Metatarsus Adductus Association With Fifth Metatarsal Fractures: Radiographic Analysis.

BACKGROUND: Metatarsus adductus is a congenital forefoot deformity associated with pathologies including proximal fifth metatarsal fractures. The aim of this study is to assess the presence of metatarsus adductus and alignment in patients with proximal fifth metatarsal fractures. We hypothesize that there is an increased incidence of metatarsus adductus in patients with proximal fifth metatarsal fractures. METHODS: A retrospective analysis was performed of patients who were treated for fifth metatarsal fractures from 2022 to 2024. Radiographic measurements including metatarsus adductus angle

Metatarsus adductus: development of a non-surgical treatment pathway.

Metatarsus adductus (MA) occurs in one to two cases per 1000 births and is the most common congenital foot deformity in newborns. The appearance is that of a curved or adducted forefoot with a normal hindfoot. A systematic literature review was conducted to answer the following question: For a child who presents with MA, what is the most evidence-based conservative treatment option? Thirteen articles were reviewed using the National Health and Medical Research Council levels of evidence and guidelines for clinical practice. Conservative treatment options reported on included the following: no

Radiographic analysis of metatarsus primus elevatus and hallux rigidus.

BACKGROUND: Controversy exists about the role of metatarsus primus elevatus (MPE) in the presence of hallux rigidus. Previous studies could neither confirm nor reject a causative relationship. Measurement of the true elevation of the first metatarsal according to current techniques lack either precision or accuracy or both. The purpose of this study was to assess MPE by means of a new radiographic measurement method and to analyze how the MPE-values differed among hallux rigidus, hallux valgus and control groups. MATERIALS AND METHODS: A retrospective study was performed of standing AP and la

Adult hallux valgus with metatarsus adductus: a case report.

It is difficult to surgically treat a hallux valgus deformity with significant metatarsus adductus, because the space between the first and second metatarsals is too narrow to correct the metatarsus primus varus with a first metatarsal osteotomy. A 55-year-old woman had severe hallux valgus with significant adduction of the second and third metatarsals. A distal soft tissue procedure and a proximal crescent-shaped osteotomy of the first metatarsal combined with corrective osteotomies of the second and third metatarsals were done. The patient's symptoms disappeared, and hallux valgus and adduct

Conservative Treatment for Metatarsus Adductus, A Systematic Review of Literature.

There are various treatment approaches for the subjects with metatarsus adductus, including nonsurgical and surgical treatment. Nonsurgical treatments such as serial casting, modified shoes (Bebax shoe, Ipos antiadductus shoe) and orthoses (Wheaton brace, counter rotation system splint, Denis Brown bar, and Fillauer bar) are widely used in this regard. The main question posted here is which orthoses are more effective in correcting metatarsus adductus. Therefore, the aim of this review was to determine the efficiency of various nonsurgical treatments used for metatarsus adductus. MEDLINE/PubMe

The Windswept Foot: Dealing with Metatarsus Adductus and Toe Valgus.

The windswept foot remains a reconstructive challenge. The hallux valgus associated with the medially displaced lesser metatarsal heads is hard to correct. Either the lesser metatarsal heads need to be displaced laterally or the deformity accepted. With the deformity, all the toes tend to be aligned into valgus with the position of the flexor and extensor tendons. Several treatment alternatives exist and may require a combination of open and percutaneous surgery. The authors think that, in severe metatarsus adductus, proximal correction of the first, second, and third metatarsals is required.

Reliability of metatarsus adductus angle and correlation with hallux valgus.

BACKGROUND: Metatarsus adductus is a common congenital foot deformity. Variable prevalence values were reported using different techniques in different populations. Numerous radiological measurements have been proposed to assess this deformity with a paucity of studies reporting the reliability of these methods. The metatarsus adductus angle was shown to correlate with the severity of hallux abductovalgus in normal feet and preselected populations of juvenile hallux valgus. MATERIALS AND METHODS: Weight bearing dorsoplantar radiographs of 150 feet were examined for 5 angles commonly used in a

The primary deformity in hallux valgus and metatarsus primus varus.

