Fresh Versus Frozen Meniscal Allograft Transplant: Revisit or Redundant? A Systematic Review.

BACKGROUND: Fresh-frozen allografts are the current standard in meniscal allograft transplant (MAT) surgery, due to their availability, ease of preservation, and affordability. However, fresh-frozen grafts are associated with several clinical challenges such as graft shrinkage and extrusion, among many others. PURPOSE: To present the current knowledge on the use of fresh meniscal allografts, presenting whether benefits associated with fresh grafts provide sufficient evidence to support their use in clinical practice. STUDY DESIGN: Systematic review; Level of evidence, 5. METHODS: A comprehe

Hypermobile lateral meniscus: A systematic review of current treatment options.

PURPOSE: The reliable data on the incidence of hypermobile lateral meniscus (HLM) and its clinical manifestations, diagnostic methods and therapeutic approaches are limited. This systematic study aimed to review available treatment options for HLM and the outcomes of each approach. METHODS: A systematic search was performed in four electronic databases (PubMed, EMBASE, Scopus, Web of Science) to identify studies in which arthroscopically confirmed cases of HLM were treated surgically or nonsurgically, and the required data comprising study characteristics, patient data, treatment approaches a

Arthroscopic Repair of Medial Meniscus Posterior Root Tear with Adjustable Titanium Plate Fixation.

Medial meniscus injury is one of the most prevalent knee disorders, with posterior root tears occurring in approximately 10% to 21% of affected individuals. A posterior root tear disrupts the meniscus's continuous annular structure, compromising its ability to absorb pressure and protect the knee's articular cartilage. If left untreated, this can result in increased stress on the cartilage, leading to conditions such as varus deformity and accelerated joint degeneration. Partial removal (resection) of the medial meniscus further exacerbates these issues, often leading to quicker joint deterior

In vitro method to quantify and visualize mechanical wear in human meniscus subjected to joint loading.

The mechanical wear and tear of soft connective tissue from repetitive joint loading is a primary factor in degenerative joint disease, and therefore methods are needed to accurately characterize wear in joint structures. Here, we evaluate the accuracy of using a structured light 3D optical scanning system and modeling software to quantify and visualize volume loss in whole human meniscus subjected to in vitro joint loading. Using 3D printed meniscus replicas with known wear volumes, we determined that this novel imaging method has a mean accuracy of approximately 13 mm3, corresponding to a me

Arthroscopic direct repair for a complete radial tear of the posterior root of the medial meniscus.

We report here on a new arthroscopic direct repair technique for a radial tear of the posterior root of the medial meniscus (PRMM) using a posterior trans-septal portal. Radial tears of the PRMM are commonly observed in the elderly population of Korea and Japan, and the life style of these people requires squatting and kneeling down in daily life. A radial tear of the PRMM results in the loss of hoop tension and this accelerates degenerative changes in the knee joint and causes early osteoarthritis. Several reports in the medical literature have focused on various repair techniques for these t

[Magnetic resonance imaging of the knee menisci. Normal aspects. Misleading images. Meniscus degeneration. Anatomic correlations].

The authors report the result of a study on 5 knees of fresh corpses explored with magnetic resonance imaging, including 1 examined before and after intraarticular contrast injection, and on 15 asymptomatic subjects examined with the same procedure. A very thorough study of the menisci and of their attachment, ie. The tibial insertion of the menisceal horns, the transverse ligament, and the meniscofemoral ligament, is possible with T1-weighted MR sequences. The T2-weighted sequences, either following intraarticular contrast injection or in cases of articular effusion, allow analyzing the capsu

Medial meniscus RAMP lesions: a novel magnetic resonance arthrography-based classification.

Meniscal ramp lesions (RAMP) are longitudinal tears at the meniscocapsular or meniscotibial attachments of the posterior horn of the medial meniscus, commonly associated with anterior cruciate ligament (ACL) tears. Their preoperative detection on standard MRI remains limited, especially for "hidden" variants. Magnetic resonance arthrography (MRA) may theoretically enhance visualization of the posteromedial compartment through capsular distension and improved tissue contrast; however, its diagnostic superiority remains to be validated. This pictorial review introduces a conceptual MRA-based fra

Arthroscopic Meniscectomy for the Degenerative Meniscus with Mild to Moderate Osteoarthritis: A Contemporary Analysis.

» Arthroscopic partial meniscectomy (APM) for the treatment of degenerative meniscal tears has been controversial, and quantification of its benefits has been limited by methodological problems and varying clinical definitions.» Among the problems with APM are patients crossing over from the treatment arm to which they have been randomized to another treatment and analyzed by the randomized treatment, whether they received that treatment or not.» Aggregated information suggests that a program of structured exercise is a reasonable initial treatment for degenerative meniscal tears with APM rese

All-inside repair is an effective treatment for medial meniscus posterior root tears: a systematic review and meta-analysis of biomechanical and clinical evidence.

