Rapid orthodontic treatment after the ridge-splitting technique--a combined surgical-orthodontic approach for implant site development: case report.

This article presents a clinical case of bilateral partial edentulism in the posterior mandible with severe horizontal and moderate vertical bone atrophy. A new technique using rapid orthodontics after ridge splitting is presented. The split-crest technique was carried out using piezosurgical instruments in the first molar and second premolar areas to widen the bone crest and open a channel for tooth movement. Immediately after, orthodontic appliances were used to move the first premolars distally and the second molars mesially into the surgical site. The rationale was to facilitate and accele

The effect of enhanced structure in the posterior segment of clear aligners during anterior retraction: a three-dimensional finite element and experimental model analysis.

BACKGROUND: Mesial tipping of posterior teeth occurs frequently during space closure with clear aligners (CAs). In this study, we proposed a new modification of CA by localized thickening of the aligner to form the enhanced structure and investigate its biomechanical effect during anterior retraction. METHODS: Two methods were employed in this study. First, a finite element (FE) model was constructed, which included alveolar bone, the first premolars extracted maxillary dentition, periodontal ligaments (PDL), attachments and aligners. The second method involved an experimental model-a measuri

Unlocking ectopically erupting permanent first molars using light wires.

BACKGROUND AND OVERVIEW: Ectopic eruption of permanent molars is one of the challenges that arise in the early mixed dentition period, particularly when the root of the primary second molar is resorbed due to mesial angulation of the impacted first molar. The authors introduce a simple and efficient method to unlock ectopically erupting first molars using a light wire. CASE DESCRIPTION: The authors describe the cases of 2 girls (8 and 7 years old) who sought treatment for locking of their maxillary and mandibular first molars, respectively. A 0.012-inch nickel titanium wire was compressed and

Modified distal shoe appliance--fabrication and clinical performance.

When the primary second molar is prematurely lost, mesial movement and migration of the permanent first molar often occurs. This is one of the most difficult problems of the developing dentition confronted by pediatric dentists. Use of a space maintainer that will guide the permanent first molar into its normal position is indicated. In cases with bilateral premature loss of primary molars, the conventional design of distal shoe poses a variety of problems and, therefore, necessitates a customized design for the eruption guidance of permanent first molars. The purpose of this case report is to

Wisdom tooth eruption secondary to localized lower molar mesialization in patients with aplastic lower second premolars.

OBJECTIVE: After localized space closure in the mandible it is desirable to preserve lower wisdom teeth as natural replacements for missing second premolars and as antagonists for upper second molars. The aim of this study was to determine whether this treatment method can obviate the need for prosthodontic restorations, implants, and third molar osteotomy. Furthermore the time point of possible third molar eruption was to be determined and whether further orthodontic interventions were required to achieve their alignment. PATIENTS AND METHODS: The test group comprised 34 patients who exhibit

Orthodontic rehabilitation for anterior teeth lost due to trauma with crowding malocclusion.

Herein, we describe orthodontic management for an adolescent girl with traumatic loss of the upper central incisors and maxillary protrusion. The central incisors were immediately replaced and fixed with application of a semi-rigid splint for 12 days, then endodontically treated. Severe root resorption and degeneration of periodontal tissue were noted after 4 years and the teeth were extracted, while the patient had also developed maxillary protrusion with severe crowding in the lower arch. The treatment objectives were to close the spaces by mesial movement of the buccal segment in the upper

Effect of power arm length and attachment configuration on maxillary molar mesialization with clear aligners: a finite element analysis.

BACKGROUND: Maxillary molar mesialization with clear aligners is challenging because achieving controlled root movement without excessive mesial tipping remains difficult. Although power arms can modify the force system, the combined effects of power-arm length and attachment configuration on molar displacement and tipping remain insufficiently characterized. This study evaluated the effects of 6-, 8-, and 10-mm power arms combined with two attachment configurations during maxillary first molar mesialization using three-dimensional finite element analysis. METHODS: The attachment configuratio

Control of Orthodontic Tooth Movement by Nitric Oxide Releasing Nanoparticles in Sprague-Dawley Rats.

