Diagnosis and Treatment of Discoid Meniscus.

There is a greater incidence of discoid meniscus in Asian countries than in Western countries, and bilateral discoid menisci are also common. The discoid meniscus may be a congenital anomaly, and genetics or family history may play a role in the development of discoid menisci. Because the histology of discoid meniscus is different from that of normal meniscus, it is prone to tearing. Individuals with a discoid meniscus can be asymptomatic or symptomatic. Asymptomatic discoid menisci do not require treatment. However, operative treatment is necessary if there are symptoms. Total meniscectomy le

Update on meniscus debridement and resection.

Arthroscopic treatment of meniscal pathology is among the most common procedures performed by orthopedic surgeons, and has evolved from open total meniscectomy to arthroscopic partial meniscectomy and repair. With knowledge of the importance of the meniscus to articular cartilage preservation, efforts to preserve the meniscus with repair have been increasing, but this is not appropriate for all patients. The decision of whether to perform meniscal debridement or repair must be made on an individual basis, and characteristics of both the patient and tear should be considered. Important patient

Medial dislocation of the medial meniscus.

We present the first reported case of symptomatic medial dislocation of the medial meniscus in a patient who had no previous history of trauma and who had an otherwise normal knee. The treatment of instability of the medial meniscus is controversial and studies have indicated that certain individuals without a firm meniscal bony insertion may be predisposed to meniscal dislocation. In our patient, the meniscal instability interfered with daily activities. Operative stabilisation by reconstruction of the meniscotibial ligaments cured the symptoms.

[The temporomandibular joint meniscus in MRI].

The continuing improvement of the MRI's equipment and software led us to a continuous adaptation of the exam technique. We use today a bilateral coil technique, 3 inches in diameter. Four slices are performed for each articulation (4 mm thick) in the true sagittal plane. A fast spin echo, balanced in proton density, for one minute, allows the study of 15 steps inside a full open/close cycle. Oblique coronal cuts are also performed parallel to the long axis of the condyle in a T1-weighted spin echo sequence in close and half-open positions. The analysis of 350 subjects allows to detail more pre

[Current developments in ultrasonography of the meniscus].

Sonography of the meniscus is a new diagnostic method, the instructive value and clinical relevance of which are subject to controversy. Sonographic assessment of the meniscus requires the use of focusing that can cope with the different anatomical conditions of the individual parts of the meniscus and can largely eliminate artifacts by means of wide sound-wave propagation. The dynamic examination technique allows the identification of both longitudinal and flap tears, which are relatively easy to diagnose, and of tear formations that are harder to visualize, such as horizontal and transversal

A simple method of meniscus repair.

Current understanding of knee meniscus pathology and the known consequences of meniscal loss dictate preservation of tissues wherever feasible. Surgical approaches to meniscal repair include open and combined arthroscopic-open or arthroscopic-percutaneous procedures. A variety of needles, guides, sutures, and retractors have been marketed. Over a 2 year period, we used these techniques in the repair of 19 medial and 5 lateral meniscus tears, with no known retears and no complications. We used an 18-gauge, 6-inch spinal needle and a strand of number one nonabsorbable suture.

On the etiology of congenital malformation of the meniscus.

In order to explain the occurrence of malformed meniscus in man, dissections were carried out in various vertebrates; they revealed that menisci shaped like a plate, disc, or ring are physiological components of the knee joint in several vertebrates. Microscopic examinations of tissue sections through the knee joint of human embryos revealed that the meniscus develops from an intermediate mesodermal blastema, which has the shape of a thick plate in the early embryonic period. The occurrence of a malformed meniscus in man may be explained by persistence of a very early embryonic stage and also

Is the meniscus of the knee joint a fibrocartilage?

A histological analysis of the structure of intact knee joint menisci was carried out in adult dogs. By means of specific histochemical methods for the connective tissue and cartilage, it was found that the meniscus as a whole does not have a unique structure. The anterior and posterior horns are populated by round chondroid cells encircled by abundant interstitial substance and branched wavy connective fibers; blood vessels are present. The outer third of the meniscus is constituted of cross bundles of connective fibers, fibrocytes and spindle-like areas of loose connective tissue with blood

Arthrography of the discoid lateral meniscus.

