investigated, and their effect on amputation rates is unknown. METHODS: We sought to analyze trends in lower extremity endovascular interventions (angioplasty and atherectomy), lower extremity bypass surgery, and major amputation (above and below-knee) in Medicare beneficiaries between 1996 and 2006. We used 100%