UNLABELLED: Orthodontic treatment commonly requires the need to prevent movement of some teeth while maximizing movement of other teeth. This study aimed to investigate the influence of locally injected nitric oxide (NO) releasing nanoparticles on orthodontic tooth movement in rats. MATERIALS AND METHODS: Experimental tooth movement was achieved with nickel-titanium alloy springs ligated between the maxillary first molar and ipsilateral incisor. 2.2 mg/kg of silica nanoparticles containing S-nitrosothiol groups were injected into the mucosa just mesial to 1st molar teeth immediately prior to

The not-so-harmless maxillary primary first molar extraction.

BACKGROUND: Premature loss of primary molars has been associated with space loss and eruptive difficulties, especially when the loss occurs to the primary second molars and when it occurs early. This has not been thought to be the case for primary first molars. METHODS: The author revisited 13 cases from an earlier study on the effects of premature loss of maxillary primary molars. These longitudinal cases were scrutinized, using serial panoramic radiographs, to explain the irregular response in terms of dental migration. The author presents two case reports. RESULTS: In the earlier study, t

Moving an incisor across the midline: a treatment alternative in an adolescent patient.

A 13-year-old sought treatment for a severely compromised maxillary left central incisor and an impacted fully developed left canine. Extraction of both teeth became necessary. As the key component of the revised comprehensive treatment plan, the right maxillary central incisor was moved into the position of the left central incisor. All other maxillary teeth were moved mesially to close any gaps. Active orthodontic treatment was completed after 34 months. Frenectomy, minor periodontal surgeries, and bonded lingual retainers were used to improve aesthetics and stabilize the tooth positions. Th

Use of an intraoral scanner to evaluate orthodontic treatment for dental malocclusion in a dog.

OBJECTIVE: In human dental practice, intraoral scanners (IOSs) are increasingly used to evaluate the oral cavity. However, there have been no reports concerning evaluation of the canine oral cavity using IOSs in veterinary dentistry. In this case, optical impressions obtained with an IOS were used for 3-D evaluation of the orthodontic treatment outcome. Additionally, few reports have addressed the optimal orthodontic force for canine teeth. Therefore, this case explored whether sufficient tooth movement into the extraction site could be achieved with a lighter orthodontic force than previously

The Effects of Rapid Periodontal Distraction on Tooth Vitality and Bone Quality/Quantity: A Split-Mouth Experimental Design.

OBJECTIVES: Periodontal distraction (PD) osteogenesis is a minimally invasive technique capable of rapidly moving uniradicular teeth, thereby shortening orthodontic treatment time. This study aimed to determine whether: (1) PD-induced tooth movement affects tooth vitality and (2) the regenerated bone meets restorative standards for implant placement. MATERIALS AND METHODS: In six dogs, the maxillary 2nd premolars on the experimental side were extracted, the mesial inter-radicular bone was removed to within 1 mm of the 1st premolar, and an intraoral distraction device was bonded. The contralat

Nonsurgical rapid maxillary alveolar expansion in adults: a clinical evaluation.

Palatal expansion in adults has traditionally been performed on a very limited basis. The expansion has been thought to be limited in scope and stability and to be associated with unacceptable complications. Instead, surgically assisted rapid maxillary expansion (SA-RME) has been advocated. Five adults with transverse arch deficiency are presented to illustrate the feasibility of nonsurgical expansion using the Haas appliance. Transmolar expansions of 3.9 to 7.5 mm, sufficient to correct the malocclusions, were achieved. Limiting the rate of appliance activation is thought to be important to a

Response to activator treatment in Class II malocclusions.

A clinical study was designed to disclose the effects of activator treatment in the correction of Class II malocclusions. The rationale for the use of the activator appliance was based on the premise that correction of distocclusion can be achieved by inhibition of forward growth of the maxilla, inhibition of mesial migration of maxillary teeth, inhibition of maxillary alveolar height increase and extrusion of mandibular molars, increased growth of the mandible, anterior relocation of the glenoid fossa, mesial movement of mandibular teeth, and combinations of these effects. The appliance, as d