Twenty-seven cases of discoid lateral meniscus (DLM) were diagnosed during the course of 985 knee arthrograms; 19 had the classic discoid shape while eight were less large and believed to represent a forme fruste. Based on anatomic and arthrographic appearances, DLM was classified into six types. Criteria for the arthrographic diagnosis of intact and torn DLM are discussed. The wide variety of size and shape is believed to result from a combination of congenital and environmental factors. It is speculated that some may arise entirely secondary to trauma without congenital predisposition. Many

Review of the Development of Meniscus Centralization.

PURPOSE OF REVIEW: With an aging population, extending healthy life expectancy is a global challenge. Maintaining healthy knee joint function is one of the essential factors to preserve the ability to walk and extend healthy life expectancy. Meniscus centralization was introduced in 2012 as a procedure for meniscus extrusion, one of the causes of knee osteoarthritis (OA). Initially, it was performed only for lateral meniscus (LM) extrusion, and favorable 2-year results were reported in 2016. Gradually, basic studies supporting the effectiveness of meniscus centralization have been reported, an

Review of Meniscus Anatomy and Biomechanics.

PURPOSE OF REVIEW: Anatomic repair of meniscal pathology is critical for restoring native joint biomechanics and kinematics for patients who suffer from meniscal tears. The purpose of this review was to summarize the pertinent anatomy, biomechanics, and kinematics of the meniscus to guide surgeons during meniscal repair procedures. RECENT FINDINGS: Over the past decade, there has been a growing trend to save the meniscus whenever possible. The goal of repair should be to recreate native anatomy as close as possible to recapitulate normal mechanics. Studies describing the quantitative and qual

Update on imaging of the discoid meniscus.

Discoid menisci represent a range of morphological meniscal variants, most commonly involving the lateral meniscus. Clinical presentation ranges from an asymptomatic incidental finding to snapping, pain, swelling and reduced range of knee movement. Symptomatic presentation of discoid menisci is usually due to meniscal tears and instability resulting from abnormal meniscal morphology and ultrastructure, with absent peri-meniscal ligamentous and meniscocapsular attachments characteristic of the Wrisberg sub-type. This article reviews the current classification systems of discoid menisci, gross m

Is repeated repair of the meniscus worthwhile?

Meniscal injuries are one of the most commonly encountered problems by orthopaedic surgeons. It is well established that meniscectomy will result in increased joint surface contact forces and the predictable progression of arthritis. The management of meniscal tears has evolved and current evidence would suggest that every attempt should be made to repair meniscal tears when appropriate. The reported success rate of meniscal repair is encouraging, but relatively little is known about the outcome of repeated repair of a re-torn meniscus. This review presents an illustrative case of a recurrent

The role of the meniscus in osteoarthritis genesis.

The history of treating meniscal lesions has been characterized by firm belief in "radical" surgery, with serious long-term consequences for the individual and society. The menisci play a critical protective role for the knee joint through shock absorption and load distribution. Currently, the consensus in surgical treatment of meniscal tears is to preserve as much functional meniscal tissue as possible. Still, meniscal lesions are common, especially in the osteoarthritic knee. For health professionals, these lesions present a challenge in choosing the treatment that is best for the patient in

Currently, the consensus in surgical treatment of meniscal tears is to preserve as much functional meniscal tissue as possible. The health professional is challenged to choose the best treatment, both in the short- and in the long-term. A degenerative lesion in the middle-aged or older patient could suggest early-stage knee osteoarthritis and should be treated accordingly. Surgical resection of nonobstructive degenerate lesions may remove only the evidence of the disorder while the os

Clinical outcomes of artificial meniscus scaffolds for partial meniscus injury: a systematic review and meta-analysis.

BACKGROUND: Meniscal injuries, involving damage to the critical fibrocartilaginous structure of the knee joint, often necessitate surgical intervention, including meniscal allograft transplantation or the use of commercial implants. Despite advances in implant based therapies, there is no consensus regarding the comparative efficacy of collagen meniscus implants (CMI) versus polyurethane-based (PU-based) scaffolds. This review aimed to systematically evaluate and compare the clinical outcomes associated with these two implant types for partial meniscal repair. METHODS: A comprehensive systema
