Hallux valgus (HV) is not a simple two-dimensional deformity but is instead a three-dimensional deformity that is closely linked to sesamoid position and first metatarsal (MT) pronation. HV may or may not be accompanied by sesamoid subluxation and/or first MT head pronation. Each of these scenarios should be assessed using weighted computed tomography scan preoperatively, and the necessary corrections should be performed accordingly.

PURPOSE: Juvenile hallux valgus deformity (JHVD) is rare but may be associated with symptoms or deformities that require surgical treatment. Literature recommends waiting to perform surgical treatment until maturity. However, if conservative treatment is not sufficient and the children's psychological or physical suffering is particularly severe, earlier surgical treatment should be considered. The aim of this study was to evaluate the safety and efficiency of temporary screw epiphyseodesis of the lateral epiphyseal plate of the first ray metatarsal as a new treatment option for JHVD during gr

BACKGROUND: Two concepts have been proposed to explain the etiology of metatarsalgia in hallux valgus patients: First, as the magnitude of hallux valgus increases, there is a mechanical overload of the lesser metatarsals. Second, increased relative lesser metatarsal length is a factor in the development of metatarsalgia. However, there is no current evidence that these structural factors lead to primary metatarsalgia. The purpose of the study was to evaluate the factors associated with metatarsalgia in hallux valgus patients.
METHODS: A cross-sectional study of 121 consecutive adult patients

A combined bony and soft tissue procedure is reported as a treatment of hallux valgus. Satisfactory subjective and objective results were obtained at an average follow-up of 49.6 months. There was a 21% incidence of avascular necrosis of the first metatarsal head, four out of the 38 patients (10.5%) having developed a varus deformity at follow-up.

In cases of hallux valgus tendon transfer combined with wedge osteotomy of metatarsus I yields good results. Considering the experience of 36 operated feet in 25 patients the operation can be recommended especially for young patients. An exact technique helps to avoid complications such as hallux varus and delayed ossification of the osteotomy.

We compared the results of proximal chevron osteotomy and double metatarsal osteotomy for hallux valgus with an increased distal metatarsal articular angle (DMAA). From October 2008 to December 2012, first metatarsal osteotomies were performed in 64 patients (69 feet) with symptomatic hallux valgus associated with an increased DMAA. Proximal chevron with Akin osteotomy and lateral soft tissue release was performed in 46 feet (PCO group); double metatarsal osteotomy and Akin osteotomy without lateral soft tissue release was performed in 23 feet (DMO group). Clinical assessments were performed u

PURPOSE OF THE STUDY Accurate radiographic measurements are crucial in treating hallux valgus (HV). This three-dimensional deformity should not be evaluated from one joint on one plane. However, in practice, surgeons measure the deformity only on transverse dorsoplantar radiographs. We determined the amount of error associated with positioning the foot incorrectly on radiographs. MATERIAL AND METHODS To simulate incorrect positions of the foot in radiographic evaluation, we designed an angled device that can move in transverse and frontal plane. In four patients with symptomatic HV, we took we

A new operation for hallux valgus consists of a horizontally directed "V" displacement osteotomy done in the head of the first metatarsal. This procedure corrects most of the primus varus, the valgus tilt of the articular surface of the first metatarsal and, if present, the axial rotation of the great toe. The osteotomy is combined with a release and balancing of soft-tissue structures on both sides of the joint. No casting and usually no internal fixation is necessary. Ambulation is generally recommended on the third postoperative day. The operative techniques, indications for surgery, clinic

Two procedures of hallux valgus correction in young patients were compared by long-term results: soft-tissue realignment (McBride) and Mitchell osteotomy. From 1970 to 1990, 33 hallux valgus deformities of 17 patients were corrected with one of these procedures at the Orthopaedic Department of the University of Mainz. Patients' age at operation ranged from 9 to 20 years and average follow-up was 16 years later. Reevaluation consisted of subjective criteria, such as pain, shoe-fitting, functional disability and cosmetics and objective parameters (hallux-valgus and intermetatarsal angle, osteoar

BACKGROUND AND STUDY AIMS: Most foot surgeons recognize the difficulties to define each patient's hallux valgus (HV) deformity and to select the most appropriate surgical treatment to achieve the best long term outcome. The goal of this study was to analyze radiologic outcomes after distal chevron metatarsal osteotomy and to identify specific preoperative radiological parameters correlating with radiological recurrence.
MATERIALS AND METHODS: One hundred twenty patients (134 feet) in patients with symptomatic moderate or severe HV deformity who underwent distal chevron metatarsal osteotomy at

BACKGROUND: There is increasing interest in the role of pronation as part of the deformity correction in hallux valgus (HV) especially with the advent of weightbearing computed tomography (WBCT) and percutaneous surgical techniques. This study aimed to assess the coronal rotation of the first metatarsal before and after percutaneous distal transverse osteotomy (Metaphyseal Extra-Articular Transverse and Akin Osteotomy [META]) using WBCT and to correlate these findings with clinical outcome scores.
METHODS: A retrospective analysis of prospectively collected data from a single center of patien

INTRODUCTION: Iatrogenic shortening and elevation of the first metatarsal (MT1) is a common complication of hallux valgus surgery, inducing metatarsalgia underneath the lesser rays, a reason for patient dissatisfaction. For resolving this problem, different types of revision surgery are described, of which lengthening MT1 by distraction osteogenesis is underreported and therefore undervalued.
MATERIALS AND METHODS: We present three cases with iatrogenic shortening after hallux valgus surgery treated by distraction osteogenesis of MT1 using a custom-made frame, made of Ilizarov equipment. To e

BACKGROUND: Modified Bösch osteotomy, or distal linear metatarsal osteotomy (DLMO), is one of the minimally invasive correctional operations for hallux valgus deformity. Although the clinical and radiographic results of DLMO have been previously shown, the relationship between clinical outcomes using a validated patient-reported outcome measure and radiographic corrections have yet to be evaluated.
METHODS: A total of 70 patients (97 feet) treated at our hospital were included in the study. The Self-Administered Foot Evaluation Questionnaire (SAFE-Q) and radiographic data were evaluated at a
