Variable Expressivity of HNF1B Nephropathy, From Renal Cysts and Diabetes to Medullary Sponge Kidney Through Tubulo-interstitial Kidney Disease.

INTRODUCTION: In humans, heterozygous mutations of hepatocyte nuclear factor 1beta (HNF1B) are responsible for a dominant inherited disease with both renal and extrarenal phenotypes. HNF1B nephropathy is the umbrella term that includes the various kidney phenotypes of the disease, ranging from congenital anomalies of the kidney and urinary tract (CAKUT), to tubular transport abnormalities, to chronic tubulointerstitial and cystic renal disease. METHODS: We describe 7 families containing 13 patients with ascertained HNF1B nephropathy. All patients underwent genetic testing and clinical, labora

Coexistence of medullary sponge kidney and ulcerative colitis in the same patient: long-term follow-up.

We describe a female patient with ulcerative colitis since the age of 17, who was accidentally diagnosed as having medullary sponge kidney 3 years after the establishment of diagnosis of inflammatory bowel disease. The diagnosis of renal disease was based on the typical appearance of both kidneys on abdominal ultrasound examination and on IV pyelography findings. All other well-known causes of medullary sponge kidney were excluded on the basis of the relevant laboratory investigation. So far, the patient experienced only one episode of urinary infection but no renal colic. Since the time of di

Beckwith-Wiedemann syndrome and medullary sponge kidney: a case report of two rare but related entities.

BACKGROUND: Beckwith-Wiedemann syndrome (BWS) is a congenital overgrowth disorder that may be associated with renal abnormalities, although benign renal manifestations presenting in adulthood are less well characterized. CASE DESCRIPTION: We report the case of a 20-year-old woman with genetically confirmed BWS who was diagnosed with medullary sponge kidney (MSK) during adulthood after imaging evaluation for suspected renal abnormalities. Contrast-enhanced computed tomography and intravenous urography demonstrated characteristic medullary collecting duct dilatation with nephrocalcinosis, consi

A TRIM8 Variant in a Child: Neuro-Renal Syndrome Causing Features Suggestive of Medullary Sponge Kidney.

Medullary sponge kidney (MSK) is a rare congenital renal anomaly characterised by cystic dilatation of the collecting ducts, often asymptomatic but occasionally presenting with hematuria, nephrolithiasis, or urinary tract infections. While familial cases exist, its precise genetic basis remains unclear. The TRIM8 gene encodes an E3 ubiquitin ligase involved in regulating cellular proliferation, immune response, and differentiation. Pathogenic TRIM8 variants have predominantly been linked to neurodevelopmental disorders and steroid-resistant nephrotic syndrome (SRNS), with or without neurologic

Medullary sponge kidney and Caroli's disease in a patient with stricture urethra: look for the hidden in presence of the apparent.

Caroli's disease is a rare congenital disorder with incidence rate of approximately 1 in 1 000 000 population. Renal anomalies which may be associated with Caroli's disease include medullary sponge kidney (MSK), cortical cysts, adult recessive polycystic kidney disease and rarely autosomal dominant polycystic kidney disease. Exact incidence of MSK in patients of Caroli's disease is not known. There are only a handful of reported cases of this association in literature. We hereby report a case of Caroli's disease with MSK with nephrocalcinosis. He presented to primary health centre with symptom

Retrograde intrarenal surgery for stones associated with renal anomalies: caliceal diverticulum, horseshoe kidney, medullary sponge kidney, megacalycosis, pelvic kidney, uretero-pelvic junction obstruction.

PURPOSE OF REVIEW: Nowadays, due to the increase of imaging diagnosis, we identify easily renal anomalies, and we can choose between a wide range of armamentarium to treat symptomatic stones in those challenging cases. However, there is a lack of evidence and consensus on its use. The aim of this narrative review is to collect all the available data about safety and efficacity of retrograde intrarenal surgery (RIRS) in the treatment of kidney stones associated to a renal anomaly. RECENT FINDINGS: Renal anomalies are uncommon findings and even more if it has to be associated with renal stones.

Accurate Assessment of Interval Change in Stone Burden Among Patients with Medullary Sponge Kidney: A Volumetric Approach.

Introduction: Medullary sponge kidney (MSK) and nephrocalcinosis (NC) are radiographically characterized by the formation of innumerable intrarenal concretions. Accurate determination of disease progression is challenging given the large and typically sporadic calcific burden, limiting the utility of traditional manual linear measurements on non-contrast computerized tomography (NCCT). Progression of stone size or quantity is an important predictor of morbidity in recurrent stone formers. As such, among these patients, we sought to test the accuracy of serial NCCT reports indicating a "stable/

Biopsy proven medullary sponge kidney: clinical findings, histopathology, and role of osteogenesis in stone and plaque formation.

