BACKGROUND: Chronic mesenteric ischemia (CMI) is a life-threatening disorder, which is usually associated with stenosis or occlusion of celiac or mesenteric arteries.
PURPOSE: To review our experience and to assess short-term results of stent placement in stenotic mesenteric arteries.
MATERIAL AND METHODS: Primary stent placement was performed in 15 patients who had nausea, vomiting, postprandial pain, and weight loss due to steno-occlusive diseases of mesenteric arteries. After stenting, the patients were followed clinically and with Doppler ultrasound at 1, 6, and 12 months. Symptomatic pa

Nonocclusive mesenteric ischemia (NOMI) is a rare abdominal pathology caused by mucosal hypoperfusion without actual obstruction to the mesenteric arteries. We present a case of NOMI after a cardiopulmonary bypass operation. The patient was a 79-year-old woman with a history of hypertension and diabetes mellitus. A coronary bypass operation was performed with stable hemodynamic conditions, and continuous venovenous hemodialysis was performed on the second postoperative day because of renal insufficiency. After 24 h of hemodialysis, the hematocrit level increased from 29.1% to 36.1%. The patien

Diagnostic imaging in patients with suspected acute mesenteric ischemia is started with abdominal ultrasound including duplex sonography of the mesenteric vessels. Despite low sensitivity even in experienced hands, ultrasound is used because operative treatment can be initiated without further imaging if a positive diagnosis is made. Plain abdominal X-rays are usually unspecific in acute mesenteric ischemia and are mainly used to rule out differential diagnoses. Spiral CT (ideally using a multislice technique) can accurately demonstrate morphology of the arterial and venous mesenteric vessels,

Mesenteric vascular insufficiency should be suspected in patients with a history of abdominal pain, marked weight loss and significant atherosclerosis. Selective mesenteric angiography is essential and should be used early in the work-up studies of such patients. To ensure clinical correlation, there should be significant occlusion demonstrated in two of the three major mesenteric arteries. Four patients studied extensively for an intra-abdominal neoplasm had occlusive disease of the mesenteric artery demonstrated by arteriography. Elective surgical revascularization of the intestine successfu

Acute mesenteric ischemia from thromboembolic occlusion is a life-threatening emergency associated with a high mortality rate. Prompt diagnosis and intervention are vital to preserve viable bowel and prevent mortality. In the past decade, a shift has occurred toward minimally invasive alternatives such as endovascular therapies. We present a case of acute mesenteric ischemia from superior mesenteric artery thrombosis treated promptly with the Penumbra suction thrombectomy device (Penumbra Inc).

Acute mesenteric ischemia is a disease with a relatively low incidence (0,02-0,9% of acute surgical hospitalizations) but a very high mortality rate which can reach up to 100% without early revascularization. Given the above, prompt and accurate diagnosis and subsequent surgical intervention is the cornerstone of treatment and essential to reduce the high mortality rate. Early revascularization can reduce overall mortality by up to 50%. Treatment of acute mesenteric ischemia requires the cooperation of gastrointestinal surgeons, vascular surgeons and interventional radiologists. Thorough clini

Acute mesenteric ischemia is a rare life-threatening diagnostic and therapeutic emergency. Lack of clinical and biological specificity makes the diagnosis difficult. Imaging, particularly computed tomography can help confirm the diagnosis. An underlying cause is identified in about 30%-70% of cases and should always be sought. We report a case of a 51-year-old man with chronic alcoholic liver disease admitted to the emergency department for abdominal pain. Computed tomography showed mesenteric venous thrombosis with signs of small bowel ischemia and cirrhosis with portal hypertension. Through

Chronic mesenteric ischemia (CMI) is an uncommon, potentially underdiagnosed clinical condition. Although there is a high prevalence of mesenteric artery stenoses (MAS), an abundant collateral network in the mesenteric circulation mitigates occurrence of ischemia. The most common etiology of CMI is atherosclerosis. CMI is a clinical diagnosis, based upon typical and atypical symptoms and consistent anatomic findings. Typical symptoms of CMI are postprandial abdominal pain, unintended weight loss and food avoidance. The main modalities to diagnose MAS are duplex ultrasound, CT angiography or MR

Despite improved outcomes in acute aortic dissection in Japan, mesenteric ischemia remains a problem. This is largely due to difficulties in early diagnosis and treatment. To address this problem, this article will review several diagnostic modalities such as ultrasound and near-infrared spectroscopy, as well as surgical treatments. Ultrasound is useful for assessing mesenteric ischemia because it can be performed at bedside and it provides information on both morphology and perfusion in real time without exposure to radiation or contrast media. Malperfusion can be assessed at four sites betwe

BACKGROUND: Gastrointestinal blood flow may be compromised during and after vasopressor support. Endothelin expression may lead to microcirculatory dysfunction. The aim of this study was to analyze the effect of vasopressin and dobutamine after mesenteric ischemia on the gastrointestinal mucosal microcirculation, endothelin expression, and morphologic injury.
MATERIALS AND METHODS: Pigs were studied in four groups (six pigs in each group): 1, sham; 2-4 ischemia (1 h superior mesenteric artery occlusion with 30 min reperfusion and 30 min of vehicle [2], dobutamine [3], or vasopressin [4] admin

Mesenteric ischemia complicated by acute aortic dissection (AAD) is uncommon, but serious, as there is no established treatment strategy and it can progress rapidly to multi-organ failure. Diagnosing mesenteric ischemia before necrotic change is difficult, not only for primary care physicians, but even for gastrointestinal or cardiovascular surgeons as it can occur at any time during surgery. Thus, measures need to be in place at the bedside to enable us to obtain information on visceral perfusion. It is often difficult to decide which of laparotomy or aortic repair should be performed first,

Acute mesenteric ischemia is commonly seen in old patients. This study was undertaken to show that mesenteric ischemia might be seen in individuals under 40 years of age and that its diagnosis is challenging. Twenty-six patients with acute mesenteric ischemia under the age of 40 were studied. The main symptom on admission was abdominal pain. Symptom duration varied between 12 h and 5 days. The medical history of the patients revealed that 9 had no previous diseases. Other 17 had predisposing factors in the first evaluation. None of the patients had any history of narcotic or drug abuse. Ten pa

Chronic ischemic lesions of the splanchnic organs are often underestimated and underdiagnosed. The etiology, therapy and prognosis of acute mesenteric ischemia can be distinguished from chronic splanchnic ischemia. Progression of underlying atherosclerosis and the polymorbidity of the aging population results in an increased prevalence of chronic mesenteric ischemia, and occlusions of splanchnic arteries are diagnosed more frequently. Due to excellent collateralization, diffuse stenotic processes may remain asymptomatic for a long time. Computed tomography angiography with 3D reconstruction is

BACKGROUND: Nonocclusive mesenteric ischemia is a rare but serious disorder with a high mortality rate; causal factors include cardiac glycosides, digoxin, and digitoxin in addition to severe hypotension, decompensated heart failure, and septic shock.
CASE PRESENTATION: We present a patient with digitoxin intoxication who died from autopsy-confirmed ischemic gangrenous colitis.
CONCLUSION: Despite the frequent occurrence of intoxication with cardiac glycosides nonocclusive mesenteric ischemia is a rare event. However, it should always be included as a differential diagnosis of diffuse abdomi