Analysis of radiographs from patients with hallux valgus showed that following surgical correction there were statistically significant reductions in the hallux valgus, intermetatarsal, and metatarsus primus varus angles. Normal values for these angles were established from a control group of asymptomatic feet and from previously published reviews. If the metatarsus primus varus deformity returns to normal following correction of the hallux valgus by a procedure that does not directly realign the metatarsal, the deformity must be secondary to hallux valgus. This is of importance in planning su

[METATARSUS ADDUCTUS - A COMMON DEFORMITY - WHAT TO DO AND WHEN?].

The most common foot deformity in newborns is the forefoot adduction deformity (FAD), where the hindfoot foot is in a normal position. The diagnosis for this problem is mainly based on a physical examination. The use of imaging methods has been described, but no advantage was shown with their utilization in determining the diagnosis and guiding treatment. Several classification systems have been proposed to characterize the degree of severity. The classifications are based on the degree of deviation and the flexibility of the foot. Early diagnosis and early treatment, if necessary, are extreme

[Lameness and swelling of the metatarsus in a Saluki - Could it be an arteriovenous fistula?].

A 12-year-old Saluki was presented with acute lameness and plantar swelling of the right metatarsus following an episode of free running. Radiographs showed soft tissue swelling only, the involved bone and joint structures were unremarkable. The lesion recurred several months later. Ultrasonographic imaging of the lesion revealed a well-vascularized cavernous structure. Subsequent surgical removal and histopathological examination of the structure raised the initial suspicion of an arteriovenous fistula, which was then confirmed histologically following a second surgical removal of another rec

A reappraisal of the relationship between metatarsus adductus and hallux valgus.

BACKGROUND: A causal link between the metatarsus adductus and hallux valgus is not clear. The aim of this study was to investigate the configurations of the metatarsus adductus deformity by radiological measurements and reappraise the relationship between hallux valgus and metatarsus adductus. METHODS: The first step was evaluation of the relationship between metatarsus adductus and hallux valgus on 143 dorsoplantar weight-bearing radiographs diagnosed as hallux valgus which was also known as bunions. Measurements including the hallux valgus angle (HVA), the intermetatarsal angle (IMA), the K

Below-knee plaster cast for the treatment of metatarsus adductus.

Sixty-five infants with moderate (37 feet) or severe (48 feet) inflexible metatarsus adductus were treated with a below-knee plaster cast. The deformity was corrected in 6-8 weeks in all cases. At the 2- to 6-year follow-up (mean, 4 years), the correction was maintained in all children who had had a moderate deformity; of the 44 feet with severe deformity that were available for examination, six now had a moderate deformity, and one a severe deformity. All forefeet were flexible, and further treatment was unnecessary. Our results are similar to those for above-knee plaster casts. We conclude t

Role of metatarsus primus elevatus in the pathogenesis of hallux rigidus.

Metatarsus primus elevatus has been suggested as a primary causative factor in the pathogenesis of hallux rigidus. The purpose of this investigation was to define the role of elevation of the first ray in the pathogenesis of hallux rigidus by comparing patients with known hallux rigidus with a control population. We reviewed 264 lateral weightbearing radiographs from 81 patients with hallux rigidus, 50 asymptomatic volunteers, and 64 patients diagnosed with isolated Morton's neuroma. Results revealed that the mean values for elevation of the first ray in patients with mild or moderate hallux r

Observer variability in the radiographic measurement and classification of metatarsus adductus.

The classification system of Berg was evaluated using four observers and the radiographs of 42 feet from patients with metatarsus adductus. Interobserver disagreement in diagnosis was 36%. Intraobserver inconsistency averaged 26%. The error range for the lateral and anteroposterior talocalcaneal angle measurement was 13.6 and 15.1 degrees intraobserver and 19.8 and 25.2 degrees interobserver, respectively. There was no correlation between classification and the length of time required for cast correction. The irregularity of hindfoot ossification centers makes measurements inconsistent and ser
