Cross-sectional analysis of trends in meniscectomy and meniscus repair.

Knee arthroscopy to address meniscus tears is among the most common orthopedic procedures performed, and technical advances in the treatment of meniscus tears have affected the treatment options available to orthopedic surgeons. The purpose of this study was to perform a large cross-sectional analysis of orthopedic patients to investigate trends in arthroscopic meniscectomy and meniscus repair in the United States. Patients who underwent arthroscopic meniscectomy (Current Procedural Terminology codes 29881 and 29880) and arthroscopic meniscus repair (Current Procedural Terminology codes 29882

Meniscectomy and repair in the anterior cruciate ligament-deficient patient.

Meniscal injuries are frequently associated with acute injuries to the anterior cruciate ligament (ACL). With the passage of time, this frequency increases significantly. The management of the torn meniscus varies with the type of lesion and the patient's goals. When possible, meniscus repair combined with ACL reconstruction is recommended in young, athletically active patients. Repair may be accomplished by open or closed techniques. Because of significant risks associated with arthroscopic approaches, an outside-in type of repair has been devised. Overall, the clinical success approaches 90%

[Arthroscopic meniscectomy complicated by lesions to the popliteal artery and vein].

Endoscopic surgery can replace operative procedures. The evolution of arthroscopy leads to realize intra articular surgery more and more complicated. However, any surgical procedure involve some risks, and surgeons have to be prudent in use of new instruments or new technics. These need a radio surgical environment adapted to all surgery.

Popliteal artery and venous aneurysm as a complication of arthroscopic meniscectomy.

A 29-year-old man presented a 2-week history of right popliteal fossa and leg swelling following an arthroscopic meniscectomy. Roentgenographic evaluation showed an arterial and venous aneurysm at the popliteal level, with an associated arteriovenous fistula. Popliteal aneurysm in young men is very rare and generally related to open surgical techniques; incidence with arthroscopic techniques is extremely infrequent.

Two-portal arthroscopic subtotal meniscectomy for temporomandibular joint disc perforation.

Disc perforation represents an advanced stage of temporomandibular joint (TMJ) internal derangement and is traditionally managed with disc repair techniques or open discectomy when the disc is non-repairable. Although arthroscopic approaches are widely used for minimally invasive management of TMJ disorders, reproducible techniques for arthroscopic disc removal remain limited. This technical note describes the feasibility of a two-portal arthroscopic subtotal meniscectomy for non-repairable TMJ disc perforation. The procedure is performed using a standard posterolateral viewing portal and a si

Low early complication rates after arthroscopic meniscus repair and meniscectomy.

PURPOSE: To evaluate the 30-day complication rates after arthroscopic meniscus repair and meniscectomy using the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database, with subgroup analysis of patients aged > 40 years. METHODS: NSQIP registries between 2006 and 2019 were queried using Current Procedural Terminology codes to identify patients undergoing arthroscopic meniscus repair (CPT 29882, 29883) and meniscectomy (29880, 29881). The following 30-day complications were assessed: pulmonary embolism (PE), venous thromboembolism (VTE), surgical site i

[Complete popliteal artery rupture one month after arthroscopic partial meniscectomy].

A patient suffered a complete popliteal artery injury one month after a partial arthroscopic meniscectomy. The patient complained of progressive and severe distal pain. A complete lesion of the popliteal artery was found. Distal vascular flow was barely supported by two geniculate arteries. Vascular repair was performed using open vascular surgery. Although the popliteal neurovascular bundle is close to the posterior capsule of the knee and is obviously potentially vulnerable, injuries in arthroscopy of the knee are very rare. To avoid catastrophic consequences, the possibility of vascular inj

Heterotopic ossification within an arthroscopic portal after uneventful partial meniscectomy.

We report the case of a 44-year-old man who underwent a partial medial meniscectomy for a meniscal tear whose postoperative course was complicated by the development of heterotopic ossification (HO) within the medial arthroscopic portal. Following a routine initial procedure, the patient presented with ongoing pain and a palpable, painful lump around the previous medial arthroscopy portal. Plain radiographs and MRI were suggestive of a bony structure within the soft tissues. Histopathological examination at repeat arthroscopy confirmed osseous tissue consistent with HO. Recovery after the seco

Nine cases of osteonecrosis in elderly patients following arthroscopic meniscectomy.

We report nine cases of osteonecrosis of the knee after arthroscopic meniscectomy between 1992 and 1996. In five women and four men aged between 58 and 82 years (mean 69 years), magnetic resonance imaging (MRI) demonstrated a meniscal tear for which arthroscopic meniscectomy was performed. MRI was done between 3 days and 72 weeks after the onset of symptoms. Signs of osteonecrosis were not present on the initial MRI scan. Postoperatively, all patients experienced persistent knee pain and joint effusion. A repeat MRI scan 6-48 weeks after meniscectomy confirmed the diagnosis of osteonecrosis. I

[Rapid chondrolysis after arthroscopic external meniscectomy. Apropos of 4 cases].