Medullary sponge kidney (MSK) is associated with recurrent stone formation, but the clinical phenotype is unclear because patients with other disorders may be incorrectly labeled MSK. We studied 12 patients with histologic findings pathognomonic of MSK. All patients had an endoscopically recognizable pattern of papillary malformation, which may be segmental or diffuse. Affected papillae are enlarged and billowy, due to markedly enlarged inner medullary collecting ducts (IMCD), which contain small, mobile ductal stones. Patients had frequent dilation of Bellini ducts, with occasional mineral pl

Medullary sponge kidney associated with primary distal renal tubular acidosis and mutations of the H+-ATPase genes.

BACKGROUND: Medullary sponge kidney (MSK) is a rare congenital disease characterized by diffuse ectasia or dilation of precalyceal collecting tubules. Although its pathogenesis is unknown, the association with various congenital diseases suggests that it could be a developmental disorder. In addition to the typical clinical features of nephrocalcinosis and urolithiasis, patients with MSK show tubular function defects of acidification and concentration. These are considered to be secondary to morphological changes of collecting tubules. Primary distal renal tubular acidosis (dRTA) is a rare gen

Spontaneous calcification process in primary renal cells from a medullary sponge kidney patient harbouring a GDNF mutation.

Medullary nephrocalcinosis is a hallmark of medullary sponge kidney (MSK). We had the opportunity to study a spontaneous calcification process in vitro by utilizing the renal cells of a patient with MSK who was heterozygous for the c.-27 + 18G>A variant in the GDNF gene encoding glial cell-derived neurotrophic factor. The cells were obtained by collagenase digestion of papillary tissues from the MSK patient and from two patients who had no MSK or nephrocalcinosis. These cells were typed by immunocytochemistry, and the presence of mineral deposits was studied using von Kossa staining, scanning

Proteomic Analysis of Urinary Extracellular Vesicles Reveals a Role for the Complement System in Medullary Sponge Kidney Disease.

Medullary sponge kidney (MSK) disease is a rare and neglected kidney condition often associated with nephrocalcinosis/nephrolithiasis and cystic anomalies in the precalyceal ducts. Little is known about the pathogenesis of this disease, so we addressed the knowledge gap using a proteomics approach. The protein content of microvesicles/exosomes isolated from urine of 15 MSK and 15 idiopathic calcium nephrolithiasis (ICN) patients was investigated by mass spectrometry, followed by weighted gene coexpression network analysis, support vector machine (SVM) learning, and partial least squares discri

Proteomics Insights into Medullary Sponge Kidney Disease: Review of the Recent Results of an Italian Research Collaborative Network.

BACKGROUND: Medullary sponge kidney (MSK) disease is a rare and neglected congenital condition typically associated with nephrocalcinosis/nephrolithiasis, urinary concentration defects, and cystic anomalies in the precalyceal ducts that, although sporadic in the general population, is relatively frequent in renal stone formers. The physiopathologic mechanism associated with this disease is not fully understood, and omics technologies may help address this gap. SUMMARY: The aim of this review was to provide an overview of the current state of the application of proteomics in the study of this

Congenital dilatation of intrahepatic bile ducts associated with hepatic fibrosis and medullary sponge kidney--reconstruction and microdissection study.

An autopsy case of a four-year-old female with congenital dilatation of intrahepatic bile ducts with bilateral medullary sponge kidney is presented. The liver and kidney were investigated by reconstruction with serial sections and the kidney was also studied by microdissection. In this case the dilatations of collecting tubules and intra- and extrahepatic bile ducts were observed. We regard these lesions as developmental anomalies since these sites of both organs are excretory ducts derived from the endoderm. The liver cirrhosis observed at autopsy had developed in the course of the disease be

The impact of modern endourological techniques in the treatment of a century old disease--medullary sponge kidney with associated nephrolithiasis.

The remarkable progresses of imagistic and interventional techniques that have been implemented during the last decades facilitated the diagnostic and allowed the treatment indication changes for numerous renal disorders. The purpose of the present lecture was to outline a data review concerning a renal anomaly first described one century ago as well as to evaluate the impact of endourologic technical progresses over the therapeutic management of the respective disease. The medullary sponge kidney (MSK) or Cacchi-Ricci disorder represents a disturbance in the renal development characterized by

A review of the natural progression of medullary sponge kidney and a novel grading system based on intravenous urography findings.

PURPOSE: Medullary sponge kidney (MSK) is a developmental abnormality of the medullary pyramids of the kidney, characterised by cystic dilatations of the collecting ducts. We investigated a cohort of patients with MSK to gain further information about its presentation, clinical course and treatment required. We devised a grading system based upon findings at diagnostic intravenous urography (IVU) and established a relationship between severity of IVU findings and severity of disease. MATERIALS AND METHODS: The clinical notes and imaging of 29 patients with MSK were analysed. The severity of I