BACKGROUND: Medial meniscus posterior root tears (MMPRTs) reproduce the biomechanics of subtotal meniscectomy. Repair is favored, but the role of all-inside repair (AR) remains unclear relative to transtibial pull-out (TP). This study aimed to review the biomechanical and clinical evidence on AR for MMPRTs. METHODS: A systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines and registered in the International Prospective Register of Systematic Reviews (PROSPERO). Lateral discoid meniscus in the pediatric knee: imaging features and tear patterns.

Lateral discoid meniscus is the most common meniscal congenital variant. A lateral discoid meniscus manifests with a spectrum of findings, including abnormalities in size and shape, disorganized collagen architecture, and/or hypermobility, predisposing the meniscus to early degeneration and tearing. The purpose of this narrative review is to discuss the anatomy of the lateral discoid meniscus and to highlight MRI findings that should be included in radiology reports to guide management.

Modified Mason-Allen vs Two Simple Stitch Fixation for Medial Meniscus Posterior Root Tears: A Systematic Review and Meta-analysis.

BACKGROUND: Various suture configurations are available for medial meniscus posterior root tear (MMPRT) repair. The modified Mason-Allen (MMA) technique has been proposed as a refixation technique for MMPRT instead of the conventional 2 simple stitches (TSS). This is in view of its superior biomechanical characteristics. PURPOSE: To perform a systematic review and meta-analysis to compare MMA and TSS configuration techniques for MMPRT repair and identify any differences between the 2 techniques in terms of clinical outcomes, medial meniscal extrusion (MME), and postoperative healing. MRI appearance of the different meniscal ramp lesion types, with clinical and arthroscopic correlation.

BACKGROUND: Meniscal ramp lesions have been defined as longitudinal vertical peripheral tears of the medial meniscus involving the posterior meniscocapsular ligament, meniscotibial ligament, and/or the red-red zone of the posterior horn. They are heavily associated with anterior cruciate ligament injuries, and because of their potentially important biomechanical role in knee stabilization, injuries to this region may require surgical repair. However, due to their location and lack of general knowledge regarding their different types and associated appearances on magnetic resonance imaging, ram

Does arthroscopic partial meniscectomy always cause arthritis?

The goal of arthroscopic partial meniscectomy is to preserve a stable rim of meniscal cartilage, removing only unstable, diseased tissue. As one of the most common orthopedic procedures performed in the United States, arthroscopic partial meniscectomy developed as an alternative to open total meniscectomy that leads to degenerative changes in the knee over time. The short-term results of this meniscal-sparing procedure are favorable. Evidence of long-term results is limited to outcome measures that are not standardized. However, the long-term trend of clinical outcomes is satisfactory, whereas

Meniscal debridement: current concepts.

The meniscus is a commonly injured structure in the knee. When intact, it serves multiple biomechanical functions that protect the underlying articular cartilage. When injured, it can cause pain and mechanical symptoms that restrict activity. Although meniscal tears can be effectively treated with arthroscopic debridement, there is concern that any amount of meniscectomy, no matter how small, is a risk factor for the development of long-term osteoarthritis. This article reviews current concepts in arthroscopic meniscal debridement, including clinical outcome studies, to understand which factor

Healing potential of experimental meniscal tears in the rabbit. Preliminary results.

This study assessed the healing potential of longitudinal or transverse incisions in the peripheral, central, and inner rim regions of rabbit medial menisci. Longitudinal and transverse incisions were made in 48 male New Zealand white rabbits. The incisions were either repaired with sutures or left open. The effect of joint motion on repair was also evaluated. In one group, longitudinal incisions were made in the peripheral, central, and inner rim regions of the meniscus. Some of these incisions were sutured, whereas others were not repaired. In another group, transverse incisions were made in

Structure-Function relationships of equine menisci.

Meniscal pathologies are among the most common injuries of the femorotibial joint in both human and equine patients. Pathological forces and ensuing injuries of the cranial horn of the equine medial meniscus are considered analogous to those observed in the human posterior medial horn. Biomechanical properties of human menisci are site- and depth- specific. However, the influence of equine meniscus topography and composition on its biomechanical properties is yet unknown. A better understanding of equine meniscus composition and biomechanics could advance not only veterinary therapies for meni

Arthroscopic repair for a flap tear of the posterior horn of the lateral meniscus adjacent to its tibial insertion.

A flap tear of the posterior horn of the lateral meniscus adjacent to its tibial insertion combined with acute rupture of the anterior cruciate ligament was successfully repaired arthroscopically by a combination of advancement of the tip of the flap into a drill hole created in the tibia and an inside-out stacked suture technique using Henning instrumentation in two cases. However, the long-term function of the repaired menisci, which was reduced several millimeters in circumferential length, is still unknown.

[Medial discoid meniscus in a pediatric patient. Case report and review of the literature].

Discoid meniscus is a congenital morphological variant of the meniscus, which tends to occur more frequently in its lateral form than in the medial form. This anomaly is characterized by central hypertrophy of the meniscus and a larger diameter than the normal meniscus, resulting in an abnormal shape and greater coverage of the tibial plateau. The clinical presentation of this condition varies depending on the stability of the meniscus. In pediatric patients, in particular, it is common to experience progressive and atraumatic symptoms, such as pain and limited mobility. Diagnosis is based on