The authors report 4 cases of rapid lateral femoro-tibial chondrolysis following arthroscopic lateral meniscectomy. All patients were young athletes. At the time of meniscectomy, the cartilage of the lateral compartment was normal. Only one meniscectomy was done with the Ho Yag laser. Clinical symptoms were always identical: pain and important swelling, persisting despite intra articular injections of corticosteroids. A second-look arthroscopy was performed after a mean delay of 6 months (5 to 8), showing numerous cartilaginous debris floating in the joint (like in a chondromatosis disease) an

[Partial meniscectomy via arthroscope in patients over 50 years old].

Between July 1983 and July 1986 115 patients, aged 50 years and older, suffering from 122 meniscal tears, were treated by arthroscopic surgery. Of these, 109 patients with 116 meniscectomy could be seen for a follow-up examination 2 years postoperatively. Patients of 50 years and older are known to need a long hospitalisation and a long rehabilitation period after an open meniscectomy. A quarter of all patients had a knee accident in their past as the cause of their complaints. Their main complaints are pain and swelling. As could be expected we found degenerative changes in 78% of the knees.

Knee Offloading by Patients During Walking and Running After Meniscectomy.

BACKGROUND: Changes in knee loading have been reported after meniscectomy. Knee loading has previously been assessed during jogging and treadmill running rather than overground running, which could give altered results. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate knee function during overground running and walking after meniscectomy. It was hypothesized that the affected limb would demonstrate higher external knee adduction moment, lower knee flexion moment (KFM), and lower knee rotation moment (KRM) compared with the contralateral limb and with healthy individuals. Concomitant Anatomic PCL and FCL Reconstructions With Partial Lateral Meniscectomy.

BACKGROUND: Fibular collateral ligament (FCL) injuries commonly present in a multiligament knee injury pattern. These injuries are associated with significant instability leading to altered tibiofemoral biomechanics and therefore require surgical intervention. Similarly, grade 3 posterior cruciate ligament (PCL) injuries may disrupt normal tibiofemoral and patellofemoral biomechanics and increase the risk of secondary osteoarthritis. Therefore, concomitant reconstruction of the FCL and PCL should be performed to decrease knee laxity and optimize functional outcomes. INDICATIONS: Early operati

Changing MRI after subchondroplasty with partial meniscectomy for knee osteoarthritis.

A 54-year old woman with primary osteoarthritis and a tibial bone marrow lesion underwent subchondroplasty with injectable calcium phosphate. Post-operatively, the patient's symptoms worsened, and she lost the ability to bear weight. Follow-up MRI revealed previously absent, diffuse STIR hyperintensity in the tibia extending far beyond the surgical site. Twelve months post-operatively, symptoms spontaneously resolved. As the prevalence of subchondroplasty grows it will be important to recognize potential complications. To the authors' best knowledge this is the first report of significantly wo

[Stage-based treatment by partial meniscectomy, meniscal refixation and transplantation].

Clinical and experimental studies have demonstrated that the meniscus is important for normal knee function. Loss of meniscus results in abnormal load transmission across the knee and may lead to degenerative joint disease. Preservation of meniscal tissue is therefore important. About 10 % of all meniscal tears are repairable. The most successful repairs occur in younger patients who have an acute, vertical tear in the vascular portion of the meniscus. Currently, arthroscopic meniscal repair procedures include the inside-out, the outside-in and the all-inside technique. Vertical suture techniq

Trabecular bone strain changes resulting from partial and complete meniscectomy.

Previous studies have documented how partial and complete meniscectomy affect articular contact pressure, but changes in load transfer through the complete osteochondral structure of the proximal tibia after partial and complete meniscectomy are not well known. The current study measured trabecular bone strain changes in the medial tibial plateau resulting from partial and complete medial meniscectomy. Midcoronal sections were prepared from knees from cadavers. High quality digital images, made from contact radiographs of loaded samples, were compared with digital images of unloaded samples us

Isolated arthroscopic partial meniscectomy. Functional radiographic evaluation at five years.

Fifty patients who underwent isolated arthroscopic partial meniscectomy with a minimum followup of 5 years were analyzed retrospectively. To analyze the factors associated with a satisfactory or an unsatisfactory clinical result, we looked at the patient's age, duration of symptoms, type of meniscal lesion, and articular cartilage abnormalities. The patients were graded with a functional knee score (Lysholm-Gillquist), and activity level before surgery and at followup was determined. The data also included radiographic evaluation of 29 of the 50 patients. Tibiofemoral alignment was measured, a
